Staff Accident
A member of Works Delivery staff was changing sleepers when they slipped and fell backwards onto a rail jack.
The Injured Party sustained a graze and no first aid was required.
The Golden Hour was applied and there was no lost time.

Portsmouth and Southsea
CAT A SPAD 1P59
1P59 passed red signal by 1 coach length.
The Driver made no allegation against the signalling equipment or unit, but stated he was “Reading ahead”.
The Driver did not indicate that they lost concentration.
The train was brought to a stand by TPWS.

Staff Accident
Whilst on W231 signal gantry, a member of S&T missed a step resulting in an ankle sprain.
The Golden Hour was applied and no lost time was anticipated.

Severe arcing over point work
2J53 reported severe arcing going over the point work and upon coming to a stand at Berrylands, a fire then started under the front coach.
The train was evacuated.

Staff Accident x2 (RTA)
Two members of Works Delivery staff were involved in a head on collision.
Upon attending hospital, one was sent home but the other had a fractured rib cage and a chipped pelvis.
The Golden Hour was applied.

Staff Accident
A team of PWay staff were turning a piece of rail over in the four foot within an engineering T3 possession, but whilst doing this a piece of ballast impeded the rail and it fell back over onto its side, landing onto the left foot of a member of the team.
The foot was examined at hospital which identified that it had been bruised.
The Golden Hour was applied and there was no lost time.

2E52 Stopped Short in Platform 3
2E52 stopped short in the middle of platform 3 resulting in the train not being fully accommodated by 2 coaches.No doors were opened.

Guildford / Farncombe
Possession Irregularity Item 95
The Engineering Supervisor handed back their worksite to the PICOP after arranging to remove 2 sets of straps on the Up Main, but had overlooked two others on the Up and Down Ash at 30m35ch.
The PICOP had not been aware of the oversight, so authorised the Electrical Control Room Operator to close the breakers which resulted in a short circuit. There was minor damage.
Upon further investigation, it was found that 6 sets of straps were left on the track.

12 Car 450 signalled into Platform 5 – 1P44
1P44 was signalled into platform 5 and the driver has accepted the route in error.
Arrangements were made for the Guard to open the doors from the 8th coach via ASDO, but all 12 coaches released.
No-one was seen to exit the train and arrangements were made for the train to carry out a shunt.
The On Call Guard and Driver Managers were advised and followed up the incident.
The S&T checked the beacon and it was working as designed

Possession Irregularity Item 79
A Raynes Park Electrical Control Room Operator gave permission to the PICOP to test and apply a strap within item 79 on the Up and Down Epsom lines at 8m60ch at Raynes Park.
The test found the conductor rail to be live after the ECO had failed to open 2 circuit breakers.

Accident Report
Whilst driving a Network Rail vehicle from Clapham Yard to Waterloo, a PWay track operative was involved in an RTA when a tow truck moved from outer into the inside lane with no warning causing.
Undercover officers witnessed the event and paramedics were called.
The Operative then began to experience pains in his right shoulder and he was advised to see his GP in the morning.
The Golden Hour was applied and no lost time was anticipated.

2P40 12 car in Platform 6
2P40 was routed as booked into platform 6, but the train was formed of a 12 coach 450 unbeknown to the Signaller.
The front 10 coaches were accommodated and a door release was offered for just those 10.
The On Call Driver and Guards manager were advised.

12 car 450 stock signalled into platform 7 – 5G64
5G64 formed of 12 car 450 stock as per diagram, was signalled into platform 7 in error vice 13 due to the late arrival of 2P62 required to attach to 1W68.
The On Call Driver Manager was advised and the Driver was relieved.

6Q66 Route Prohibited
The Eastleigh SSM reported that a Matisa Tamping Machine (6Q66) had traversed the Down Line between St Denys and Woolston in error. Matisa Tampers are restricted from using this route.

Wylye Level Crossing
Staff Accident / Staff Incident
Whilst putting cable into troughing, a team of Works Delivery staff returned to a place of safety for a train to pass at line speed.
The vortex of the train resulted in a hard hat being blown off of one of the teams head and striking another with force on the bottom of the shin on the left leg causing discomfort and bruising.
Upon investigation it was established that the helmet had blown off due to a woolly hat being worn underneath.
The Golden Hour was applied and no lost time was anticipated.
This incident is subject to further investigation.

Staff Accident
A member of PWay staff experienced pain in the left of his abdomen whilst carrying out a sleeper change and dispersing of ballast with a shovel by means of a twisting action.
The Golden Hour was applied and no lost time was anticipated.

Salisbury
Suspect Cat A SPAD – SY240 Ground Position light (Late report)
A fault was reported on Wednesday 21/2/18 by the Salisbury Signaller regarding track circuits flooding (failing) following a shunt move beyond SY240 signal involving 5L56.
The S&T have since investigated this and confirmed that they believed a Cat A SPAD to have occurred as no route was set and SY240 signal never displayed a proceed aspect.
The Forward Facing Camera was downloaded by SWR which proved that a SPAD did occur in line with the S&T evidence provided.

Aldershot North Junction
Staff accident
Whilst unloading/loading an RRV within a work site, a PWay technician caught his leg on the spike used for speed boards and grazed his shin.
The Golden Hour was applied and this became a non-lost time.

Waterloo
Signaller error
Post a regulation error at Waterloo, the Signaller requested that the Driver should change ends and return to Queenstown Road.
Due to a breakdown in communication and a clear understanding not being reached, the Driver began to carry out the movement without first checking with the Signaller once they changed ends.
Upon investigation, it had appeared that the Signaller had not been specific with their instruction.
This incident is subject to further investigation.

Staff Accident
A member of Works Delivery staff was lifting the sleeper end by hand when they felt their back spasm.
The Golden Hour was applied and the injured party was able to return to work the following day.
This incident is subject to further investigation where the manual handling technique used will be visited.

Raynes Park / New Malden
Allegation of unsafe activities lineside
South West Railway service 2G37 reported a member of staff in full PPE walking towards Raynes Park in the Down Slow Cess who did not acknowledge the horn.
A further service (2F37) reported a similar event in the vicinity of a Goals football pitch where the Driver also blew the horn with no acknowledgement. It was also observed that the member of staff was writing things down and texting on a mobile device. After a few more blasts on the horn, they then acknowledged the train.
2K37 was cautioned with a view to stop and identify them, but no-one was seen.
The Safe System of Work was found and the individual was identified.
This incident is subject to further investigation.

Staff Accident
Whilst squaring up concrete sleepers, a member of PWay was struck by a jack on the shin which caused bruising.
The Golden Hour was applied and there was no lost time.

Staff Accident
A Gannymead Operative was digging up ballast with a shovel as part of a sleeper change.
Whilst doing so, he lost his footing and turned his right ankle.
His ankle became badly bruised and required a hospital visit.
The Golden Hour was applied and lost time was anticipated.
