Staff Accident (RTA) An S&T Section Manager was in York Road when a vehicle in front of him put their car into reverse and struck the front of the railway vehicle causing minor damage. There were no injuries. Golden Hour applied..

Near Miss
2V27 reported a near miss by 1780 points with 2 members of staff standing in the 4 foot. The driver stated that when blowing the horn they had to jump out of the way to get clear.This incident is subject to further investigation

Staff Accident
A Colas Site Supervisor working on behalf of IP was assisting in moving a piece of pipe using a crane when some liquid ran out of the end and fell onto his clothing which caused a burning sensation after 30 seconds. He removed his clothing and noticed a red patch on his skin and applied water.First aid was given on site, but he did not seek further medical attention.

Overton
Unsafe Work 1L39 reported that the train had struck a piece of equipment approximately 1 mile on the approach to the station. The Driver stated that there was a team in a position of safety within the cess as he felt the train struck what appeared to be measuring equipment (Tripod and gauge). This incident is subject to a Babcock investigation.

Early indications are pointing toward the gauge being sucked into the train having not been fixed down properly.

London Waterloo International
Staff accident A contractor working for Ekspan required 10 stiches in their stomach after being cut by a metal spike attached to a bridge bearer they were removing.

The stitches were due to be removed after 10 days and he was due to resume work the following day albeit at a different site.

Golden Hour applied.

Staff accident A member of Works Delivery was crossing the track on 06/09/16 when his left leg locked and caused his calf to swell. He returned to work the following day, but was sent home and advised to visit his GP.

The Injured party visited a walk in clinic and was advised to rest for a couple of days.

Due to the nature of role however, he was able to continue without any lost time.

Staff accident PWay Operative was using a hammer to unkey a rail in order to carry out a stress, but caught his finger causing a cut to his left index finger. First aid was given and the bleeding stopped. The injury was cleaned and bandaged.

Staff accident S&T Section Manager stepped out of his vehicle when arriving at work and when he put his foot down, he twisted his right knee causing swelling. The Injured Party took anti-inflamatories and was fit to continue duty. Golden Hour applied.

Staff accident Telecom Operative reported upon arriving home after work on 07/09/16, he found a 2 cm cut on his right shin and applied a plaster. No lost time.

Staff accident PWay Manager parked his van having just finished work.

When he stepped out and put his weight on his left ankle, he twisted it on uneven tarmac. Medical attention was not required. Golden Hour applied. No lost time.

Reading Spur
Staff Accident  Member of Basingstoke S&T pulled a muscle in the right hand side of the lower back when removing the front cover from 8787 points. (A set of HPSA’s). The Injured Party was working with one other person (Team Leader) and was using correct manual handling techniques. Section Manager reported that the Injured Party attend the A&E in Reading but there was a 4+ hour wait to be seen, so went to the Basingstoke hospital. He was told to take it easy for 3 – 4 weeks, prescribed codeine and advised to take Paracetamol and Ibuprofin if he experienced further pain.

Golden Hour applied.

Woking
Staff Accident An operative from Guildford Off-Track was loading the work van with scrap fencing, when a piece cut him on his arm. The cut was approximately an inch in length, believed to be deep and first aid was given on site. The injured party attended a walk in clinic where the wound was dressed the wound. The Golden Hour was applied and no lost time was anticipated.

Hounslow
Staff accident A CRE operative was installing a 21 foot cable with a group. A Lug was at awkward angle and the group attempted to straighten it up by tightening the bolt with a big spanner. The Injured Party was in front of person with the spanner holding an impact wrench when the spanner slipped off the bolt. The person holding the spanner fell forward and the spanner struck the Injured Party in the mouth causing a cut lip and broken/wobbly tooth. Due to the late report of this incident, the Golden Hour was not applied, but the Responsible Manager did follow the incident up with the individual involved.

Cat A SPAD 1D43 1D43 (1554  London Waterloo – Dorking) passed W91 signal on the down main slow at Danger by 2 coaches. The signal had been at danger due a failure on W151 and a backlog of trains and the Driver had not controlled his train correctly upon leaving Vauxhall and reacted too late at W91 to stop. This incident is currently under further investigation.

Staff accident A Ganymead contractor (working for Works Delivery) was digging out timbers by 1585pts and a whacker plate compacting ballast disturbed a wasp nest. A wasp then stung the Injured Party in the throat which caused swelling and difficulty breathing. Having attended St.Thomas hospital and taken medication (including anti-histamine cream) , he was sent home and advised to seek further medical attention if he did not feel any better the following morning. The Golden Hour Applied and the Injured Party was back to work the following night.

Yeovil / Crewkerne
1L29 struck Cow 1L29 reported striking a cow at 126m 20ch at line speed.

The train did not de-rail. Having inspected the train, the Driver reported no running damage seen, but the cow catcher was broken. The MOM later found a broken 2nd strand of a 3 wire fence at 125m 64ch where the cow may have gained access.

Cat A SPAD (P) FY1 / Stop Board / Terminal gate Collision   DB Cargo advised that 6B41 0648 Eastleigh to Fawley had a Cat A SPAD and collided with the closed terminal gates, pushing them back by three feet. The Driver alleged poor railhead conditions.

This incident is subject to further investigation by the Freight Operator DBS.

Staff Accident A member of the PWay was loading stressing kit onto a trolley to push back when the long bar of the stressing kit slipped and squashed his little finger causing a small cut. The finger was cleaned, a plaster applied and no hospital visit was required.

The Golden Hour was applied.

Staff Accident A Wimbledon Grinder picked up a spent fire extinguisher by the handle to place on the platform.

When as he did this, pressure was applied to the handle which fired the extinguisher and whipped the nozzle into his face causing a sore eye. Sterilisation water was used to clean the eye and a visit to the hospital was required. There was no damage to his eye, just swelling.

He was provided eye drops and resumed work the following night.

The Golden Hour was applied.

Staff Accident An Eastleigh Distribution Technician was cutting back vegetation when one of his feet got caught in a bramble bush.

On trying to get it free he felt his right knee twist causing him to fall over. The knee became swollen and a hospital visit was required where it was confirmed that there was soft tissue damage.

It was recommended that the Technician rest it for the rest of the week and take pain killers. This accident was anticipated to accrue at least 5 days (3 working days) time free from duty.

The Golden Hour was applied.

Wimbledon
Staff Accident – Wimbledon Track Operative was a passenger within a works van when it hit the kerb causing it to lift in the air.

The Operative felt a sharp pain to his left shoulder where the seatbelt was positioned, and an ache further down his left arm momentarily.

A hospital visit was not anticipated.

The Golden Hour was applied.

Item 70 – Points Run Through 29B

PICOP for item 70 (Shalford junction to Petersfield) reported via the Signaller that 29 points had been run through within worksite B after Tamper 6J61 authorised to move by the ES.

This incident is currently under further investigation.
