| 10/01/2016 | Southampton – A member of Eastleigh Signal works sustained an injury to his lower back and left gluteal after falling whilst undoing a tight lock on a tubular stretcher. |
| 11/01/2016 | Staines – An Operative working within Kent/TLP Possession Item 37 outside Cannon St called Feltham Signal Box for 701 points to be moved across by Signaller in error.When the points did not move on the ground, it became clear that the Operative on the ground had called Feltham Panel 2 instead of London Bridge Panel 2 |
| 12/01/2016 | Hook – PICOP for item 63 reports the ES was taken ill with a suspected stroke. At 0100hrs the ES was feeling sweaty and clammy and his colleges called for arranged an ambulance. This person has subsequently passed away. |
| 13/01/2016 | Havant – A member of Havant PWAY whilst offloading sleepers from a lorry strained his lower back. |
| 13/01/2016 | Wimbledon – 2J49 had a CAT A SPAD at W187. |
| 14/01/2016 | Warminster-Wilton Jn – PICOP reported that the Barrier man within possession item 119 had authorised 6N70 over the crossing with a green flag without the crossing barriers being lowered. |
| 14/01/2016 | Woking – Member of staff within the AMM office at Woking burnt her middle and ring finger of her left hand on a kitchen hotplate. |
