  1. The Slade Green Electrical incident information really makes my blood boil each time I read it. The lack of mention of the importance of; testing the Live Line Tester, immediately before and immediately after testing the Conductor Rail is extremely frustrating.
    The use of a proving unit, to carry out the pre and post test verification of correct function of the Live Line Tester has not been stated, despite its clear importance.
    Also the Slade Green incident doesn’t add up, there is information missing in all of the safety bulletins about using a Proving Unit. The safety bulletins state anecdotally, that the Live Line Testers were faulty, yet if a working Proving unit had been used to test the Live Line Tester ( LLT ) before testing the conductor rail, the LLT would have been found to be faulty and not used.
    The fact that a “faulty Live Line Tester” was supposedly used beggars belief and worse still work commenced on the basis of an incorrect line Dead indication from a faulty Live Line Tester set, suggesting that a proving unit was never used to verify the Live Line tester before and after testing the conductor rail.
    I am not an electrician but have a better than average knowledge of Electricity from my previous work experience, the way I see it; Network Rail is missing a golden opportunity to force a message home, about the absolute importance of the use of a Proving unit as part of the verification of Conductor Rail isolation status.
    Even the latest bulletin from Simon Morgan ( in the above list of bulletins ), fails to mention a proving unit’ use as part of the LLT testing procedure, although it is in the picture which is a slight step forward from the initial safety bulletins.
    The lack of mention of the importance of using a proving unit as part of the LLT correct use procedure, is sending a implicit message that its use is irrelevant and not necessary. It is therefore unfortunately likely, that as a result, there will sadly be further ( potentially avoidable ) incidents of this type in future.

    1. Hi Ivan, thank you for your comment. Very good point about the missing details around the use of live line tester and proving unit, I will follow this up with the Safety team. A new strapping video has been produced (link in the top menu) which should reiterate the correct process to follow, hopefully this may help reduce such incidents in the future. Regards, Adam
