Incidents this week –

12/06/2016 New Malden ES for item 37 (Berrylands – New Malden), reported that the while moving a switch rail in the possession the jib of a crane struck the canopy on the Up Slow platform.

The crane lost all hydraulic fluid having split the hose.

12/06/2016 Petersfield Terminations were taking place due to engineering works and the Signaller overlooked that they had not normalised PF9 having been distracted by other operational problems in the box.

Due to the points being in reverse, the Signaller was unable to set PF240 or PF242 to a proceed aspect, so passed them at danger (red).

The result was a point run through i.e, the passage of the train damaged them due to not being set correctly.

13/06/2016 Basingstoke A member of Basingstoke S&T accidently caught his right hand little and ring finger in a door. His little finger began to bleed and his ring finger was bruised.

He continued with his duty and did not require medical attention.

15/06/16 Ashurst A ballast profiling attachment on an Road Rail Vehicle led to approximately 240 yards of consecutive e-plus pandrol clips to be removed from the low rail, field side of the Down Road BML2 between 84m 10c and 84m 21ch

The line was re-opened in error with a risk of derailment.

15/06/2016 Haslemere A Signal Works Operative was leaving the track access car park compound and had not realised once opening the gates, that they had begun to close again.

The vehicle collided with the gate on the passenger side sliding door and as a result, the window for the sliding door was smashed and the side panel damaged. Nobody was injured as a result of this accident.

16/06/2016 Totton A member of Eastleigh Works Delivery tripped over a small spike sticking out of the ground when moving equipment and fell to the ground injuring his right wrist causing a light sprain. No medical attention was required.
17/06/2016  Ashtead (Surrey) 2E76 1617 London Victoria to Horsham passed W479 signal at danger without authority (CAT A SPAD).   The Driver had acknowledged the AWS and slowed down to shout at some children on the foot crossing and then sped up forgetting the next signal (W479) was at danger.
