Staff Accident A PWay Team Leader was walking along the cess when some loose ballast gave way twisting his left knee. An ice pack was used to reduce any swelling and the Golden Hour was applied. This incident did not result in any lost time. |

Derailment DB Cargo reported that a Coalfish wagon had become derailed between 4 and 6 Road over a set of hand points whilst carrying out a shunt move. Upon examination by Network Rail PWay (who patrol, but are not responsible to maintain), they found that there had been 3 to 4 sleepers replaced consecutively after a previous derailment, but not packed.There was also evidence of soil / sand being used and not ballast.The incident was subject to a DB Cargo investigation and Network Rail patrolling responsibilities were visited. |

Staff Accident A member of Feltham S&T was driving along a road in Sunbury when a the car in front braked to turn into a driveway. The S&T Driver went to brake themselves, but their foot slipped off the peddle resulting in a low speed collision between 5 to 10mph. There were no injuries sustained to either party and no apparent damage to the vehicles. The Golden Hour was applied and there was no lost time. |

Late Acknowledgment of train horn 1L37 reported that a track worker did not acknowledge the train horn until the 3rd time of sounding it. The Driver did not declare the incident as a Near Miss. |
| 09/06/2017 White Hart Lane Level Crossing |
Level Crossing Irregularity The Signaller on Wimbledon Panel 5 lowered the crossing and cleared an Up line signal for train service 2U22 to proceed. Upon doing so, they had realised that they had trapped a member of the public within the crossing next to the Up side Facing barrier and corrected their error by making an urgent call on the GSMR to the driver and replacing the signal to danger (red). Once the signal timed out, the barriers were raised and the member of public released. This incident is subject to further investigation. |

Staff Accident Upon carrying out de-vegetation work at the Electrical Control room, a Ground Control contractor noticed 7 ticks on his body. Having been trained to do so, the ticks were removed safely. Ground Control followed the incident up and there was no lost time. |

Possession Irregularity – Item 115 Whilst travelling on a passenger train at approximately 14:54hrs, a member of Feltham PWAY noticed that the protection for the T3 possession may not have been applied correctly in the Hounslow / Whitton Junction area. Upon the Operations Delivery Manager (ODM) attending site, it was identified that an irregularity had in fact occurred and that the detonator protection was placed 7 chains out of position and beyond the Work Site marker board. The protection had been like this since approximately 01:16hrs.The ODM corrected the error made and arranged a follow up investigation into the incident which had involved McGinley contract staff.
