Cat A SPAD
South West Railway service 2N09 07:23 Ascot – Guildford reported passing GD956 at danger on the Up Ash line approaching Ash Station after a loss of concentration. There was no allegation against the signal.

Stoneleigh
Staff accident
A member of Inner PWay was walking into the Up Cess country side of Stoneleigh when a train was seen to be approaching. Upon stepping into a position of safety, they slipped on a landslip board and fell awkwardly onto their left wrist injuring the wrist and thumb. The Golden Hour was applied and monitored overnight. There was visible bruising on the wrist, but the injured party was able to resume.

Staff accident
Whilst assisting to de-train passengers and their luggage from a failed Great Western Railway service, an Inner Mobile Incident Officer pulled their right bicep resulting in difficulty to lift their arm and carry small items for a period of time. The Golden Hour was applied and this injury became a lost time.

Feltham West Level Crossing
Signaller Level Crossing irregularity
A Trainee Signaller trapped a Level Crossing User within Feltham West Level crossing and ran 2 South West Railway services (1 on the up and 1 on the down) without realising an error had been made. The incident was reported via social media and is under investigation.

West Byfleet
Signaller cautioning irregularity
South West Railway service 2B24 07:38 Southampton Central – London Waterloo was cautioned about a speed restriction being put in place, but was not given specific instructions about what was required. As a result, the train was travelling in excess of 20mph over where the speed applied. Upon investigation it was established that the Signaller had not carried out a robust handover and a breakdown in communication had occurred. This incident was subject to further investigation.

Haslemere
Staff assault
A Contractor from B&M McHugh was carrying out scoping for a brick work job at Haslemere and was approached by a group of youths. The youths appeared to be drunk and became abusive. The Contractor asked them to leave them alone, but the youths then set upon them as they started to attempt to flee away. The Contractor eventually made it into Haslemere Signal box where chain of care was carried out. The injuries sustained were to the head (nose, face and eyes) and a hospital visit was recommended.

T3 Strapping irregularity
The Guildford Signaller reported that CUM track circuit had become occupied and suspected it to be a short circuit strap that had been placed in wrong location. This was confirmed by the Person in Charge of the T3 Possession and it was identified that the strap had been placed on the wrong side of the block joint.

Signaller Line Blockage irregularity
The Wimbledon Shift Signaller Manager reported that the Signaller on Panel 2 at Wimbledon ASC had granted a Line Blockage on the Up Windsor Slow at 06:36 which overlapped with T3 possession item 14 in error. Upon realising, the Signaller contacted the Controller of Site Safety give it up. This incident was subject to further investigation.

Wylye Level Crossing
Staff accident
A member of Salisbury S&T was walking back to an access point and whilst track side at Wylye Automatic Half Barriers, they slipped off the troughing route and twisted their right knee. They arranged to rest it for the remainder of shift and the Golden Hour was applied. Lost time was anticipated.

Staff accident (RTA)
A member of Salisbury PWay was traveling back to Salisbury depot in a Network Rail van and whilst navigating the St Pauls Roundabout in Salisbury, they moved from the left hand lane to the right lane and bumped/scuffed the rear driver’s side of another car. There were no injuries sustained and the Golden Hour was applied for chain of care.

Possession Support Staff Line Blockage irregularity
The Three Bridges Route Operations Centre Shift Signaller Manager reported that a Line Blockage irregularity involving McGinley Possession Support Staff (PSS) within Wessex WON 30, Item 50 had occurred on the St Helier Lines. Post carrying out their initial PSS duty at the start of the possession, the Line Blockage was given up at 02:45. No further contact was then made until approximately 05:00 from the same PSS saying they had now removed equipment and wanted to give the Line Blockage up. This incident was subject to further investigation.

Staff accident (RTA)
A Network Rail van being driven by a member of Wimbledon Signal Works was involved in an RTA when a member of the public pulled out from Merton High Street onto Abbey Road in Merton without checking that it was safe to do. The Network Rail van clipped the rear wing of the member of public’s car causing minor damage. A second member of staff was within the Network Rail van, but there were no injuries sustained to any party. The Golden Hour was followed for chain of care.

A Project Scheme Manager was driving to Woking Stores in a Network Rail hired van when a vehicle reversed into the temporary traffic lane they were driving on at low speed. The vehicle clipped the rear side of the Network Rail van and apart from minor damage, no injuries were sustained. The Golden Hour was applied for chain of care.

Byfleet and New Haw
Staff accident
A Contractor from Cleshar Rail was walking through a worksite with artificial lighting when they tripped on a piece of loose ballast within the four-foot and fell over. The Injured Party put their left elbow out to break their fall, but in doing so the elbow also landed on some ballast. There was no cut or graze present, but the injury begin to swell and arrangements were made for them to go to A&E for an x-ray. The Golden Hour was applied and lost time was anticipated.

Parkstone
Staff accident
A member of Outer PWay rolled their foot on a piece of ballast whilst walking trackside near Parkstone.  They did not make any comment about it at the time and continued working. The following day, the injured party returned to work but then went to hospital to get it checked. The Hospital found that soft tissues had been injured. Upon the injury being reported, the Golden Hour was applied and the injury became lost time.

T3 Possession irregularity
Upon a Rail Grinder exiting T3 Possession item 88A at Gillingham, the Driver reported that the Possession Limit Board was the wrong side of the Worksite Marker Board by signals SE4683 and SE4681. The PICOP of item 88A reviewed the situation and confirmed that the issue was with item 88 and not 88A and that the Engineering Supervisor had not been aware of the change resulting in the error being made. This incident was subject to further investigation.

A member of Bournemouth PWay was carrying out a track side inspection and whilst walking across Lymington Viaduct, the wooden walkway of the bridge gave way resulting in their left leg falling through up to the hip and jolting their back. The injured party was stuck for a few minutes, but managed to free themselves. The Golden Hour was applied and apart from suffering from some discomfort, no lost time was anticipated.

Cat A SPAD
The Driver of South West Railway service 2G68 20:34 Guildford to London Waterloo reported passing W2 signal by 1 ½ coaches at danger having thought it had been displaying a yellow aspect. Forward Facing CCTV footage showed otherwise and there was no evidence of read across as all the other signals on the same gantry were also displaying a red aspect. This incident was subject to further investigation.

Staff accident
A member of Works Delivery staff set up a disk cutter whilst at Yeovil and when pulling the start cable, they felt their right shoulder go and believed that it had become dislocated. The Golden Hour was applied and having sort medical attention, this became a lost time injury.

T3 Strapping irregularity
The Wimbledon PWay Section Manager received advice that a Strapman for T3 Possession item 37 had placed a strap on the open St Hillier lines at Wimbledon having become disorientated. It was later found that they had not tested nor applied a short circuiting bar on this line before applying. Chain of care was carried out for potential arc eye. This incident was subject to further investigation.

Yeovil Junction
Staff accident
A member of Eastleigh Works Delivery staff was working on track at Yeovil Junction when an On Track Machine knocked a piece of scrap rail which fell onto its side trapping their right foot just above the steel toe cap. The rail was removed and the injured party was taken to hospital as a precaution. Although in a lot of pain, x-rays confirmed no broken bones. The area later became numb and the injured party was prescribed morphine based painkillers. The Golden Hour was applied and this became a lost time injury.

Electrical Control Room irregularity
The Eastleigh ECR reported that one of their Operators was carrying out continuity checks on the Ash Vale line (PAA1) and had inadvertently closed a Circuit Breaker (CB) within T3 possession item 46. The CB tripped on close and the Operator was stood down pending further investigation. There were no injuries, but the isolation straps applied for the T3 had to be replaced.

Hydraulic Oil Leak
6U26 05:00 Eastleigh East Yard to Woking Up Yard had a hydraulic oil leak and lost approximately 150 litres on the Down Main in the Micheldever / Wallers Ash Tunnel area. The fluid was treated with sand and the British Waterways and the Environment Agency were made aware. A NOP Form F4 14A was completed and emailed to the route Workforce Health, Safety and Environment Advisor. This incident was subject to further investigation.

Hydraulic Oil Leak
Possession staff working within T3 possession item 136 reported that the RRV railvac had leaked approximately 10-20ltrs of hydraulic fluid onto the track within Kingston Bay platform 3. A spill kit was used.A NOP Form F4 14A was completed and emailed to the route Workforce Health, Safety and Environment Advisor. This incident was subject to further investigation.

CAT A SPAD
Ariva Overground service 2L93 16:14 Clapham Junction – Stratford had a Cat A SPAD on W306 signal upon becoming distracted and passed the signal by 20 metres. There was no allegation against the signal.This incident was subject to further investigation.

Cat A SPAD
Balfour Beatty tram 6J61 23:34 Chard Junction to Honiton passed SE4764 Ground Positon Light shunt signal at danger without the authority of the Signaller by 511 metres. The signal had been unsighted by the Driver having stopped on top of it rather than clear of it whilst performing the shunt. This incident was subject to further investigation.

Staff accident (RTA)
A member staff from Telecoms Inner was involved in an RTA on the M25 (junction 7 with the M23) with a white van that attempted to join from the slip road. The white van clipped the Network Rail van causing minor damage. There were no injuries. Golden Hour was applied for chain of care.

T3 Possession irregularity
The Site Supervisor within T3 item 9 reported that W757B points at Wimbledon West Junction had been run through by a Rail Road Vehicle through a breakdown in communication when carrying out the movement. This incident was subject to further investigation.
