34. Staff Accident (RTA)
An Eastleigh Signal Works Supervisor was driving a Network Rail Flatbed transit through a tight section of road.
As he did so, the safety fall arrest system on the back (being slightly wider than the lorry width), knocked an oncoming lorry’s wing mirror causing the outer housing to crack.
There were no injuries to either party sustained.

20. Staff accident
A Signal Works Team Leader was tightening a bolt with a hand ratchet by a set of points.
He momentarily lost purchase with the bolt and his right hand was deeply cut on a point fitting when he slipped.
He had been wearing PPE gloves at the time.
First aid was self-administered and the Golden Hour applied.
There was no lost time.
23/02/2017 Guildford
2F67 – CAT A SPAD
2F67 (21:50 Waterloo to Guildford) experienced a Cat A SPAD at GD825.
The train passed the signal by 3 ½ coaches.
Early indications point toward the Driver not reacting to the poor weather conditions correctly, where the strong winds combined with rain led to the rails being slippery.
This incident will be investigated by South West Trains.

Staff Accident
An Electrification section manager was walking over the Westminster Bridge when he tripped on one of numerous items sticking up (troughing, tools, cables et). As he tripped he twisted right ankle, banged his left knee and had a slight knock to bridge of nose where his glasses caused a minor cut.
Golden Hour was applied and there was no absence from work.
This incident is subject to a Level 1 investigation.
13/02/2017 Waterloo
Possession Irregularity
An ES for T3 Item 3 was setting a route to allow a train movement out of a worksite.
When walking back, he noticed that the route he had set had been changed and had not given any authority for this.
Upon investigation it was established that testers had moved them having had permission to do so on a previous occasion.
Initial indications point to a breakdown in communication, but is subject to further investigation.
This incident is subject to a Level 1 investigation.
14/02/2017 Wimbledon
2G21 CAT A SPAD
2G21 (1003 London Waterloo – Guildford) passed W187 signal at danger (red) without authority after he believed it had been displaying a single yellow. The TPWS brought train to a stand 7 metres beyond the signal.
The signal had been at danger due to a Kingston service in the section ahead.
Initial indications point to the Driver having a lapse in concentration and is subject to a SWT Level 2 investigation.
19/02/2017 Millbrook
Staff Accident
A member of staff working for Eastleigh Works Delivery within a gang changing sleepers within possession Item 42.
The rail had been jacked up by a simplex jack and as the injured party was in the process of clearing out ballast from the base plate of the rail, another operative released the lift without prior warning. (Fingers and Toes Clear, 1, 2, 3, Lower).
The injured party’s hand became trapped and immediately swelled up.
Upon being taken to hospital it was established that the right hand had been bruised.
Golden Hour was applied and there was no lost time.

Possession Irregularity Item 49
6Q66 was given authority to enter a T3 possession by the Signaller up to the detonator protection, but the Blockman on site had lifted the protection without authority from the PICOP.
This incident is subject to a Level 1 investigation.
05/02/2017 Brockenhurst
Cat A Spad
1J29 (Brockenhurst – Lymington service) passed BH13 signal at danger by 3 coaches after the Driver lost concentration.
Although the Driver had put in the emergency brake, the train triggered the TPWS at 34mph.
It was established that the Driver had lost concentration after the Guard gave the tip to depart which he should not have done, when the OFF indicator was not illuminated.
The Signaller had been holding the train to connect with a late running Bournemouth service.
This incident is subject to further investigation by SWT.
08/02/2017 Eastleigh
Staff Accident
A Cable technician had been was removing troughing lids a week ago and had heard his left wrist click. He did not think anything of it at the time, but it had since become swollen.
09/02/17 – The Injured Party had an x-ray which confirmed that nothing was broken.
He resumed duty, but was advised not to carry out any heavy lifting until the swelling had gone down.

Staff Accident
A member of Salisbury P-Way was working on the Warminster line slewing rails and putting in nylons during wet conditions when he slipped. He fell forward onto hands hurting his wrists.
Golden Hour was applied.
No lost time.

CAT A SPAD
2C73, a London Waterloo to Reading service reported a TPWS activation on W79 signal after passing the signal at danger (red) by reading across to the signal alongside.
The train was travelling between 10-15mph.
This incident is subject by a SWT led investigation.
31/01/2017 Cosham Junction
Staff Accident
A Havant Technician was walking off of the track in wet conditions when he slipped on a cable and badly twisted his left knee.
Golden Hour applied.
No lost time was anticipated.
CAT A SPAD
Freightliner 0M04 passed E965 signal at danger whilst carrying out a shunt move.
The Driver had been distracted by a fire in the exhaust of the loco.
This incident is subject to a Freightliner led investigation.
02/02/2017 Durnsford Road
Staff Injury
A Technical Officer was walking up access steps and felt a twinge in their upper leg.
Golden Hour was applied.
Post incident the Injured Party still had some pain, but there was no cause for concern and no lost time was anticipated.
03/02/2017 Leatherhead / Dorking
WON 44 Item 143 – Possession Irregularity
The Dorking Signaller reported that the PICOP had contacted him to advise that he had gained signal protection his T3 possession from Wimbledon and Three Bridges Area Signalling Centres at 0047 however; a set of empties (5E32) had only just left Dorking heading toward Leatherhead which had meant that the T3 had been given signal protection with a train still in section.
The main underlying cause subject to further investigation is that neither Wimbledon or Three Bridges had communicated with Dorking to establish if it was safe for the T3 to be taken.
03/02/2017 Raynes Park
Location Cabinet Struck
Surrey Steels reported that one of their forklift trucks had accidently knocked over a pile of steel.
The steel slid down, knocked over a fence and rolled into location cabinet which then fell toward the running line.
The workers managed to make the area safe without going trackside until response teams arrived to rectify the situation.
This incident is subject to further investigation.
