Incidents last week

12/02/2017 Waterloo Staff Accident
An Electrification section manager was walking over the Westminster Bridge when he tripped on one of numerous items sticking up (troughing, tools, cables et). As he tripped he twisted right ankle, banged his left knee and had a slight knock to bridge of nose where his glasses caused a minor cut.
Golden Hour was applied and there was no absence from work.
This incident is subject to a Level 1 investigation.
13/02/2017 Waterloo Possession Irregularity
An ES for T3 Item 3 was setting a route to allow a train movement out of a worksite.
When walking back, he noticed that the route he had set had been changed and had not given any authority for this.
Upon investigation it was established that testers had moved them having had permission to do so on a previous occasion.
Initial indications point to a breakdown in communication, but is subject to further investigation.
This incident is subject to a Level 1 investigation.
14/02/2017 Wimbledon 2G21 CAT A SPAD
2G21 (1003 London Waterloo – Guildford) passed W187 signal at danger (red) without authority after he believed it had been displaying a single yellow. The TPWS brought train to a stand 7 metres beyond the signal.
The signal had been at danger due to a Kingston service in the section ahead.
Initial indications point to the Driver having a lapse in concentration and is subject to a SWT Level 2 investigation.
19/02/2017 Millbrook Staff Accident
A member of staff working for Eastleigh Works Delivery within a gang changing sleepers within possession Item 42.
The rail had been jacked up by a simplex jack and as the injured party was in the process of clearing out ballast from the base plate of the rail, another operative released the lift without prior warning. (Fingers and Toes Clear, 1, 2, 3, Lower).
The injured party’s hand became trapped and immediately swelled up.
Upon being taken to hospital it was established that the right hand had been bruised.
Golden Hour was applied and there was no lost time.
