| 07/02/2016 | Salisbury Station Platform 3 – ISS Labour trapped their right foot under the track after the jack was dropped. It had been raised to fit plates. |
| 07/02/2016 | White Hart Lane Level Crossing – Member of public trapped within the barriers and the Crossing Clear pressed for 2O41. Signal replaced, barriers raised. |
| 10/02/2016 | Byfleet and New Haw Station – A member of PWay staff was carrying stressing kit down the station stairs and tore muscles in his back due to the weight of the equipment. |
