Hilsea
Staff Accident
A member of Outer PWay was using an iron man and whilst releasing the ratchet, it had spun around and caught the lower part of the right arm.
The Golden Hour was applied and there was no lost time.

Feltham
Crane collapse in the Lidl car park
A crane collapsed within the Lidl car park next to Feltham Station connected to Network Rail upgrade works being carried out by Contract Staff working on bridge 3/50. There were no reported injuries or infrastructure damage.
All the paperwork was said to be correct (ie lifting plans etc).
The incident was reported to HSE and is currently under further investigation.

Worplesdon
Staff Accident
An Inner PWay Operative was walking on ballast which then gave way under their right foot causing them to fall to the ground bruising, grazing and cutting their left leg.
First aid was administered and the Golden Hour was applied.
There was no lost time.

Yeovil Junction
Cat A SPAD
5L51 had a Cat A SPAD at SE4733 Stop Board, passing it by 91 yards. The Driver advised that the Signaller had advised them to pull the train all the way down the sidings as an attachment was coming in on top. The Signaller disputed this, as the instruction had not included passing the Stop Board where further authorisation was required.
This incident is subject to further investigation.

Waterloo
Item 5 Line Blockage Irregularity
Waterloo Platform 20 was under line blockage as part of T3 possession item 5.
The Line Blockage had been taken correctly, but upon giving it up the incorrect authority number was quoted on 3 occasions. Upon reviewing the paperwork, it appeared that the COSS had written the correct authority number on the top of their form, although the 6 was not clearly defined. The COSS then wrote a 0 instead of a 6 where the authority number was meant to be recorded thus quoting the wrong number when giving up.

Feltham
Staff Accident
A member of Inner Maintenance S&T was unloading light tools from a works vehicle into the S&T store when they noticed pain in their middle finger and saw a minor cut / abrasion.
The wound was cleaned, First aid and the Golden Hour was applied.
It could not be established what cut the individuals hand, but they had not been wearing gloves at the time.
