Item 48 Irregularity
A Machine Controller working for Ganymede Solutions left an RRV unattended a couple of times within a T3 engineering work possession and carried out movements without the appropriate authority of the Engineering Supervisor.
This incident is subject to further investigation.

Eastleigh
Road Traffic Accident
An Outer DU Welder reported that he had an RTA on 08/01/2018 whilst approaching a roundabout at the M27 junction.
A vehicle in front stopped unexpectedly causing him to swerve at the last minute and clip the rear bumper and wheel arch of the other car leaving a scuff mark.
There were no injuries.
The Golden Hour was not applied due to the late report.

Clapham Junction
Near Miss 5R05
South Western Railway empty service 5R05 reported a near miss with a team of 3 S&T on the Up Windsor Fast line London side of the station.The S&T had been called out to a failure on 663B points situated within the Up Windsor Slow, but during the fault finding process the team begun to focus their attention on the point machine situated between the Up Windsor Slow and Up Windsor Fast putting them in danger of approaching trains.
Having been conscious to rectify the fault, the team overlooked further adequate protection resulting in the Near Miss.
The Golden Hour and Chain of Care was applied to all parties in an exceptional manor.
The incident is subject to further investigation.

Woking
Staff Accident
Whilst selecting concrete sleepers within the yard, a PWay Team Leader caught the ‘dunnage’ (wood used to secure the storage of sleepers), resulting in one of them stacked 2 high to fall and trap his ankle against an adjacent sleeper pile.
The injured party attended hospital for an x-ray as a precaution, but apart a graze and bruising, there were no broken bones.
The Golden Hour was applied and this became a Lost Time Injury.
