Woking Junction
2P11 CAT A SPAD (WK375)
2P11 passed WK375 at danger by a third of a coach length after the Driver misread the signal on the gantry.
There was no allegation against signalling or rail head.
This incident is subject to a Level 2 investigation.
20/11/2017
Woking Down Yard
Derailment Engine 6Y55
The DB Cargo Shunter at within the down yard reported that a locomotive on 6Y55 had derailed within No 2 road.
Upon investigation it was established that the sand / aggregate pile had compacted into the check rail and allowed the locomotive wheel to ride over and derail.
The incidence occurred on Network Rail infrastructure.
This incident is subject to a Level 1 investigation.

Staff Accident
Whilst fitting an AWS electro magnet on WK608 an S&T member of staff trapped and crushed their right index finger.
The Golden Hour was applied and there was no lost time.

Staff Accident
A member of staff from Woking D&P twisted their ankle by the substation.
First aid was given on site and the Golden Hour was applied.

Waterloo
Worksite irregularity, Item 29     
ES for WON 34 Item 29 reported that a Wessex Capacity member of staff was working alone without a COSS present.
Upon being approached, the individual advised that he had signed in however; this was not reflected on the COSS paperwork.
This incident is subject to an IP investigation.

Weybridge
6J06 Oil Leak
The TQS working in item 41 (Berrylands – Byfleet Jn/Byfleet and New Haw – Effingham Jn – Addlestone Jn) reported that 6J06 on the up fast line country side of the station had an oil leak.
100 litres of biodegradable oil was lost.
2 staff were affected by the leak, an accident form was generated and the Golden Hour was applied.
This incident / accident is subject to a Level 1 investigation

Bournemouth
Staff Assault
The Bournemouth Mobile Operations Manager observed a drunken male walking in the road coming towards his vehicle and reduced his speed.
The male then proceeded to throw themselves onto the bonnet of the vehicle, aggressively knocked on the driver’s side window and hurled abuse at the MOM about being ran over.
The incident was witnessed by a Bus Driver and was reported to the Police.
The Golden Hour was applied for chain of care.

Templecombe
Operational Irregularity
Upon communicating with a COSS to take a line blockage, the Signaller made an error and granted the block with a train still in section on the belief that it had passed the work group.
It was upon the authority number being given and when the staff walked onto the track to start work, that the train then approached.
The staff were able to move in a position of safety and a Near Miss was not declared by the Driver.
This incident is subject to further investigation.

Eastleigh South Junction
Irregular Working Contractors
CRE Section Manager saw two males wearing only orange tops. When seen, they appeared to run and hide in trees bushes behind a disused washer. Upon being approached, the males stated that they had been taking pictures of HV switchgear.
The men left site and were seen to get in a McGinley van.
The incident was reported to BTP and McGinley to follow up and investigate accordingly.

2F94 Cat A SPaD W186
2F94 reported passing W186 at danger by 15 feet after losing concentration.
There was no allegation against the signalling or the unit.  This incident is subject to further investigation.

Wilton Junction / Warminster
Near Miss – 3S80
3S80 called into Salisbury Signal Box to advise them of a near miss, which occurred at approximately  118m 20ch, near bridge E29/34 Tytherington.
The Driver advised that as the train came around a curve, they saw a member of staff with a blue bag and sounded the horn. The member of staff acknowledged.
As they continued, the Driver then came across two further staff members in orange clothing within the 4 foot, who looked surprised. One of the staff members pulled the other from the track into the cess.
Upon further investigation it was identified that Salisbury P-Way were dealing with a track defect in the area who had been taking line blockages in the area.
The Signaller was not implicated.This incident is subject to further investigation.

Staff seen crossing track on mobile phones – 3S80
An S&T Cover team working on the behalf of Works Delivery were seen to cross the track using mobile phones.
This incident is subject to further investigation.

Waterloo
Staff Accident
A member of Ganymede staff working for Clapham DU carrying out a sleeper change, twisted their left knee damaging a ligament.
He attended hospital where he was fitted with a leg brace and given crutches for support.
The Golden Hour was applied and this became a lost time injury.
18/11/2017
Wimbledon
Staff accident
A member of staff Pway staff walked into a bracket at the back of their van whilst loading equipment causing a lump to appear and bruising under the right eye.
The Golden Hour was applied and there was no lost time.

Ash
Cat A Spad 3S83 (GD1331)
Guildford SB reported that 3S83 has passed ground position light GD1331 without authority.
The train passed the signal by 18-20 foot.
The signal should have been displaying a red aspect, but the driver disputed this and upon visiting the location the following day, the driver manager identified that sunlight had been affecting the signal giving a false aspect.

2P20 CAT A SPAD and Station Overrun
GSMR Rec call sent by 2P20 to report that the train would be unable to stop due to poor railhead conditions.
The emergency brake had been applied.
The train came to a stand 6 coaches beyond the signal and the barriers were not fully lowered.

Maiden Newton / Dorchester West
Staff Accident
A member of Outer Pway was un-keying e-clips using a hammer, when one of the keys ricocheted off of the top of the boot off a member of staff put behind the key to block it and left a mark on their left shin.
Once they had been assisted back to the van, a cold compress was applied.
The Golden Hour was applied and no lost time was anticipated.

Surbiton
Staff Vehicle Collision No Injured Parties
A Works Delivery vehicle had been parked on Upper Brighton Road whilst a team worked on the line.
Upon their return, they found that the vehicle had rolled back and collided with a short term hire vehicle.
There was light damage to the Network Rail vehicle.
The hire vehicle had a large dent on the near side rear panel and the rear near side window had also been damaged.
It was unclear if the handbrake had been applied correctly.

Wimbledon
2L10 Derailed
As 2L10 from Basingstoke to London Waterloo crossed from the Main line to the East Putney (LUL) lines, the rear coach derailed on LUL (TFL) infrastructure.
The derailed coach travelled approximately 150 yards until the train came to a stand beside the Wimbledon Waitrose and under a signal gantry.
There were approximately 300 passengers on board who were full evacuated 2hrs 15 mins later.
There was 1 reported injury.
This incident had a good response and the line was open 2 days later.
Early indications point toward the track spreading under load.
LUL (TFL) will be conducting the investigation into this event.

Eastleigh
Possession Irregularity – Item 60
The ODM reported that an irregularity had occurred within item 60 (Worting Jnc to Eastleigh East Jnc (2250 – 0500)) where Outer DU staff were working on 422 points within Eastleigh East Yard, but outside of the worksite.
There were further reports that an RRV being had also been involved.
01:21 – The On Call level 1Eastleigh P-Way manager was appointed IO, due to the ODM managing another item 60 incident involving a PICOP communication oversight between himself and Basingstoke.

Clapham Junction
CAT A SPAD 9M48
Wimbledon SSM reported that 9M48 had passed W304 signal at danger within platform 2
Upon receipt of the RT3189, it was established that the Driver had pressed the TRTS and then departed without realising the signal was at red.
The train was brought to a stand by TPWS, one coach length beyond the signal.
The Driver made no allegations against the signalling or the unit.
(Note: Upon further investigation it was identified that a fault had been outstanding on the Automatic Route Setting which would normally set the route for a train when the TRTS is pressed).
