Message from Ivan Kimble (Area Director)

There seems to be trend developing of staff not recording safety hour conversations by the deadline. As result, this week’s safety hour attendance has dropped to 49% across this route. This is well below target and is unacceptable and doesn’t show all the good work you are doing with your teams. You shouldn’t need chasing via call each week to record this as it should be reported each week (by you or your deputy if you are away).

Please can you ensure that you all record your safety hour conversation either via email or app by Friday 12:30pm every week.

3. Near miss at Leatherhead.

Attached is audio taken from my ‘Go, Look, See’, which demonstrates the massive difference between a lookout warning with and without machinery running, even at low revs.

From what we understand –

  •  2 men were involved in a near miss with a passenger train(1D31) at approximately 13:30, whilst retrieving an impact wrench from the 4 foot.
  • The team involved had been fitting fishplates and clamps to a rail defect on the down BTH3, between Leatherhead and Boxhill.
  • The Lookout gave an audible warning on his horn, but this may have been initially missed, due to the Impact Wrench running.
  • There was not a Touch-Lookout with the team.
  • It is believed that the Safe System of Work had been tested prior to the commencement of work, but not tested with the Impact Wrench running.
  • The COSS believes he reached the position of safety with approximately 6 seconds to spare.
  • The COSS called in the near miss on himself.

Statements were taken from all members of the team. This incident is under investigation
