Clapham Junction / Berrylands
Possession Irregularity (Item 42)
The PICOP of item 42 reported that 6N03 had left the worksite with no certificate of readiness (to confirm that the load was in gauge and secure).
This incident is subject to further investigation.

Boxhill
Irregularity Line Blockage
Wimbledon P-Way reported that they had a line blockage irregularity after the COSS had mixed up their mileages.
They had a Line Blockage between 18m 45ch – 18m 20ch, but meant to take it between 20m 69ch – 20m 68ch.
As they got halfway across the River Mole bridge, they became aware of a train approaching and were able to reach a position of safety.
This incident is subject to further investigation.

Blackwater
Cows struck
A GSMR Emergency Rec call was made by 2O53 (Great Western Railways) to report a herd of cows in the cess on the Up line, one of which had been swiped by the train.
Upon the MOM attending, the cows were returned back into their field and a broken fence line was repaired.

Waterloo
Staff accident
An Operations Delivery Manager was involved in a multi-vehicle RTA near the Shell Building and sustained injuries to the neck, back, head and shoulders. There were no broken bones.
The Golden Hour was applied and this became a lost time injury.

Hinton Admiral / Christchurch
Tree cut down in front of 1W17
1W17 reported that the train had just ran over a section of tree which had just been cut by tree fellers.
The Driver was very shaken by the event, but there was no damage to the train.
Upon investigation it was established that trees were being cleared by Works Delivery staff (Off Track).
This incident is subject to further investigation.

Staff assault
Feltham S&T reported a male trespasser crossing between platforms 1 and 2 shouting abuse, filming them.
As time went on, the male then started spraying graffiti and became physically violent with the S&T when challenged.
The male was later detained by the police.

Emsworth
Staff Accident
An Outer Delivery Unit Section Manager [Rail Testing & Lubrication] was stationary waiting for a vehicle to turn right into Sandy Lane from the A334, when he heard heavy braking from behind and was struck on the off side rear by a large van.
The Golden Hour was applied and the injuries are to be confirmed.

Aldershot
Worksite Irregularity
Tamper 6J61 was given authority by a McGinley Engineering Supervisor to travel through a worksite at caution within T3 Possession Item 116, but came across 2111 points set against them and stopped short.
The ES admitted their error, but was permitted to continue.
The Till Dawn process had not been in place on this line as it was a single transit movement.
