Item 5 Line Blockage Irregularity Waterloo Platform 20 was under line blockage as part of T3 possession item 5. The Line Blockage had been taken correctly, but upon giving it up the incorrect authority number was quoted on 3 occasions. Upon reviewing the paperwork, it appeared that the COSS had written the correct authority number on the top of their form, although the 6 was not clearly defined. The COSS then wrote a 0 instead of a 6 where the authority number was meant to be recorded thus quoting the wrong number when giving up. |
