CAT A SPAD 2C73, a London Waterloo to Reading service reported a TPWS activation on W79 signal after passing the signal at danger (red) by reading across to the signal alongside. The train was travelling between 10-15mph. This incident is subject by a SWT led investigation. |

CAT A SPAD Freightliner 0M04 passed E965 signal at danger whilst carrying out a shunt move. The Driver had been distracted by a fire in the exhaust of the loco. This incident is subject to a Freightliner led investigation. |

Staff Injury A Technical Officer was walking up access steps and felt a twinge in their upper leg. Golden Hour was applied. Post incident the Injured Party still had some pain, but there was no cause for concern and no lost time was anticipated. |

WON 44 Item 143 – Possession Irregularity The Dorking Signaller reported that the PICOP had contacted him to advise that he had gained signal protection his T3 possession from Wimbledon and Three Bridges Area Signalling Centres at 0047 however; a set of empties (5E32) had only just left Dorking heading toward Leatherhead which had meant that the T3 had been given signal protection with a train still in section. The main underlying cause subject to further investigation is that neither Wimbledon or Three Bridges had communicated with Dorking to establish if it was safe for the T3 to be taken. |

Location Cabinet Struck Surrey Steels reported that one of their forklift trucks had accidently knocked over a pile of steel. The steel slid down, knocked over a fence and rolled into location cabinet which then fell toward the running line. The workers managed to make the area safe without going trackside until response teams arrived to rectify the situation. This incident is subject to further investigation. |
