Kingston
RRV Derailment
An RRV had derailed at F625 points within a T3 engineers possession caused by the adverse weather conditions and a build-up of ice.
The RRV was re-railed and the only damage sustained was to an insulating pot.
This incident was subject to further investigation.

West Drayton
Staff Accident
A Section Manager at Feltham had a low speed collision whilst driving onto the slip road to join the M4 when the vehicle in front braked unexpectedly.
Both vehicles sustained minor damage, but there were no injuries.
The Golden Hour was applied for chain of care.

Windsor and Eton Riverside
Signaller Error – 10 coaches signalled onto 5 coaches
The Signaller had been dealing with track failures within Windsor and had not been aware that a train had been spilt (as booked) leaving 5 coaches behind.
The Signaller then cautioned the next 10 coach arrival to pass the signal at danger only for the Driver to see the other train still in the platform.
The train was arranged to return back behind the signal and be re-platformed.This incident was subject to further investigation.

Woodsford No 38
Car on the Line
A member of public reported that they had a car accident, struck Woodsford No 38 crossing and the car was on the track.
An emergency switch off was arranged.
The MOM arrived on site and having made an assessment, advised that the car was clear of the railway and it was safe to run trains.
There were no track issues identified, but the S&T had to repair the damage caused to their equipment.

Dorking
Staff Accident
Whilst driving along a country lane, a member of Wimbledon PWay slid and skidded on ice/snow and careered to the right hitting the curb.
They tried to correct the skid, but ended up veering to the left and going down a bank into a bush.
The Golden Hour was carried out and apart from being shaken, there were no injuries.

Rowlands Castle
Cat A SPAD 2P50
South Western Railway service 2P50 reported passing HT18 signal at danger without authority by up to two coach lengths.
The signal had been at danger due to a signalling failure caused by a cable theft attempt and Emergency Special Working being in place.
The Driver made no allegation against the signalling equipment or unit.
This incident was subject to further investigation.

Wokingham
Member of the Public Injured
A Member of the Public was struck by 3 blue poles on the leg that slid from the top of stairs leading to a track access gate on 20/03/2018. (Emmbrook Road, bridge 19/1383, RDG2).
Upon inspection, there were up to 50 other poles connected to some project / engineering work and the 3 that fell had not been secure.
This incident was subject to further investigation.

Waterloo P17 – Passenger Accident
A member of the public was running to catch a train, but fell between the train and the platform edge.
They were helped back onto the platform by other members of the public, but they had noticeable blood within their mouth and was left in the care of platform staff.
No trip hazards were identified.

Staff Accident
A member of PWay staff pulling on rail fastenings, fell back when the hook used to do this slipped out.
The member of staff fell back onto another colleagues left leg awkwardly causing immediate pain.
The Golden Hour was applied and although there were no broken bones, the injury became lost time.

Weybridge
Track Noise/Defect 2F10
At 06:43 South Western Railway service 2F10 reported hearing a noise from the Up Fast line as 2P96 was travelling through the section.
2 further trains were cautioned and reported nothing amiss, so the Signaller returned to normal working prior to required PWay attendance.
At 10:14 2F24 (the same driver as 2F10 earlier), further reported a ‘scoop’ of rail missing from the nearside rail at the same location.
Trains were then diverted around the defect until a rail clamp was applied.This incident was subject to further investigation.

Staff Accident (RTA)
A Senior Project Engineer was the 3rd car back from a roundabout turning onto the M3.
The 1st car moved away and the 2nd car appeared to pull away too, but then stopped leading to a slow speed rear end collision.
There were no injuries.
The Golden Hour was applied for chain of care.

Vehicle Damage / Accident
A member of Feltham S&T damaged the work van by scrapping a wall/barrier when driving too close to the gate key pad.
There were no injuries and Chain of Care was carried out.

Bedhampton Level Crossing
RRV Oil Leak
An RRV working within T3 item 76A engineers possession experienced a burst hose and lost approximately 2 litres of hydraulic oil onto the track.
The majority of the spill was contained and cleaned.
A further incident then occurred as the machine was lifting a trailer off of the track where a further 180 litres of fluid was lost. The majority of which was contained in buckets and by spill kits. Approximately 5 Litres reached the ground.
This incident was subject to further investigation.

Raynes Park
Safety Incident
A PWay Section Manager reported staff  had advised them that a metal ‘J Bar’ which is placed through holes in wheel timbers and interlocks with other ‘J Bars’ and tightened with nuts had fallen off an embankment by bridge 33, landed on the roof of a car smashing the sunroof.
No-one was in the vehicle and a search was carried out to find the owner without success.
A note was left on the windscreen.
This incident was subject to further investigation.

Raynes Park
Staff Accident
A Contractor working for Works Delivery was carrying a Strail unit as part of a 4 man lift when they slipped over and the Strail unit landed on their left leg causing bruising.
They were assisted back to the welfare unit to be checked over. His leg was sore, but was able continue to do light duties (no lifting).
The onsite lead arranged for them to be taken home as a precaution and to report to work tomorrow morning to possibly be given light duties.
The Golden Hour was applied.

Eastleigh West Junction / Portchester
Possible Possession Irregularity – Item 106
The Outer ODM reported that whilst Possession Support Staff were removing protection to shorten Item 106 from Part 1 to Part 2, one detonator appeared to have been exploded beyond No 10Pts in Back Road No1 some time between 01:09 and 23:32.
Investigations are ongoing.

Worting Junction
Staff Accident
A member of Eastleigh Distribution and Plant slipped on the uneven ground and twisted their ankle.
The Golden Hour was applied and first aid was given.
The Injured Party later attended hospital where they diagnosed that the ankle may have been fractured.

Feltham /Ashford
Possession Irregularity – Item 514
A contractor, working for McGinley’s on behalf of Feltham Track Section Manager attempted to strap at the wrong location.
The contractor, who had strapped at this location the previous night, used the track locator app to verify their position due to the poor weather conditions however; they were approximately 2 chain from the correct location.
The contractor tested the conductor rail, but did not follow the process of strapping the running rails first and did not use either rubber gauntlets or short circuiting bar.
When the contractor tried to attach the strap to the conductor rail, the result was the con rail flashing over and causing the electrical breakers to trip.
Chain of care and a follow up investigation then took place.
During the Injured Party’s interview some 2hrs later, he complained of dry eyes and as a precaution, attended hospital where they confirmed that he had not suffered arc eye.
The Golden Hour was applied and no lost time was anticipated.

Staff Accident
A member of Woking PWay was digging some ballast on the track when he hit something hard and jolted his back causing some pain.
The Golden Hour was applied and no lost time was anticipated.

Havant
Van Door fall off
When leaving the maintenance depot, the Pway van crew sliding door fell off and into the road.
No one was injured and there was no damage to any other vehicles.
It was thought that the mechanism which had been faulty previous to the incident was rectified.
This incident is subject to further investigation.

Farlington Junction
HT2629 points failure / possession irregularity
Havant ASC reported that HT2629 points would not go back into auto after being in manual during possession Item 84.
Upon investigation it was established that the points had been put onto manual without the Signaller’s knowledge or permission.
This incident is subject to further investigation.

Hinchley Wood
Staff Accident
A member of Woking PWay staff stepped into a drain gulley and rolled his ankle when getting out of a van.
The injured party experienced a lot of pain and the ankle became swollen.
First aid was administered.
The Golden Hour was applied and this became a lost time injury.

Wimbledon
2M02 wrong route
South West Railway service 5M02 was offered to depart from No.2 road to arrive in platform 4 at Wimbledon on the LUL side which is restricted from this type of stock.
Arrangements were made for the Driver to be talked out and to run as an empty train to Clapham.
This incident is subject to further investigation.

Staff Accident
A member of Works delivery staff hand slipped and struck the location cabinet casing when removing old cables. In doing so, they aggravated an old injury.
The hand became swollen and warm to the touch and a cold compress was applied.
The Golden Hour was applied and the injured part was taken to A&E. There were no broken bones and no lost time anticipated.
The injured party had been wearing cat 5 classified gloves which had become wet, oily and contaminated.

North Sheen Level Crossing
Operational Incident – (Person trapped within crossing)
South West Railway service 2R35 advised the Signaller that a male was stuck within the Level Crossing limits.
The Signaller replaced the signals to danger on the up and down lines as fellow members of public helped the male out.
Upon reviewing the CCTV, it was established that the incident had been a Signaller error and is subject to further investigation.

Item 132 Possession Irregularity
A Trainee Signaller granted signal protection at 00:50hrs for T3 engineering possession item 132 with South West Railway service 2O77 still within possession limits at F91 signal.
No protection had been laid when the error was made.
This incident is subject to further investigation.

Staff Accident
A member of Eastleigh PWay was walking from an access gate toward the down line when they slipped on icy underfoot conditions.
The injured party broke their fall with their left arm which became swollen but did not require medical attention.
The Golden Hour was applied and there was no lost time.

Staff accident (Low speed RTA)
Whilst stationary in a work vehicle at the station, a Stores Coordinator was struck from behind by another vehicle.
There were no injuries and the Golden Hour was applied for chain of care.

Staff Accident (Low speed RTA)
Whilst driving a works van within the South West Railway car park, a member of Telecoms staff turned left down a slight slope and upon applying the brake, the wheels picked up on the ice and slid into the front of a parked car.
There was no damage to the van however; the parked car sustained a crack to the bumper and a dent to the bonnet. There were no injuries.
The Golden Hour was applied for chain of care.

Haslemere
Staff accident (RTA)
A member of Woking Distribution staff collided with an abandoned parked car that was in the middle of Woolmer Hill Rd. The work van was too damaged to move, but there were no injuries.
Arrangements were made for the recovery of the vehicle and a colleague picked up the Driver.
The Golden Hour was applied for chain of care and there was no lost time.

Waterloo
Wrong head code – 12 car 450
Due to degraded mode, South West Railway service had been described as 2F24 vice 2F98 in error.
The error was realised before the train arrived at Waterloo on the Up Slow line and arrangements were made to allow the service to arrive in platform 6.
A clear understand was reached between the Signaller and train crew, as the service was formed of 12 coaches and would not be fully accommodated.

Portsmouth and Southsea
Reported operational incident – 1P33
Due to degraded mode, 1P33 terminated within a low level platform formed of 12 coaches and was not fully accommodated.
The selective door opening system however; only opened doors on the coaches that were within the platform.

Kew Bridge
Staff accident (RTA)
(Late report) – At 09:35hrs on 02/03/2018, a member of Feltham PWay was driving through an access gate at approximately 5mph in a Network Rail van, but due to the icy conditions the van slid sideways and made contact with the gate post.
Damage was sustained to the left hand side door and the window shattered.
There were no injuries sustained, but Chain of Care was carried out.
The Golden Hour was not applied.
