Staff accident
Whilst assisting with the recovery of swans from the line, a Mobile Operations Manager received scratches on his arms caused by a swans claws.
This was the MOMs first incident involving swans and had not been wearing long sleeves due to the hot weather.
Golden Hour applied and there was no lost time. |

Staff Accident
A member of welding staff struck their left thumb when using a small hammer to place a wedge in the rail. The thumb became blistered and required draining at hospital.
The hospital prescribed a course of antibiotics.
The Golden Hour was applied and no there was no lost time. |

Staff Accident
A Works Delivery Operative crushed his right hand and he sustained a deep laceration after an Iron Man fell onto it whilst loading it onto a vehicle.
He attended hospital and was detained whilst the injury was cleaned, x-rayed and stitched.
There were no broken bones.
The Golden Hour was applied and lost time was anticipated. |

A Weld Inspector trapped his left thumb in a van door.
The injury became sore and he was advised to seek medical assistance at A&E if it became any worse.
The Golden Hour was applied and this became a non-lost time injury. |

Possession Irregularity Item 4 (Waterloo-Queenstown Road).
Upon being authorised to do so, engineering train 6Y81 made its way from a Worksite to exit the possession.
Upon arriving at the exit protection, the train exploded the first detonator having not seen it through lighting glare.
There were no injuries.
This incident is subject to further investigation. |

Staff Accident – Nash Contractor
A Nash Contractor was carrying out vegetation management and received a rash which was believed to be connected to brushing against an Oak Processionary moth (Thaumetopoea processionea).
The correct PPE been worn.
The Injured Party took anti histamines and arranged to see a doctor.
The Golden Hour once the injury appeared, was carried out.
The injured party was able to resume duty.
Note:
The oak processionary is a moth whose caterpillars can be found in oak forests. They may pose a human irritant because of their poisonous setae, which may cause skin irritation and asthma. |

Staff Accident (Electric shock)
Whilst carrying out maintenance on and a dead test on W633A points heating, an E&P member of staff received an electric shock to the arm.
He then started to sweat and his throat became dry, but after 10 minutes it subsided.
He attended hospital as a precaution.
The equipment had been isolated prior to the work taking place and investigations are ongoing to establish why the equipment was still live.
The Golden Hour was applied and no lost time was anticipated. |

Staff Accident
A member of PWay was driving a work van on the down gradient at Old Farm Junction when a blue Nissan Note pulled out from the left into the path of his vehicle.
He was unable to stop and the driver’s side of the van collided with rear of car.
During the collision, the driver hit his right knee on the dashboard which caused considerable pain.
Once details were exchanged, the van was driven back to the depot.
The Golden Hour was applied and there was no lost time injury. |

A Works Delivery member of staff injured their left ankle when stepping from the platform ramp onto the ballast.
First Aid was given on site and there was no obvious sign of injury.

Possession Irregularity
0B69 reported that the train had come to a stand at a Possession Limit Board on the down Portsmouth line whilst departing Eastleigh approaching E745 points.
On investigation, it was established that the protection had been placed incorrectly on the Hedge End Side of the points vice Eastleigh side which was later corrected.
This incident is subject to further investigation. |

Staff Accident
A PWay team were lifting sleepers from a pack, but one fell out and landed on an individual’s left foot.
The foot became swollen and bruised.
No first aid was given, but arrangements were made for the Injured Party to go to hospital.
The injury was not x-rayed at hospital, but he the injured party was advised to apply an ice pack and rest by elevating the foot.
The Golden Hour was applied and this became a lost time injury. |

Level Crossing Irregularity
The Signaller on Wimbledon Panel 4 trapped two people within the crossing limits and cleared W407 signal for a Chessington to Waterloo service (2M55).
The Signaller on realising their error, replaced W407 back to danger (red) and was able to stop the train.
The barrier was then raised after a timeout to allow the two people to get clear.
The Signaller was relieved of duty and the incident is subject to further investigation. |

1D58 – Track Defect / Irregular Working
1D58 has reported a rough ride at the country end of Mickleham Tunnel and due to a breakdown in communication between the Wimbledon Panel 4 and Dorking Signallers, a caution was applied to the incorrect line for 6 trains.
Though no defect was found, Sussex will be carrying out an investigation into this incident. |

Staff Accident
A Storeman had just finished re-drumming a cable drum and was putting the cable jacks and spindle bar away in the back of a wagon/van.
Whilst lifting the spindle bar, it slipped from his grasp and landed on the brow of his foot.
The spindle bar weighed between 40-50kg and was made of solid steel.
The Golden Hour was applied and it was later established that a bone had been broken and was subject to an operation.It is likely that the Injured Party will be unable to work for approximately 6 weeks.

Staff Accident
A member of staff from Off Track was stung multiple times by a wasp/s whilst de-vegetating a drainage ditch at bottom of Guildford Off track yard.
He was stung on the right hand (which went through the gloves he was wearing), on the right and left arms and on the right hand side of the torso and face.
First aid was administered with antihistamine and the area cleaned with antiseptic wipes.
They were then sent home.
The Golden Hour was applied and it was anticipated that the incident would not result in further lost time. |

Train too Long for Platform – 1W94
1W94 was due to be routed into platform 7, but due to another train awaiting fitters attention, it was routed into 6.
The Driver accepted the route, but with the train being formed of 10 coaches it not fully accommodated.
The Guard also released the doors, although the ASDO system prevented the doors on the last 2 coaches from opening.
Upon review it was established that the stock formation information was over 2 pages on signal box workings and the Signaller had missed it.

Late reported Trespass / Irregular working
The Driver of 1L25 reported that they had witnessed 2 contractors adjacent to the down slow not wearing any form of Hi Viz clothing and believed by looking at a logo on their van that they worked for Kiers.
Upon the EIU Mobile Operations Manager attending, they could not find any contractors, but the access gate wide open.
This incident is subject to further investigation. |

Line blockage irregularity
A PWay team had a line blockage in place to cross the Thames Bridge, but during this time, the Feltham Signaller had an issue at Kew bridge between a class 2 passenger train and Class 5 empty train and which had to run first.
2S24 booked to run first, so a route was set from F124 signal to F122 which required the Signaller to remove the reminder appliance for the line blockage and then replace it.
The Signaller had also lowered Grove Park CCTV level crossing in readiness for the Line Blockage being given up.
The action of pressing the signal buttons and lowing the barriers, meant that 2S24 received a proceed aspect and entered the Line Blockage limits.
The train was stopped by Wimbledon Panel 5 by replacing W556 signal to danger and the PWay team were not affected.The Feltham Signaller was relieved of duty.This incident is subject to further investigation. |
