07/08/2018 Epsom | Staff Accident A Ganymede contractor was walking back to the access gate over a wheel timber bridge when his foot went partially through the decking and he tripped, landing on his leg and arm. There were no injuries. No first aid and no absence from work. A form of Golden Hour was performed by Ganymede and arrangements were made to inspect the bridge to warn other users and make safe. |
07/08/2018 Waterloo | Staff Accident A member of Interserve Contract staff was driving a buggy transporting 3 wheelie bins of rubbish and a member of SWR station staff was caught on their right hand side causing cuts / lacerations to her right arm, hand and bruising to her right shoulder. They attended hospital where they found nothing broken, but the wrist had been sprained and a support was fitted. There was no lost time. |
08/08/2018 Raynes Park | Security Breach Raynes Park ECO reported that a security breach had occurred on the Ops Floor with BBC journalists who had arranged to film with CRE staff overnight on the track working on faulty hook switches. Having been let in at the gate, no-one was then present at the door to brief them and sign them in. The visit had been arranged via the Network Rail Press Office and Inner Project Manager. |
15/08/2018 Brockenhurst | Staff Accident A member of Eastleigh whilst undertaking TCR testing, received an electric shock from the TCR relay that became live. The IP confirmed that they felt fine but were advised to get a check-up following the electric shock. The shock was received to the right hand. IP later reconfirmed that they were suffering no ill effects. No absence from work anticipated. |
18/08/2018 Epsom | Cat A SPAD Wimbledon SSM advised that the Driver of GTR 1I20 10:25 London Victoria – Horsham has reported that they just gone past W457 at Epsom at Danger. The Driver made no allegation against the signalling or the unit and stated that the cause was a loss of concentration. Driver relieved of duty at Epsom. This incident will be investigated by GTR. |
23/08/2018 Datchet | Staff Accident A member of Feltham S&T was walking down the track to site as a lookout when the ballast shifted beneath them and they tripped over. The injured party sustained a grazed right arm and landed on their right knee. No first aid was required and no lost time was anticipated. The Golden Hour was applied. |
25/08/2018 Kingston | Staff Accident A member of Feltham S&T was crossing the line at Kingston and heading back towards the van when they stepped across the running rail and stepped either on some loose ballast or the edge of a sleeper and rolled their left ankle. The Injured Party was assisted back to the van after feeling light headed. The ankle became swollen, the Golden Hour was applied and the Injured Party was sent home to rest it. |
26/08/2018 Southcote Junction | Possession Irregularity – Item 13 (Wessex 90) The Basingstoke ASC Signaller reported that T3 possession item 90 Wessex (Western 13) between Southcote Junction – Mortimer had been given up at 07:40, but the PICOP had not called them. The first that Basingstoke ASC was aware, was when 1O06 09:52 Reading – Bournemouth appeared. The Didcot Thames Valley Signaller was made aware and T2815 was placed to danger to stop 1O26 until the correct process was followed through with Basingstoke. This incident will be investigated by the Western Route. |
| 26/08/2018 | 2C46 Wrong Route Offered and Taken 2C46 14:54 Reading – London Waterloo was offered the route towards Blackwater vice Ascot and the Driver took the route. As a result of the wrong route, all lines were affected with the exception GWR services toward Reading and it required a response team to assist recover the unit. This incident is subject to further investigation as the Signaller, Guard and Driver had all made an error resulting in this event occurring. |

Staff accident
A member of Havant Pway rolled their ankle on ballast whilst carrying out a track patrol within the 4 foot at Vernon Bridge (Portsmouth Harbour). The ankle was sore, but it could be walked on.
The Golden Hour was a applied and no lost time was anticipated. |

Staff Accident
An Admin Clerk at Woking was hole punching some documents in the office and caught their left index finger in the hole punch lever and sustained a cut and bruising.
Ice was applied and they attended a walk in centre at the hospital as a precaution. The finger was not broken.
The Golden Hour was applied and there was no lost time. |

Staff Accidents (RTA)
Two members of Guildford S&T were stationary on the A25 (Middleton Road) due to traffic when they were shunted from behind by a vehicle which had been struck by a bus.
There was no visible damage to the Network Rail vehicle and no injuries sustained to the staff.

Irregularity with Possession item 34
The Eastleigh Electrical Control Room Operator advised the Person In Charge of Possession for T3 Possession item 34, part 3, that a circuit breaker had tripped at Northam.
Upon investigation it was established that a short circuiting bar had been placed by Possession Support Staff ready for strapping out within the Down Eastern Docks area, but had not received authority to do so and had not tested before touch either.
There were no injuries.
This incident is subject to further investigation. |

Line Blockage Irregularity
Havant S&T reported a Line Blockage irregularity had occurred when a train came through upon gaining authority to work.
The Line Blockage was taken between PF234 signal to EW3/6 signals and the Signaller had not come to a clear understanding with regard to the location of the train.
The train, 1P42 12:32 Portsmouth Harbour – London Waterloo was slowing down for the Haslemere station stop. A Near Miss was not declared.
The incident is subject to further investigation. |

2V59 19:37 London Waterloo – London Waterloo had a Cat A SPAD on W79 signal. The signal had been at danger due to 2C99 in the section ahead. The Driver did not dispute the SPAD and minimal testing took place post event. |

2B80 – Operational Incident
2B80 21:55 Southampton Central to Waterloo service (12-450) was diagrammed incorrectly into Platform 7 where it could not be fully accommodated. The Signaller followed the booked working and the Driver accepted the route in error. All passenger doors were accommodated and the passengers were dealt with safely. |

Mount Pleasant Level Crossing |
Person trapped in barriers
Eastleigh Area Signalling Centre reported that a person had been trapped within the barriers of Mount Pleasant CCTV level crossing and the Signaller had pressed “Crossing Clear”.
The Signaller then checked the picture again (with colleagues) and felt that something did not look right. It was therefore arranged for 2B36 to be cautioned. The Driver reported that there was a woman dressed in black with a pushchair, right up against the pedestal on the down side. Once 2B36 had passed, the female with the buggy left the crossing as the barriers raised.
The CCTV picture within Eastleigh ASC is compromised by being black and white and of poor quality.
This incident was subject to further investigation. |

Possession irregularity Item 32
The Operations Delivery Manager Inner reported an irregularity had taken place within T3 Engineering work possession item 32 where Possession Support Staff had opened up a wrong hook switch at Nine Elms. (HS1094 vice HS1095).
There were no injuries sustained, the area was made safe and the investigation process then followed. |

Worksite irregularity – Item 39
The PICOP of T3 Engineering work possession item 39 became aware of an irregularity involving a crane which had ran over an S&T cable on the Up main at 80m 46ch. The cable had been due for replacement prior to the error made, so no issues were experienced. |

Staff Accident
Whilst moving sleepers on the Ludgershall branch ready for installation, a Gannymede Contractor got some creosote on their glove and accidently wiped it their left arm and neck resulting in mild burns and irritation to the skin. The affected area was washed and cream was applied. The Contractor then visited Winchester hospital as a precaution where they treated him for dermatitis inflammation.
The Golden Hour was applied and there was no lost time. |

Possession Irregularity Item 133
1B01 01:05 London Waterloo – Basingstoke reported running over detonators and a Possession limit board at Ashford on the down line and observed possession staff jumping out of the way. The PICOP of T3 Engineering work possession item 133 had not given any permission for the McGinley Possession Support Staff (PSS) to lay out protection, as signal protection to do so from the Signaller had not been granted.
The PSS had also made a previous error within the same possession when they keying back F194 signal vice F195 signal to danger (red) to protect the T3 on the up line.
There were no injuries sustained, the area was made safe and the investigation process then followed. |

Cat A SPAD 6O35
DB Cargo 6O35 06:10 Westbury Down T.C. – Fareham A.R.C. Sidings had a Cat A SPAD at E32 signal protecting Eastleigh Junction. The Driver had experienced a micro sleep post acknowledging the automatic warning system (AWS) prior to the signal. Minimal post SPAD checks were arranged. The signal had no SPAD history. |

Staff Accident
A member of staff from Eastleigh E&P was carrying out electrical re-earthing works at Worting Junction substation and whilst crimping up the cables, their finger was caught in the handles crushing and breaking a finger nail.
First aid was given and the Golden Hour applied.

Road Traffic Accident
An Inner Maintenance PWay Section Manager was involved in a road traffic accident on the way to work in a company van on the A335 in Eastleigh when another van which coming from the other direction veered over the central white line.
The Section Manager swerved to the left to avoid a head on collision, but both vans wing mirrors made contact.
The Golden Hour was applied for chain of care. |

Staff Accident
A Mobile Operations Manager was stepping over a running rail to get to the cess, but had not seen an exposed drainage hole and fell into it. They sustained an injury to their left arm and jarred their right hip.
The Golden Hour was applied and although bruised, the MOM was able to resume their duty. |

Staff Accident
Upon driving to work in a Network Rail leased vehicle on the A312 near Cranford (Hounslow), an Inner Maintenance Section Manager misjudged the speed of a car in front and collided with it as it was slowing down. There was minor damage to both vehicles, but no injuries.
