Hersham
Irregular Working Item 39
The Engineering Supervisor tor T3 possession Item 39 was advised by the plain line tamper crew that there were two members of staff surveying the track on the Up Slow Cess.
The ES was not aware of this, as the staff had not signed into their worksite.
It was believed that the staff were watchman for the Sink Hole incident.
This incident was subject to further investigation.

Possession Irregularity Item 39
The PICOP for T3 possession Item 39 reported that the straps had been placed incorrectly on the Up Slow/Fast at 10m 12ch vice 10m 01ch.
The irregularity was realised upon the Driver of tamper 6Z03 who stopped short of them.
This incident was subject to further investigation.

Rowlands Castle
Staff accidents x2
Two members of Havant PWay staff were sprayed in the face with hydraulic fluid whilst removing a hose from a track stressing kit.
First aid was administered and the Golden Hour was applied.
The staff suffered no ill effects.

While reversing the van to park it at the Depot, a Track Quality Supervisor accidentally hit a skip. There was minor damage to the rear door, but no injuries.
The Golden Hour was applied for chain of care.

Byfleet and New Haw
Staff accident Incident
A Woking PWay Technician was walking back through an alleyway with his team after giving up a T3 possession when his ankle gave way causing it to twist.
He had experienced he same injury 1 month previous.
The Golden Hour was applied and no lost time was anticipated.

Epsom
Line Block Irregularity
A Line Block irregularity took place when the Signaller granted the authority number to the COSS to start work and then realised that a South West Railway service (2D38) was still in section and asked the COSS to stand clear.
The train concerned had been beyond the workgroup at the time, but within the blocking limits.
This incident was subject to further investigation.

Wimbledon Park Depot
Possession Irregularity – Item 506
The South West Railway Wimbledon Park Depot Performance Manager reported that a marker board for the possession on no.1 road had been placed in a different position to the previous night (at the Earlsfield Exit).
Upon investigation, it was confirmed that an error had been made by a Possession Support Staff member from the Clapham PWay.
This incident was subject to further investigation.

Staff Accident
A member of the Outer Ultrasonic team received a shoulder injury whilst lifting Ultrasonic equipment.
The Golden Hour was applied and there was no lost time.

Staff Accident
A member of Feltham S&T twisted their left ankle whilst walking on ballast.The ankle became sore, but there was no immediate bruising or swelling.
The Golden Hour was applied and there was no lost time.

Fratton
Damaged AWS for HT55 Signal Incident
A Track Quality Supervisor reported that a Tamper working within T3 possession Item 91 had damaged the AWS for HT55 signal at 44m 3ch.
The AWS was unreliable and trains cautioned post the T3 being given up until a temporary solution was put in place.
This incident was subject to further investigation.

Staff Accident
A member of Havant S&T trapped their finger when opening a location cabinet door.
The Golden Hour was applied and there was no lost time.
