Woking Junction
2P11 CAT A SPAD (WK375)
2P11 passed WK375 at danger by a third of a coach length after the Driver misread the signal on the gantry.
There was no allegation against signalling or rail head.
This incident is subject to a Level 2 investigation.

Woking Down Yard
Derailment Engine 6Y55
The DB Cargo Shunter at within the down yard reported that a locomotive on 6Y55 had derailed within No 2 road.
Upon investigation it was established that the sand / aggregate pile had compacted into the check rail and allowed the locomotive wheel to ride over and derail.
The incidence occurred on Network Rail infrastructure.
This incident is subject to a Level 1 investigation.

Staff Accident
Whilst fitting an AWS electro magnet on WK608 an S&T member of staff trapped and crushed their right index finger.
The Golden Hour was applied and there was no lost time.

Winchfield
Staff Accident
A member of staff from Woking D&P twisted their ankle by the substation.
First aid was given on site and the Golden Hour was applied.
This accident is subject to a Level 1 investigation

Waterloo
Worksite irregularity, Item 29     
ES for WON 34 Item 29 reported that a Wessex Capacity member of staff was working alone without a COSS present.
Upon being approached, the individual advised that he had signed in however; this was not reflected on the COSS paperwork.
This incident is subject to an IP investigation.

Weybridge
6J06 Oil Leak
The TQS working in item 41 (Berrylands – Byfleet Jn/Byfleet and New Haw – Effingham Jn – Addlestone Jn) reported that 6J06 on the up fast line country side of the station had an oil leak.
100 litres of biodegradable oil was lost.
2 staff were affected by the leak, an accident form was generated and the Golden Hour was applied.
This incident / accident is subject to a Level 1 investigation
