GZAC Declined The Basingstoke WOE signaller declined a GZAC Line Blockage between SE2819 points to SE4740 points owned by Osborne, as the worksite was Crewkerne tunnel. Had the Signaller not queried where they were working, they would have been at risk from approaching trains. This incident was Close Called. |

Staff Accident An Operative from Guildford off track was putting a petrol blower back into the van when he accidently touched the hot exhaust section of the blower causing a burn on the tips of 3 left hand fingers. First Aid was given and the Golden Hour was applied. No lost time was anticipated. |

Near Miss / Operational Close Call A Senior Contracts Manager from Environmental Forestry advised that a near miss had occurred with a South West Trains service and a Lookout crossing a limited clearance bridge (ref: 17/100 at 62m27ch). The bridge was 18 metres in length and the Lookout was said to have reached a place of safety within 4 seconds of the train passing. The incident had initially been reported as a Close Call to Control and had not been reported as a Near Miss by the Driver. This incident is subject to further investigation and is awaiting further evidence to fully understand events. |

Staff accident A Clapham Team Organiser was bumped into accidentally by another passenger on some stairs and she consequently landed awkwardly and sprained her right ankle. She was offered first aid at the time but declined, although did visit A&E who confirmed the sprain and was advised not to put any unnecessary weight on it. The Golden Hour was applied and lost time was anticipated. |

Andover Yard A Wessex Route Health and Safety Rep reported that he was carrying out a follow up visit after finding the access gate to the Yard being left open. The gates were in such a state, that they could not be closed and secured. It also appeared that they had been like this for some time, as there were a number of public cars and good vehicles parked within the boundary. This was Close Called. Note: This type of issue is currently being presented at the Safety Workshop and poses a risk to the public. Please ensure all access gates are in good condition and are secured correctly after use. |

Coping stone displaced An On Track Machine hit a coping stone within platform 2 at the 18 mile post. The stone was removed and repositioned, but was still a tripping hazard. The MOM attended and sprayed some signage to highlight the area, but also noted other stones in a similar condition requiring repair. The fault was reported to the Property Action Line. |

CAT A SPAD 1C93 (1732 Southampton Central – Barnham) questioned a TPWS activation at E811 signal on the down Netley line after he thought it had been displaying a single yellow. 18:31 – The S&T logger was reviewed which confirmed the signal to have been at danger due to another train (1T49) occupying the platform ahead. 00:10 – Eastleigh S&T completed their testing post incident and all was in order. |
