Pirbright Junction / Fleet
A Contractor from Morson Vital placed detonator protection onto the open Down Slow Line in error for WON 13 Item 49.
It had been noticed by a Strapman who advised the contractor and the protection was cleared.
The reason for the error was due to contractor having accessed the track from the wrong side, they had become confused which line was which.

Wimbledon Depot Wimbledon Station Exit 
A member of Wimbledon PWAY bruised their left hand when  getting into a van via the sliding door whilst holding onto the door frame.

When the passenger door was closed by another member of staff, it trapped the persons hand.
He applied ice at the time of the incident did not require a hospital visit.

5R00 reported a near miss with 2 members of McGinley P-way staff.

The investigation is ongoing to understand why they were in the position they were whilst carrying out strapping duties for T3 item 21.

Berrylands Station – Byfleet Junction  
A Ganymead contractor applied earthing straps for work site B (item 57) at 19m 36ch rather than 19m 50ch.

The ES advised that the strapman had been to the site previously and had assumed the straps were to be placed at the same location.

Romsey
Member of Works Delivery PWay had their little finger trapped within the left switch blade of 588 points.
The team had been changing stretcher bars when one of them barred the switch open to remove a bolt.
The injured party attended A&E after the pain had been keeping him awake.
The x-ray showed a crush to the tip of his finger which was also extremely swollen.
Medication was prescribed and he was told to elevate his finger and rest.

Axminster
At 11:04 the Basingstoke WoE Signaller granted a line blockage of the Up and Down Exeter lines to the Yeovil Junction MOM to complete an Insulated Rail Joint inspection for a fault picked up by the Measurement Train.
SE4790 signal button was thought to have been cancelled by the Signaller (putting the signal to red) so he granted the line blockage without checking the aspect indication for the signal.
At 11:14 the Signaller realised that 1L40 had entered the limits and attempted to call the Driver, but this call was cancelled as the MOM had called in from the SPT for SE4788 signal to report the train coming into the limits and was sounding the horn.
The Yeovil MOM was clear and safe of the line and no near miss was reported.
