Epsom
Near Miss 2D27
2D27 reported a near miss with a member of Clapham PWay staff approaching the station.
The Driver stated that he had sounded the horn and member of staff acknowledge the warning, but kept walking towards the track. The emergency brake was applied. This incident is subject to further investigation.

Waterloo
Incident Waterloo Platform 15
It was noted by the Station Control that a member of Platform Staff was retrieving something off the track with Litter Pickers without a block taken. This incident is subject to further investigation.
19/01/2017
Farnborough
Staff Accident
A member of Works Delivery staff working within the MMT at was coming down the steps within the train stepped onto the ballast and his right ankle gave way. He fell, but did not sustain any other injury.
The injury had been pre-existing, but all the correct PPE had been worn.
Golden Hour applied.
No lost time. A referral to Rehabworks was being considered.

Dorchester West Station
2O66 – Struck Detonator
2O66 reported to have struck a detonator on the approach to the station (DR179 signal). The Signaller advised there had been a T3 in the area overnight (WON 42 Item 113).
09:09 – Yeovil MOM found the remains of the detonator. ODM made aware. This incident is subject to further investigation.

Wimbledon
Staff Accident
A Wimbledon Welder was loading welding equipment onto the van when the generator slipped and fell onto the middle finger. (Left or right not declared).
Golden Hour was applied.No Lost Time was anticipated.

Staff Incident/Accident
A member Off Track was sprayed with effluent when GWR 1F14 (1023 Portsmouth H and Cardiff) passed.
They finished their shift to go home and clean up.
16:15 – The individual had a Hepatitis jab.
Golden Hour applied.No lost time.

Surbiton
2L52 CAT A SPAD
2L52 (1524 Basingstoke to Waterloo) had a CAT A SPAD at WK130 signal on the Up Main Slow.
Pictures from FFC show the signal was displaying a red aspect on approach, although the Driver stated that he has thought it had been showing a double yellow due to sunlight shining on the signal.
The train passed the signal by 1 coach length.This incident is subject to further investigation.

Surbiton
Staff Accident
A Works Delivery contractor was transporting a 150lb length of conductor rail with an Iron Man.
Once on site the individual was lowering the con rail and it slipped from the ratchet and landed on his right foot.
The pain got progressively worse and an ambulance was called.
08:50 – The injury was confirmed to be a broken metatarsal and put in a cast.
Golden Hour applied.This was a Lost Time Injury.

Wimbledon
Item 7 Possession Irregularity
6U41 Stone blower was working in item 7 WON 43 at Wimbledon.
The Driver was asked to go into platform 6 by the ES, but had to stop short of the intended location due to a engineering trolley on the line in front of the train being used by the Raynes park CRE team who had mistakenly placed it there.This incident is subject to further investigation.

    Would it be possible to give a break down of relevant speeds by zones: 20 mph = X , 30 mph = X and so on – also hour between.

    This would make a good topic re safety conversations – re- behaviours – ” why we still speed even with a reminder device !!!!”
