Item 48 Irregularity
A Machine Controller working for Ganymede Solutions left an RRV unattended a couple of times within a T3 engineering work possession and carried out movements without the appropriate authority of the Engineering Supervisor.
This incident is subject to further investigation. |

Road Traffic Accident
An Outer DU Welder reported that he had an RTA on 08/01/2018 whilst approaching a roundabout at the M27 junction.
A vehicle in front stopped unexpectedly causing him to swerve at the last minute and clip the rear bumper and wheel arch of the other car leaving a scuff mark.
There were no injuries.
The Golden Hour was not applied due to the late report. |

Near Miss 5R05
South Western Railway empty service 5R05 reported a near miss with a team of 3 S&T on the Up Windsor Fast line London side of the station.The S&T had been called out to a failure on 663B points situated within the Up Windsor Slow, but during the fault finding process the team begun to focus their attention on the point machine situated between the Up Windsor Slow and Up Windsor Fast putting them in danger of approaching trains.
Having been conscious to rectify the fault, the team overlooked further adequate protection resulting in the Near Miss.
The Golden Hour and Chain of Care was applied to all parties in an exceptional manor.
The incident is subject to further investigation.

Staff Accident
Whilst selecting concrete sleepers within the yard, a PWay Team Leader caught the ‘dunnage’ (wood used to secure the storage of sleepers), resulting in one of them stacked 2 high to fall and trap his ankle against an adjacent sleeper pile.
The injured party attended hospital for an x-ray as a precaution, but apart a graze and bruising, there were no broken bones.
The Golden Hour was applied and this became a Lost Time Injury. |

Staff Accident
Whilst walking to site, a Strapman working within T3 possession item 7 dropped a short circuiting strap onto a live conductor rail causing a tripping and flashover.
The flashover caused arc eye and he attended hospital where he was administered eye drops.
The Golden Hour was applied and this became a lost time injury. |

Generator Fire
Stray sparks caused by grinding within T3 possession item 98 caused a generator to catch fire.
As the fire subsided, extinguishers were then used to put it out.
There were no injuries.

Staff Accident
A PWay Trackman carrying out back filling on a sleeper change within a T3 possession lost his footing stepping over a conductor rail and twisted his right ankle.
He was taken back to the Clapham Yard Maintenance depot and attended the Hospital as a precaution.
The Golden Hour was applied.
Non lost time injury. |

Staff Accident (RTA)
A Technician from Clapham PWAY was reversing a Network Rail vehicle within the depot and had a low speed (2-3 mph) collision with another car.
There were no injuries, although the Golden Hour was applied for chain of care. |

Staff Accident
A Lineside Asset Engineer fell on muddy ground and injured their right ankle.
They were to hospital by ambulance and an x-ray found that they had sustained a multiple fracture, ligament damage and a dislocation.
The Golden Hour was applied and this became a lost time Injury. |

Staff Accident
A contractor working for IP Track was walking on sleepers within a T3 possession when they slipped and fell causing a graze to their right arm.
They were given first aid to deal with the graze and there was no lost time.

Staff Accident
A contractor working for IP Track was crossing the track within a T3 possession to retrieve a disc cutter from the stores, but slipped on a sleeper due to ice and sustained a bruised and grazed right arm.
First aid was refused and there was no lost time.

A contractor working for IP Western & Wales was replacing a concrete sleeper within a T3 possession and as it was being lowered into the cess, the contractors foot was trapped.
The Injured Party was taken to hospital and x-rays showed slight swelling.

A Contractor working for IP was installing a conductor rail within a T3 possession , but it slipped off a pot due to not sitting correctly and caught the Injured Party’s finger between the ramping machine and the rail.
First aid was administered on site and the Injured Party was taken to hospital as a precaution.
The injury was x-rayed and apart from swelling, there were no broken bones.
There was no lost time.

A member of Colas S&T working for IP suffered an injury to their hand after trapping it under a troughing lid whilst putting cables away within a T3 possession.
The Injured Party attended hospital where they had treatment for a broken left ring finger.
They were able to return to work carrying out light duties.

Irregular Working / Red Bardic Left Out
Havant Area Signalling Centre received a call from the Booking clerk querying if the T3 possession (Item 123) had been given up, as there were Red bardic lamps left out at the Level Crossing.
Upon investigation it was identified that a member of Possession Support Staff had not collected them after a breakdown in communication.
This incident was subject to further investigation. |

Possession Irregularity Item 505
South Western Railway service 5T14 reported a very loud bang approaching HT59 signal on the down line.
It was identified that a member of Possession Support Staff had overlooked recovering one of the three detonators that had been placed after a T3 possession.
This incident was subject to further investigation. |

Irregular Working 2135 pts/1A36.
Woking Distribution and Plant reported that whilst working on points heating, one of the heating strips was left unsecure for the passage of South Western Railway service 1A36.
The train struck the strip, but there was no injuries or infrastructure damage.
This incident was subject to further investigation. |

Debris blown on track
Station staff reported that part of the station roof had fallen onto the track due to high winds and South Western Railway service 1W36 had struck it.
Fortunately the section of roofing was a Perspex glazing panel which caused no injuries or further damage.
Upon investigation it was established that the material holding the panel in had become rotten and the inspection regime was reviewed.
A safety net was installed. |

A PWay Team leader trapped their left middle finger within a sliding van door.
First aid was administered and the Golden Hour applied.

Staff Accident
Whilst walking back from site to the van, a Senior Technical Officer slipped on some wood hidden underneath the ballast.
He sustained a displaced fracture and tore both ankle ligaments which required the ankle to be re-set and pinned.
The Golden Hour was applied and it was likely that the Injured Party would be off for between 6-12 weeks. |

A Mobile Operations Manager had a low speed Road Traffic Accident at a roundabout when car drove into the side of his van.
On investigation it was established that there had been confusion over who’s right of way it was on behalf of the other driver.
The Golden Hour was applied for chain of care.
There were no injuries sustained. |

5B07 Struck Possession Board
South Western Railway service 5B07 reported striking what was described as a Possession Limit Board on the reception line in the vicinity of E914 signal.
Upon inspection, the board said ‘Stop check the points are set correctly before proceeding’ and had been used by IP as part of a T3 possession.
This incident was subject to further investigation. |

Staff incident – Rotten Timber Decking
A Nash Contractor reported that a member of staff had fallen through rotten timber decking and bruised their left leg.
The Golden Hour was applied and there was no lost time.
Post incident was identified that the decking had been banned from use but not signed as such site.
The work to replace the decking had also been postponed due other emergency work requiring priority.
Temporary signs have since put in place and the area sprayed to reflect the danger. |

Possession Incident, Item 33
A Shift Manager for IP track working within T3 possession item 22 reported that whilst propelling a train up the Eastern docks branch, a track panel on the train was out of gauge and had pulled E842 signal out of its base and caused cable damage.
The signal and cable was later repaired.
It was established that the panel not been loaded correctly before the movement took place.
This incident was subject to further investigation. |
