2P62 Platforming irregularity A 5 carriage train was signalled on top off an 8 carriage train leaving a coach not being accommodated. No doors were released and a shunt to rectify the situation was arranged. |

Crossing misuse irregularity The Signaller working Feltham Panel 4 lowered the barriers and pressed crossing clear with a person within the crossing. F301 signal also cleared to a proceed aspect. The Signaller realised the error, but saw the person leave the crossing by climbing over the barrier. |
