  1. Please find incidents below from the past week:
    21/02/2016 Andover – ODM reported a possession irregularity connected to item 76 where the Blockman had placed the PLB protection on the Down vice Up Exeter line at 67m 67ch.
    A Tamper which had been travelling on the Down line came across the protection and stopped short of it.
    Early indications show that the person had lost their bearings and had not checked where they were before applying their protection.
    24/02/2016 A member of Wessex Asset team reported having a car accident when he was trying to join the A31 from a side road.
    He was shunted from the rear by another vehicle whose driver was not concentrating on what was in front of them.
    No injuries were sustained.

  1. Please find incidents below from the past week:
17/02/2016 Petersfield – A Signaller was walking past an open locker box in the signal box when they caught their right leg above their knee on a padlock clasp holder breaking the skin and bruising.
18/02/2016 Wool – A Pway Operative was on a bance (machinery for track chair screws) for within the specified time of up to 15 when he reported a back ache to the COSS. The Operative was taken off duty and it required a GP visit.
19/02/2016 Wool – A member of Works Delivery was stacking up sleepers within a possession with sleeper dogs when his hand slipped trapping his middle finger on his right hand. He did not think much of it at the time, home but later had to go to hospital with the onset of a lot of pain due to him having fractured/ broken it.
19/02/2016 Basingstoke Capus – Control Centre Technician at was waiting at barriers leading into car when the traffic light changed to green and he drove forward.

The road barrier had not fully lowered and the underside of his car struck it.  The Individual was driving his personal vehicle (which was damaged) but he sustained no injuries.

19/02/2016 Raynes Park Station – A member of Electrical & Plant was carrying a transformer over the tracks when his glove slipped.

He tried to re-adjust his grip but as he did so, his foot became caught and his chest landed on the running rail.  The individual continued working but was in a lot of pain.

After a hospital visit the next day he was told that he had cracked his rib/s.

19/02/2016

Waterloo – Works Delivery Manager reported an accident which had occurred on Monday 15/02/16 where he had been stepping down 2 steps from a Clapham P-Way cabin. By the steps were 2 round ferrels (a part connected to base chairs) which he was not aware of.

As he stepped on them he ell backwards injuring neck and finger requiring a hospital visit.

07/02/2016 Barton Mill access gate – A Technician was attempting to lock an access in windy conditions when the gate flew open catching his left hand finger.
07/02/2016 Salisbury Station Platform 3 – ISS Labour trapped their right foot under the track after the jack was dropped. It had been raised to fit plates.
07/02/2016 White Hart Lane Level Crossing – Member of public trapped within the barriers and the Crossing Clear pressed for 2O41.
Signal replaced, barriers raised.
10/02/2016 Byfleet and New Haw Station – A member of PWay staff was carrying stressing kit down the station stairs and tore muscles in his back due to the weight of the equipment.

    Date Details
    01/02/2016 Witley / Haslemere – A member of Havant PWay walked up some steps to open an access gate at Brook Gardens (WPH1 40m 20ch) when he slipped on wet mud as he opened the access gate. He then fell over and landed on to some abandoned troughing outside the gate and strained an injury to his back.

    He was able to resume duty and the abandoned troughing was Close Called via the App.

    02/02/2016 Clapham – A blockman for item 9 placed detonator protection within Clapham Yard (6 Road) vice the Down Windsor Slow.

    A train within the yard then ran over them.

    02/02/2016 Upton Lovell (Warminster Line) – A Welding and Grinding Technician was pushing a trolley loaded with welding equipment on a gradient with a colleague within 121 when his back gave way causing a sharp pain.

    (Possibly a sprain).

    02/02/2016 Portsmouth and Southsea Down Carriage Sidings – A member of the PWay was unloading tools from a van when they tripped on a wire concealed by leaves.
    He put out his hand out to break his fall and hit a key housing which twisted his middle finger on the right hand.
    First aid was given but a hospital visit was also required.
    06/02/2016 Guildford Station – Member of Guildford Off Track Operative was assisting with erecting concrete shuttering underneath a make shift shelter.

    A large gust of wind knocked over the shelter made of Tarpaulin and Wood and the Operative was struck on the head causing a 5-7 inch gash.

    First aid was applied and they were taken directly to hospital where the gash was glued together.

    06/02/2016 Romsey / Dunbridge – The Salisbury MOM was involved in a road collision where their Network Rail car then rolled.
    The MOM sustained a cut to his hand where he got out of the vehicle and suffered from shock.
    The car was a right off.
    06/02/2016 Salisbury  – Staff working in Item 108 ruptured a gas main within platform 2.
    The Project Manager reported that a CAT scan was carried out prior to works, but was not detected.
    The gas main was not listed within the Buried services list or Hazard directory.

  1. Please find incidents below from the past two weeks:
    20/01/2016 Tisbury Quarry Level Crossing – The Local Operations Manager was advised by the Salisbury Signaller he had given permission to a Member of the Public with a car to cross, but upon doing so the Level Crossing Attendant reported that this had not been enough time before the train passed.
    Although the Signaller acted within his remit, it is clear that a risk assessment is required to implement a more suitable method of working.
    21/01/2016 Branksome – Circuit breakers E108 & 109 both tripped on short circuit whilst staff were next to them. The force of the noise caused ringing in the ears.
    21/01/2016 Wimbledon – Member of Wimbledon S&T reported observing what he believed to be unsafe working.
    Upon a site inspection and taking into account the amount of staff working within it, their plan and paper work was inadequate.
    There was no provision either, for the St Hillier lines.
    24/01/2016 Northam Depot Reception Line – ODM reported a possession irregularity within WON 44 Item 32 where a contractor working for Morsons had operated the wrong hook switch E8401 vice E8390 in error.
    25/01/2016 Tavells Lane Level Crossing – A member of Southampton S&T’s foot slipped down the side of a rail and they twisted an ankle. He was able to walk on it, but the twist was quite severe. He did not require a hospital visit
    26/01/2016 Waterloo South Sidings – 5J76 became gapped entering no. 2 sidings having been routed into the engineers road via the C&P points half way down.
    This was the first time the South Sidings had been used since Item 5, 25th Jan 1100hrs – 1230hrs was given up.
    27/01/2016 Havant Station – A member of the Eastleigh Works delivery team were stepping over the running rail when he caught the rail with the bottom of his boot. His foot has landed awkwardly and he twisted his ankle. He did not attend hospital at the time, but was expected to have time off work as a result.
    27/01/2016 Chandlers Ford Station – 4O11  struck a large tree on the single line. The train went straight through the middle at the 77mp and debris became stuck under the 6th and 7th waggon. The tree had also landed on top of a location cabinet, but o signalling was lost.
    28/01/2016 Marchwood – LOM within the signal box reported that 6B93 had SPAD MW12 signal at danger.  The Driver did not dispute the signal, but had observed that the route was set and assumed the signal was OFF.
    The signal had been at danger due to a failure.
    29/01/2016 London Waterloo Station – 5Y01 passed W6 signal on the UMR at danger.
    The Driver misread W4 and thought they had been signalled into platform 15.
    The train bought to a stand by TPWS 1 coach past the signal.
    29/01/2016 Guildford Station – 2N18 had a CAT A SPAD at GD931 and came to a stand after departing the station. It was unknown at the time and the incident disputed on the Driver thinking that they had a change of aspect.
