Incidents since last post

There was excellent work carried out by all concerned during the wintery weather conditions.
The Teamwork demonstrated by Train Crew and response Teams amongst others to clear the line of trees, debris, snow from platforms and points was second to none.
Thanks to all of you who made it into work to keep the railway running and our offices responsibilities functioning.

28/01/2019 Bentley / Alton Line Blockage irregularity
A Team of 3 Inner Maintenance Ultrasonics staff were working between Bentley and Alton within a planned Line Blockage (GZAC 6115047). The Team had just finished their work and were heading back to an access gate when a train approached and blew the horn. The Team were able to stand clear in plenty of time. Although the Line Blockage had been between Bentley and Alton, the Signaller had thought the team were working within the Bentley loop and had ran the train in error. The Signaller was relieved of duty. This incident is subject to further investigation.
28/01/2019 Godalming Road Traffic Accident
A Senior Technician from Woking Telecoms reported a minor road traffic accident in Godalming when a South Coast Ambulance struck and broke the Drivers side wing mirror. There were no injuries and chain of care was carried out. The damage was reported to Fleet.
31/01/2019 New Malden / Shacklegate Junction Possession irregularity
DB Cargo reported that an Operator on board Autohopper 6G26 within T3 possession item 160 (New Malden / Shacklegate Junction) was not happy to continue to work due to multiple issues.
  • There were a lack of dust masks
  • The COSS speculated to have to leave site to collect a van from Kensington
  • Whilst unpinning wagons to drop stone, the loco began to move
  • The ES was within the loco.
The Operations Delivery Manager was appointed Investigating Officer and an investigation then took place.
31/01/2019 Chertsey / Egham Possession irregularity
T3 possession item 178 between Staines and Ascot was estimated to overrun by 20-25 minutes due to grinding that was required. Upon the possession being given up at 05:17hrs, it was established that F300 signal had not been keyed back to auto working due to the Hand Signaller forgetting to do so. Trains were cautioned passed the signal at danger (red) until 05:59.
01/02/2019 Salisbury Road Traffic Accident
A Welding Team Leader from Eastleigh was driving up the slope into the Salisbury Pway depot in a Network rail van and when trying to turn the corner, the vehicle went straight on in the snow and hit the fence. There were no injuries sustained and chain of care was carried out. Fleet were advised that damage was sustained to the Driver’s front wing and part of the bumper was pulled off. The fence damage was also reported.
02/02/2019 Liss SWR Cat A SPAD
The Driver of SWR empty stock 5T94 Basingstoke to Fratton (which had been diverted via Woking owing to an icing incident at Basingstoke), reported having a Cat A SPAD at PF229 signal. The Driver advised that they had been following a stopping service (1P29) on single yellows for quite some time and had become caught out expecting another yellow vice red when 1P29 stopped at Liss station. This incident is subject to further investigation.

Incidents since last post

17/01/2019 Staines Staff Accident
A Feltham S&T Senior Technician was re-terminating a cable at an awkward position within F252 signal. Whilst applying the nuts, they felt a sharp pain and noticed that their finger on their left hand was bleeding severely. It was unknown as to what broke the skin. First Aid was administered and the Golden Hour applied. There was no lost time.
17/01/2019 Eastleigh Staff Accident
A Telecoms Manager was erecting a screen within the office. The top part of the screen became loose causing the screen to fall onto their hand / wrist. The Injured Party sustained a cut and bruising. The Golden Hour was applied and there was no lost time.
18/01/2019 Surbiton Report of Unsafe Working
After receiving advice of a potential trespasser, an Inner Mobile Incident Officer (MIO) attended site and found a Vodaphone employee working on the behalf of Mitie behind the old signal box at Surbiton within a caged Vodaphone cabinet. The individual did not have a Sentinal card and was not wearing the full PPE whilst trackside. The MIO escorted them from site, contacted Mitie and Close Called the incident. The Route Workforce Health, Safety and Environment Advisor followed this incident up locally.
20/01/2019 Aldershot North Junction Possession Irregularity
A Signaller on Woking Panel 3  reported that they had granted a Line Blockage which overlapped with T3 possession (item 155). The Signaller had believed that they were able to utilise the exit signal of the T3 to protect the Line Blockage under a back to back arrangement, but had not clearly established that this was not possible. The error was realised when the Controller of Side Safety (COSS) for the Line Blockage reported seeing the T3 Possession Limit Board within their block. It was noted within this incident that the Line Blockage had also been planned this way and had not taken into account the conflicting T3 possession. This incident is subject to further investigation.
22/01/2019 Woking Staff Accident
A Woking PWay Technician was walking in the cess on the Down Slow near Woking and upon crossing into the 4ft, they caught their foot on a cable, fell, twisted their ankle and banged their shoulder. The Golden Hour was applied and this became a lost time injury.
25/01/2019 Wimbledon Staff Accident
The Local Operations Manager Response Inner was trying to remove a cable tie with a pair of scissors when the scissors they were using followed through and cut his right hand. Their hand became quite swollen and bled quite badly. A dressing was applied. The Golden Hour was carried out and post visiting hospital, a butterfly stitch was applied. There was no lost time.
27/01/2019 Guildford Staff Accident x 4 – (Road Traffic Accident)
Four members of Guildford PWay were involved in a road traffic accident on the A3 near Guildford travelling approximately 65mph. The weather conditions were wet and a car ahead of them in lane 1 was travelling very slowly between 5-10mph. The van closed very quickly due to the difference in speed. The Network Rail Driver turned into the next lane to avoid a collision but lost control and the van spun around slamming into the central reservation barrier. All on board were ok, but there was extensive damage to the van which needed to be recovered. The Golden Hour was applied and all staff involved remained ok.
27/01/2019 Havant Staff Accident
A late reported accident was received at about a Ganymede Contract member of staff who was assisting an Outer Maintenance Welding Team to remove equipment from a T3 possession. The Contractor was seen to slip over on a cable, but got up straight away and appeared not to have suffered any injuries. Subsequently it was found that the Contractor had sustained a cut and swollen knee. The Golden Hour was applied once the accident was known about and no lost time was anticipated.

09/01/2019 Wokingham Junction / Bracknell Staff Accident
A member of S&T Works Delivery staff was walking in the Down Cess to site of work between Wokingham Junction and Bracknell. Upon walking through an area that was overgrown, the member of staff fell into a section of troughing route that had a missing lid hurting their knee. The Golden Hour was applied and although the knee was sore, there was no lost time.
13/01/2019 Eastleigh South Junction Possession Irregularity
Eastleigh Works Delivery PWay reported that a possession irregularity had occurred having located a rail that they were about to remove on the Up Portsmouth between 74m 5ch and 74m 26ch, had isolation straps applied to it.
The straps have been placed at the wrong location (74m22ch vice 74m29ch as planned) at the time the worksite was set up.
This incident is subject to further investigation.
13/01/2019 Walton-on-Thames Staff Accident
A member of PWay Works Delivery Track staff was using a Setter for installing track nylons and whilst leaning against it with some force, they damaged their shoulder muscle ligament and found it difficult to lift their shoulder without quite a lot of discomfort and pain. The Golden Hour was applied and the Injured Party was able to return to work, albeit on light duties.

Incidents last week

13/12/2018 New Kew Junction Fallen tree – securing ropes snapped
Environmental Forestry working within a Line Blockage on the Kew East Spur reported that a section of tree had fallen onto the line after their securing ropes snapped. There was no reported injuries or damage to the infrastructure.
16/12/2018 Windsor and Eton Riverside TPWS Grid damaged during T3 possession item 168
Colas Rail S&T reported that the Train Protection Warning System (TPWS) grid for F286 signal within platform 1 at Windsor and Eton Riverside had been damaged whilst a rail was being lifted. Feltham S&T attended to rectify the damage.
16/12/2018 Botley Damaged Cable during T3 possession item 109
The Eastleigh Electrical Control Room Operator received a call from the Engineering Supervisor of T3 possession item 109 to report that a tamper had clipped a cable at Botley between 483A and 483B points. The cable had become exposed and a temporary repair was arranged.
24/12/2018 Longcross Category A Signal Passed at Danger – 1C22
The Driver of SWR 1C22 09:00 Reading to London Waterloo reported having a CAT A SPAD at F312 signal on the Up Main after departing Longcross. The train passed the signal by 5 coach lengths after the Driver experienced a loss of concentration. There were no allegations against the signal, that had been displaying a red aspect due to SWR 2C22 08:46 Reading to London Waterloo within the section ahead.
29/12/2018 Feltham S&T Depot Staff Accident
An Operative from Feltham S&T was walking down some steps at the Feltham S&T depot at the end of their shift when they missed the edge of one of them. In their motion to adjust, they then felt a sharp pain in their lower back. The incident was not reported straight away, but the pain persisted and a Doctor’s appointment was planned for 31/12/18. The Golden Hour applied for chain of care and this became a lost time injury.

10/12/2018
Raynes Park

Staff Accident
A member of staff from the Inner CRE team was getting PPE out of a Transit van within the Raynes Park Depot when the door slid shut on their right hand little finger.
The Injured Party sustained a cut and some bruising and once a plaster was applied, they were able to continue.
The Golden Hour was applied and there was no lost time.

11/12/2018
Waterloo

Short Circuit
A member staff from Raynes Park Distribution received a report from a Works Delivery Manager at Waterloo that a stack of fishplates had fallen in a ‘daisy chain like fashion’ on platforms 8 and 9.
The fishplates had then come into contact with the negative return and a buffer stop cage and the danger was identified when a gate to the cage sparked when opened.
There were no reported injuries and the incident was Close Called.

Queenstown Road

Line Blockage Irregularity
A Wimbledon Signaller reported that they had signalled engineering train 6J61 out of T3 possession item 30 and onto the Windsor Reversible at Queenstown Road where an S&T Line Blockage was still in situ.
The Signaller had been dealing with a high workload at the time and fortunately no-one had been trackside when the error occurred.
This incident is subject to further investigation.

12/12/2018
Woking

Staff Accident
A member of staff from Woking Distribution and Plant was driving from Woking Station to the depot when a taxi braked abruptly in front of them and then reversed clipping the Network Rail vehicle.
The D&P staff member sounded their horn, but the taxi Driver did not react to it.
Apart from minor damage to both vehicles, there were no injuries.
The Golden Hour was carried out for chain of care.

Brockenhurst

Staff Accident
A member of staff from Works Delivery Signals had a Road Traffic Accident involving a deer whilst driving back from the New Forrest in the Brockenhurst area.
Minor damage was sustained to the vehicle, but there were no injuries to the driver.
The Golden Hour was applied for chain of care.

25/11/2018
Basingstoke
Possession irregularity item 89
Whilst 5W84 was performing a shunt between Basingstoke station and Barton Mill sidings, the train lost the current from the third rail. Upon investigation it was established that the  Person In Charge of Possession had requested an isolation for the operation of Hook Switches without first checking with the Signaller that there were no trains in the electrical section.

Berrylands
Possession irregularity Item 38
Whilst the Person in Charge of Possession was giving up the possession with the Signaller, the Signaller advised the PICOP that ‘MW’ track circuit was still showing occupied on the Up Slow near Berrylands. The track circuit was treated as a failure and the S&T technicians were dispatched.
Upon the S&T arriving on site, they found an engineering trolley that had been left on the line and it was removed with an Emergency Switch Off.
The incident was subject to further investigation and also found that the Engineering Supervisor had not been aware that a trolley had been in use.

New Malden
Staff accident
Whilst walking within the 6 foot at New Malden, a member of Inner PWay stepped on some ballast which moved underfoot and they fell to the ground. As they fell, they twisted their left ankle. First aid was given on site and an icepack was applied back at the depot. The Golden Hour was applied and there was no lost time anticipated.

Clapham Junction
Near Miss – 5V39
The Driver of South West Railway empty service 5V39 reported an alleged Near Miss with track workers working close to their line with no Lookout on the London side of Clapham Junction on the Up Windsor Fast line. It had also appeared that one was pulled by a colleague.
The staff were found to be a team of Inner S&T attending a failure and upon further investigation and on reviewing the footage closely taken from the train, no one was seen to be pulled clear and the incident was not deemed to be as serious as first reported.
There had also been a Line Blockage in place on the Down Windsor Fast, so no Lookouts were required.
Lessons however were learnt and the incident was subject to further investigation.

Lymington Town Level Crossing
Possession irregularity item 102
The Driver of South West Railway service 1J01 05:59 Brockenhurst to Lymington reported that there were red lamps left either side of Lymington Town level crossing.
Upon investigation it was established that they had been left in place in error after T3 possession item 102 had been given up.
This incident was subject to further investigation.

Kew Gardens
Low speed road traffic accident
A member of Inner Electrical & Plant staff was involved in a low speed (5mph) traffic accident whilst driving out of Kew Gardens station with a Mini. Both vehicles sustained superficial damage and there were no injuries. The Golden Hour was applied and there was no lost time.

Lymington Viaduct
Staff accident
A Section Supervisor from Bournemouth PWay was walking across the Lymington Viaduct within the four foot of the single line when one of the walkway boards gave way and they fell through up to their hip. The injured party a graze to their left leg, hurt their knew and also jolted their back.
The Injured Party was pulled out by colleagues and helped back to the van. The Golden Hour was applied and after a visit to the hospital there were no serious injuries sustained apart from soft tissue damage and pain that remained within the knee. This was a lost time injury.

Strawberry Hill
Staff Accident
A member of Feltham PWay was walking along the Down cess near Strawberry Hill when they stood on loose ballast which gave way under foot. As a result the Injured Party twisted their left knee. The Golden Hour was applied and having attended hospital for a check-up, they were advised just to take ibuprofen and no lost time was anticipated.

Staff Accident
A member of Woking Pway was using an access gate from the PWay depot at Woking to access the line via the Down Sidings and stood on some loose ballast which moved underfoot. As a result the Injured Party twisted their right ankle. First aid treatment given on site and an ice pack later applied. The Golden Hour was applied and no lost time was anticipated.

Woking Junction
Staff Accident
A Keltbray Rail Contract Welder working on the behalf of Works Delivery injured their back whilst unloading a trolley from a van at Woking Junction. They sustained a lot of pain in their back and were taken to hospital by another member of staff. The Golden Hour was applied and were prescribed strong painkillers, they were advised to rest for a few days. This became a lost time injury.

Pirbright Junction
Staff Accident
A member of Contract staff from Resourcing Solutions working on the behalf of Eastleigh Works Delivery injured their left shoulder when fitting a new points motor to WK2260 points at Pirbright Junction. The injury was sustained when they were being assisted by another member of staff who slipped whilst lifting the point motor into place. The Golden Hour was applied and an ice pack was used. No lost time was anticipated.

London Waterloo
Possession irregularity item 6 (part 2)
At 10:52 the Driver of South West Railway empty 5Z13 10:19 London Waterloo to London Waterloo reported a loss of line light within Platform 21 at London Waterloo.
Upon investigation the Mobile Operations Manager (MOM) was asked to check TIS 8566 by the Raynes Park Electrical Control Room Operator (ECRO), which should have been closed at 03:50 when T3 possession item 6 (part 2) was given up.
On arrival, the MOM found it still to be open and closed it upon being authorised to do so by the ECRO.
This incident was subject to further investigation.

Eastliegh
Staff accident
An Operative from Works Delivery Eastleigh was driving a hire vehicle to refuel it along Arnold Road, Eastleigh.
Upon turning left into Argosy crescent; they saw another car coming toward them around a parked car and braked to give way to them.
A car that had been following the Operative did not react in time resulting in a rear end shunt at approximately 5mph. Both vehicles suffered some minor damage and the Operative experienced some shock.
The Golden Hour was applied and no lost time was anticipated.

Wool
Emergency Special Working irregularity
At 22:13 during Emergency Special Working due to a multi signalling equipment failure, the Basingstoke Dorset Coast Signaller reported that 174b points at Wool appeared to flick up in reverse as South West Railway service 1W81 passed over them.
174 points should have been in the normal position, but upon investigation it was established that the points had been reversed for a movement earlier, but not placed back to normal prior to the first train being authorised through.
This incident was subject to further investigation.

Cat A SPAD
South West Railway service 2N09 07:23 Ascot – Guildford reported passing GD956 at danger on the Up Ash line approaching Ash Station after a loss of concentration. There was no allegation against the signal.

Stoneleigh
Staff accident
A member of Inner PWay was walking into the Up Cess country side of Stoneleigh when a train was seen to be approaching. Upon stepping into a position of safety, they slipped on a landslip board and fell awkwardly onto their left wrist injuring the wrist and thumb. The Golden Hour was applied and monitored overnight. There was visible bruising on the wrist, but the injured party was able to resume.

Staff accident
Whilst assisting to de-train passengers and their luggage from a failed Great Western Railway service, an Inner Mobile Incident Officer pulled their right bicep resulting in difficulty to lift their arm and carry small items for a period of time. The Golden Hour was applied and this injury became a lost time.

Feltham West Level Crossing
Signaller Level Crossing irregularity
A Trainee Signaller trapped a Level Crossing User within Feltham West Level crossing and ran 2 South West Railway services (1 on the up and 1 on the down) without realising an error had been made. The incident was reported via social media and is under investigation.
19/10/2018
West Byfleet
Signaller cautioning irregularity
South West Railway service 2B24 07:38 Southampton Central – London Waterloo was cautioned about a speed restriction being put in place, but was not given specific instructions about what was required. As a result, the train was travelling in excess of 20mph over where the speed applied. Upon investigation it was established that the Signaller had not carried out a robust handover and a breakdown in communication had occurred. This incident was subject to further investigation.

Haslemere
Staff assault
A Contractor from B&M McHugh was carrying out scoping for a brick work job at Haslemere and was approached by a group of youths. The youths appeared to be drunk and became abusive. The Contractor asked them to leave them alone, but the youths then set upon them as they started to attempt to flee away. The Contractor eventually made it into Haslemere Signal box where chain of care was carried out. The injuries sustained were to the head (nose, face and eyes) and a hospital visit was recommended.

Guildford
T3 Strapping irregularity
The Guildford Signaller reported that CUM track circuit had become occupied and suspected it to be a short circuit strap that had been placed in wrong location. This was confirmed by the Person in Charge of the T3 Possession and it was identified that the strap had been placed on the wrong side of the block joint.

Waterloo
Signaller Line Blockage irregularity
The Wimbledon Shift Signaller Manager reported that the Signaller on Panel 2 at Wimbledon ASC had granted a Line Blockage on the Up Windsor Slow at 06:36 which overlapped with T3 possession item 14 in error. Upon realising, the Signaller contacted the Controller of Site Safety give it up. This incident was subject to further investigation.

Wylye Level Crossing
Staff accident
A member of Salisbury S&T was walking back to an access point and whilst track side at Wylye Automatic Half Barriers, they slipped off the troughing route and twisted their right knee. They arranged to rest it for the remainder of shift and the Golden Hour was applied. Lost time was anticipated.

Staff accident (RTA)
A member of Salisbury PWay was traveling back to Salisbury depot in a Network Rail van and whilst navigating the St Pauls Roundabout in Salisbury, they moved from the left hand lane to the right lane and bumped/scuffed the rear driver’s side of another car. There were no injuries sustained and the Golden Hour was applied for chain of care.

Possession Support Staff Line Blockage irregularity
The Three Bridges Route Operations Centre Shift Signaller Manager reported that a Line Blockage irregularity involving McGinley Possession Support Staff (PSS) within Wessex WON 30, Item 50 had occurred on the St Helier Lines. Post carrying out their initial PSS duty at the start of the possession, the Line Blockage was given up at 02:45. No further contact was then made until approximately 05:00 from the same PSS saying they had now removed equipment and wanted to give the Line Blockage up. This incident was subject to further investigation.

Merton
Staff accident (RTA)
A Network Rail van being driven by a member of Wimbledon Signal Works was involved in an RTA when a member of the public pulled out from Merton High Street onto Abbey Road in Merton without checking that it was safe to do. The Network Rail van clipped the rear wing of the member of public’s car causing minor damage. A second member of staff was within the Network Rail van, but there were no injuries sustained to any party. The Golden Hour was followed for chain of care.

Staff accident (RTA)
A Project Scheme Manager was driving to Woking Stores in a Network Rail hired van when a vehicle reversed into the temporary traffic lane they were driving on at low speed. The vehicle clipped the rear side of the Network Rail van and apart from minor damage, no injuries were sustained. The Golden Hour was applied for chain of care.

Byfleet and New Haw
Staff accident
A Contractor from Cleshar Rail was walking through a worksite with artificial lighting when they tripped on a piece of loose ballast within the four-foot and fell over. The Injured Party put their left elbow out to break their fall, but in doing so the elbow also landed on some ballast. There was no cut or graze present, but the injury begin to swell and arrangements were made for them to go to A&E for an x-ray. The Golden Hour was applied and lost time was anticipated.

Parkstone
Staff accident
A member of Outer PWay rolled their foot on a piece of ballast whilst walking trackside near Parkstone.  They did not make any comment about it at the time and continued working. The following day, the injured party returned to work but then went to hospital to get it checked. The Hospital found that soft tissues had been injured. Upon the injury being reported, the Golden Hour was applied and the injury became lost time.

Tisbury / Gillingham
T3 Possession irregularity
Upon a Rail Grinder exiting T3 Possession item 88A at Gillingham, the Driver reported that the Possession Limit Board was the wrong side of the Worksite Marker Board by signals SE4683 and SE4681. The PICOP of item 88A reviewed the situation and confirmed that the issue was with item 88 and not 88A and that the Engineering Supervisor had not been aware of the change resulting in the error being made. This incident was subject to further investigation.

Staff accident
A member of Bournemouth PWay was carrying out a track side inspection and whilst walking across Lymington Viaduct, the wooden walkway of the bridge gave way resulting in their left leg falling through up to the hip and jolting their back. The injured party was stuck for a few minutes, but managed to free themselves. The Golden Hour was applied and apart from suffering from some discomfort, no lost time was anticipated.

Cat A SPAD
The Driver of South West Railway service 2G68 20:34 Guildford to London Waterloo reported passing W2 signal by 1 ½ coaches at danger having thought it had been displaying a yellow aspect. Forward Facing CCTV footage showed otherwise and there was no evidence of read across as all the other signals on the same gantry were also displaying a red aspect. This incident was subject to further investigation.

Staff accident
A member of Works Delivery staff set up a disk cutter whilst at Yeovil and when pulling the start cable, they felt their right shoulder go and believed that it had become dislocated. The Golden Hour was applied and having sort medical attention, this became a lost time injury.

T3 Strapping irregularity
The Wimbledon PWay Section Manager received advice that a Strapman for T3 Possession item 37 had placed a strap on the open St Hillier lines at Wimbledon having become disorientated. It was later found that they had not tested nor applied a short circuiting bar on this line before applying. Chain of care was carried out for potential arc eye. This incident was subject to further investigation.

Staff accident
A member of Eastleigh Works Delivery staff was working on track at Yeovil Junction when an On Track Machine knocked a piece of scrap rail which fell onto its side trapping their right foot just above the steel toe cap. The rail was removed and the injured party was taken to hospital as a precaution. Although in a lot of pain, x-rays confirmed no broken bones. The area later became numb and the injured party was prescribed morphine based painkillers. The Golden Hour was applied and this became a lost time injury.

Electrical Control Room irregularity
The Eastleigh ECR reported that one of their Operators was carrying out continuity checks on the Ash Vale line (PAA1) and had inadvertently closed a Circuit Breaker (CB) within T3 possession item 46. The CB tripped on close and the Operator was stood down pending further investigation. There were no injuries, but the isolation straps applied for the T3 had to be replaced.

Hydraulic Oil Leak
6U26 05:00 Eastleigh East Yard to Woking Up Yard had a hydraulic oil leak and lost approximately 150 litres on the Down Main in the Micheldever / Wallers Ash Tunnel area. The fluid was treated with sand and the British Waterways and the Environment Agency were made aware. A NOP Form F4 14A was completed and emailed to the route Workforce Health, Safety and Environment Advisor. This incident was subject to further investigation.

Hydraulic Oil Leak
Possession staff working within T3 possession item 136 reported that the RRV railvac had leaked approximately 10-20ltrs of hydraulic fluid onto the track within Kingston Bay platform 3. A spill kit was used.A NOP Form F4 14A was completed and emailed to the route Workforce Health, Safety and Environment Advisor. This incident was subject to further investigation.

CAT A SPAD
Ariva Overground service 2L93 16:14 Clapham Junction – Stratford had a Cat A SPAD on W306 signal upon becoming distracted and passed the signal by 20 metres. There was no allegation against the signal.This incident was subject to further investigation.

Cat A SPAD
Balfour Beatty tram 6J61 23:34 Chard Junction to Honiton passed SE4764 Ground Positon Light shunt signal at danger without the authority of the Signaller by 511 metres. The signal had been unsighted by the Driver having stopped on top of it rather than clear of it whilst performing the shunt. This incident was subject to further investigation.

Staff accident (RTA)
A member staff from Telecoms Inner was involved in an RTA on the M25 (junction 7 with the M23) with a white van that attempted to join from the slip road. The white van clipped the Network Rail van causing minor damage. There were no injuries. Golden Hour was applied for chain of care.

The Site Supervisor within T3 item 9 reported that W757B points at Wimbledon West Junction had been run through by a Rail Road Vehicle through a breakdown in communication when carrying out the movement. This incident was subject to further investigation.

Wimbledon Town Centre
Two members of staff from Raynes Park D&P were driving around the Wimbledon multi lane One Way system when a bus pulled out of the bus lane and into their Network Rail vehicle. Apart from minor damage sustained, there were no injuries.
The Golden Hour was applied for chain of care.

Windsor and Eton Riverside
Whilst testing Track Circuits at Windsor and Eton Riverside, a member of Works Delivery S&T went to step over the running and conductor rails in one movement and placed their foot on what they believed to be ballast. Unfortunately their foot had been placed on a timber sleeper resulting in them slipping and falling onto their side causing pain and bruising to their arm, shoulder and neck resulting in limited movement on their left side. They went home to rest and were going to seek medical attention if the pain persisted. The Golden Hour was applied and this became a lost time injury.

Southampton Airport Parkway / Eastleigh
A Line Blockage was requested by the Controller of Site Safety for the Eastleigh area, but was declined by the Signaller until a train passed through that location. Once the train passed, the COSS then asked again and the Signaller queried where they were as the train had not yet passed them according to their indications in the Panel. The COSS then advised the Signaller that they were located at Southampton Airport Parkway and not Eastleigh as per the Line Blockage limits requested. This incident is subject to further investigation.

Waterloo
South Western Railway service 2C20 reported near miss involving three track workers on the approach to Waterloo station who were in the four foot of the Up Windsor on the approach to W10 signal. The Driver sounded their horn and the track workers acknowledged, but not all of them moved to a position of safety. The Driver then applied the brakes reducing their speed to 20mph and sounded the horn again. The track workers acknowledged once again and moved into the Windsor Reversible four foot where a planned Line Blockage was in place. This incident is subject to further investigation.

Clapham
The Clapham Junction SCP reported a Dispatch/Operating Irregularity involving 1D32 12:03 Dorking to London Waterloo where the Guard closed the doors whilst an accessible ramp was attached to the train. The ramp had been used for a Wheelchair Passenger and was seen to fall out of the train as the doors closed. It was also established that a visually impaired person had also been left on board the train and arrangements were made for them to circulate via Vauxhall. Upon investigation the Guard had been situated in coach 9 (456 unit) of the 10 coach train and was aware of a passenger alighting but did not see the ramp when using the CCTV camera’s and the platform staff had obscured their vision. This incident is subject to further investigation.

A member of Havant PWay was alighting from their van to unload some equipment on public land and twisted their right ankle in a pothole. The following morning they visited their Doctor who advised the ankle needed to be rested. The Golden Hour was applied and this became a lost time injury.

Woking Up Yard Reception
Woking S&T reported that WK1206 Ground Position Light had fallen over as its base had been broken. An RT3187 was arranged to sign the signal out of use.
Upon investigation it was established that RRV’s working for Works Delivery had been the cause. The signal has since been rectified. This incident is subject to further investigation.

Whilst carrying out a tandem lift of a generator a Contractor working for Works Delivery was injured when ballast which was freshly laid and had not been consolidated properly gave way. The Injured Party sustained a twisted left ankle, but it did not become swollen and they were able to walk. The Golden Hour was applied and there was no lost time.

Point Pleasant
Whilst undertaking wheel timber renewals on bridge 8 between Point Pleasant and Wandsworth Town, an Inner Maintenance Track Technician overlooked the gap between the timbers resulting in them slipping and falling. The Injured Party sustained cuts to an elbow and swelling / bruising to their left ankle and arranged to go to hospital due to being in considerable pain. The hospital confirmed it to be a sprain. The Golden Hour was applied and there was no lost time.

A member of Bournemouth PWay fell onto an isolated section of conductor rail after being attacked by hornets. The Injured Party cut their right leg. First aid was given and the Golden Hour was applied. There was no lost time.

Chard
The Basingstoke West of England Signaller reported that they had to stop a tamper before it was going to pass through a Chard Junction CCTV as it would have been unprotected. Also 2824 and 2820 points were not set in the correct position for the route. Upon investigation it was established that the PICOP for item 93, had communicated with the Tamper Driver, but had overlooked contacting the Signaller about the movement. This incident is subject to further investigation.

Clapham
Wimbledon SSM reported station staff at Clapham Jn had called in to ask for a block on the up line on the Windsor side, the signaller tried to question and to confirm which up line, confirmed that this was through platform 4 and before the signaller could confirm with the station staff member they had hung up the phone. The signaller on panel 2 applied the reminder appliances on the signals and then called the platform staff to find out what was going on. A station manager answered the phone and said that he had retrieved a pram from the line, which they had believed had a baby in it. The DSM had incorrectly believed that his colleague had obtained a block of the line and had jumped down on to the track as he was concerned that there was a baby in the pram and due to its proximity to the third rail. This incident is subject to further investigation.

Eastleigh ECRO advised that as they were going from Part 3 to Part 4 of item 37, Country end breaker would not go back in. PICOP was advised and attended and found a strap was left on at 80m44ch. Initial info suggested that the B2 had been changed during the day indicating a potential isolation planning issue. This incident is subject to further investigation.

Guildford Station Staff reported that the fire alarms were sounding and that they were evacuating the station. They also requested that trains be stopped. SIO reported that emergency services had found the cause of the fire alarm being a coffee machine in M&S on Platform 2. Authority was given for the station to reopen.

Aldershot Tunnel
Guildford PWay requested a Line Blockage for a patrol which included Aldershot Tunnel with the Woking Panel 3 Signaller and were advised to wait for a train to pass them before Signaller could grant it. Once the train had passed, the Line Blockage was granted but a further train had since entered the section. The Site Warden noticed the train approaching and the patrolling team were able to quickly exit the tunnel and get clear of the line. Chain of care was carried out with all concerned and the Signaller was relieved of duty. This incident is subject to further investigation.

Popham
Two members of Outer Distribution and Plant were struck in the driver’s side of their Network Rail crew van by a black Mercedes which had gone through a red traffic light at speed near Popham on leaving the M3 at Junction 7. Both vehicles were badly damaged. The D&P members of staff suffered with stiffness post incident and were shaken. The injuries of the Mercedes driver were not disclosed. The Golden Hour was applied and 1 member of staff received a lost time injury through concussion.

A member of Eastleigh Works Delivery PWay Outer caught their right index finger between the web of a rail and a sleeper chair whilst carrying out a renewal. They were able to carry on with their duties and did not require medical attention. The Golden Hour was applied and apart from bruising, there was no lost time.

A member of Works Delivery Outer was carrying a bag of screws when they slipped over and fell onto their right hand. Their thumb became swollen and the injured party was taken to Hospital where they identified that the thumb had become dislocated. The Golden Hour was applied and this became a lost time injury.

Salisbury
Two members of Works Delivery staff were travelling on the A36 near Salisbury when a deer appeared. The Driver had no time to brake and hit the deer which was small to medium in size at 40 mph. The damage sustained was cosmetic and there was no injuries. The Golden Hour was applied for chain of care.

Fatigue and Alertness

Whether you work in an office or on the front line, drive your car home from work or a train full of passengers, you need to be awake and alert to do your job safely and efficiently.  Managing fatigue is everybody’s responsibility. Our aim is to make sure that everyone, at all levels, understands their role in managing fatigue. Based on our research and consultation, we have put together a range of resources to help with this.

Fatigue is a state of weariness because of working for too long, working against our body clock, heavy physical or mental workload, insufficient rest or inadequate sleep. Fatigue can develop quickly, for example if we are doing heavy lifting. But it can also develop slowly, for example if we lose an hour of sleep a few nights in a row. The environment we work in and what we eat and drink can influence how quickly we start to suffer from fatigue.

Fatigued people are very likely to make mistakes.  The effects of serious fatigue are comparable to being over the drink driving limit: poor judgements, slow reactions, poor memory, impaired vision.

In our industry fatigue is a factor in 20% of high risk incidents. Long term exposure to fatigue is associated with health problems.

Upwey
Yeovil S&T reported that they had been involved in a line blockage irregularity for routine maintenance which had been granted by the Dorchester Signaller at 10:59. The Line Blockage was on the Down Main from 558pts to DR193 signal and the team were working just country side of Upwey Station when a train approached at approximately 11:01. The train involved was 1W57 08:05 London Waterloo to Weymouth and was booked to stop at Sway. The Driver blew their horn and the team managed to reach a position of safety. The Signaller admitted that they had overlooked this train when granting the block. Chain of care was carried out with all parties and arrangements were put in place to relieve the Signaller.
This incident is subject to a Level 3 Formal investigation.

Guildford
A NASH Contractor was getting out of the works van within Guildford Yard and stepped on an uneven surface resulting in them twisting / spraining their left ankle. The Injured Party was sent home and advised to seek medical attention. Upon attending A&E they advised that a tendon had been badly damaged. The Golden Hour was applied and it is anticipated that the Injured Party would be off work for approximately 3 weeks.

Southampton Airport Parkway
Eastleigh PWAY called to take a booked Line Blockage (GZAC 5921817), which was through Eastleigh station on the Up Fast line from E87 to E91 signal, but the Signaller asked for the PWay to wait for a train to pass and call them back. Upon the PWay calling back to say the train had passed, the Signaller challenged the PWay and asked where they were. The PWay advised that they were at Southampton Airport Parkway station. The Signaller then denied the Line Blockage and the error was investigated.

Egham
During degraded working whilst the signalling was occupied with another incident, the Signaller thought they had instructed the Level Crossing attendant at Egham to lower the barriers for 1S62 18:52 Woking – Staines and gave the Driver authority to pass F264 at danger and to stop short of the level crossing before passing over if it was safe to do so. The Attendant however; had not received this information and the train approached with the barriers raised. Having seen the train approaching, the Attendant lowered the barriers and the train then continued. The Signaller was relieved of duty.

Staff Accident
A Ganymede contractor was walking back to the access gate over a wheel timber bridge when his foot went partially through the decking and he tripped, landing on his leg and arm. There were no injuries. No first aid and no absence from work.
A form of Golden Hour was performed by Ganymede and arrangements were made to inspect the bridge to warn other users and make safe.

Staff Accident
A member of Interserve Contract staff was driving a buggy transporting 3 wheelie bins of rubbish and a member of SWR station staff was caught on their right hand side causing cuts / lacerations to her right arm, hand and bruising to her right shoulder. They attended hospital where they found nothing broken, but the wrist had been sprained and a support was fitted. There was no lost time.

Security Breach
Raynes Park ECO reported that a security breach had occurred on the Ops Floor with BBC journalists who had arranged to film with CRE staff overnight on the track working on faulty hook switches. Having been let in at the gate, no-one was then present at the door to brief them and sign them in. The visit had been arranged via the Network Rail Press Office and Inner Project Manager.

Staff Accident
A member of Eastleigh whilst undertaking TCR testing, received an electric shock from the TCR relay that became live. The IP confirmed that they felt fine but were advised to get a check-up following the electric shock. The shock was received to the right hand. IP later reconfirmed that they were suffering no ill effects. No absence from work anticipated.

Cat A SPAD
Wimbledon SSM advised that the Driver of GTR 1I20 10:25 London Victoria – Horsham has reported that they just gone past W457 at Epsom at Danger. The Driver made no allegation against the signalling or the unit and stated that the cause was a loss of concentration. Driver relieved of duty at Epsom.
This incident will be investigated by GTR.

Staff Accident
A member of Feltham S&T was walking down the track to site as a lookout when the ballast shifted beneath them and they tripped over. The injured party sustained a grazed right arm and landed on their right knee. No first aid was required and no lost time was anticipated. The Golden Hour was applied.

Kingston
Staff Accident
A member of Feltham S&T was crossing the line at Kingston and heading back towards the van when they stepped across the running rail and stepped either on some loose ballast or the edge of a sleeper and rolled their left ankle. The Injured Party was assisted back to the van after feeling light headed. The ankle became swollen, the Golden Hour was applied and the Injured Party was sent home to rest it.

Possession Irregularity – Item 13 (Wessex 90)
The Basingstoke ASC Signaller reported that T3 possession item 90 Wessex (Western 13) between Southcote Junction – Mortimer had been given up at 07:40, but the PICOP had not called them. The first that Basingstoke ASC was aware, was when 1O06 09:52 Reading – Bournemouth appeared. The Didcot Thames Valley Signaller was made aware and T2815 was placed to danger to stop 1O26 until the correct process was followed through with Basingstoke.
This incident will be investigated by the Western Route.
26/08/20182C46 Wrong Route Offered and Taken
2C46 14:54 Reading – London Waterloo was offered the route towards Blackwater vice Ascot and the Driver took the route. As a result of the wrong route, all lines were affected with the exception GWR services toward Reading and it required a response team to assist recover the unit.
This incident is subject to further investigation as the Signaller, Guard and Driver had all made an error resulting in this event occurring.
