Staff Accident A contractor working for IP Track was walking on sleepers within a T3 possession when they slipped and fell causing a graze to their right arm.
They were given first aid to deal with the graze and there was no lost time.

28/12/2017 Southampton
Staff Accident A contractor working for IP Track was crossing the track within a T3 possession to retrieve a disc cutter from the stores, but slipped on a sleeper due to ice and sustained a bruised and grazed right arm.
First aid was refused and there was no lost time.
(Note: IP do not apply the Golden Hour process).

Staff Accident A contractor working for IP Western & Wales was replacing a concrete sleeper within a T3 possession and as it was being lowered into the cess, the contractors foot was trapped.
The Injured Party was taken to hospital and x-rays showed slight swelling.
There was no lost time.

Staff Accident A Contractor working for IP was installing a conductor rail within a T3 possession , but it slipped off a pot due to not sitting correctly and caught the Injured Party’s finger between the ramping machine and the rail.
First aid was administered on site and the Injured Party was taken to hospital as a precaution.
The injury was x-rayed and apart from swelling, there were no broken bones.
There was no lost time.

Staff Accident A member of Colas S&T working for IP suffered an injury to their hand after trapping it under a troughing lid whilst putting cables away within a T3 possession.
The Injured Party attended hospital where they had treatment for a broken left ring finger.
They were able to return to work carrying out light duties.

Staff Accident
A member of inner DU PWay injured their left hand when high wind blew a store container door and knocked them to the ground.
The incident was not reported real time, so the Golden Hour was not applied.
There was no lost time.

Cosham Level Crossing
Irregular Working / Red Bardic Left Out
Havant Area Signalling Centre received a call from the Booking clerk querying if the T3 possession (Item 123) had been given up, as there were Red bardic lamps left out at the Level Crossing.
Upon investigation it was identified that a member of Possession Support Staff had not collected them after a breakdown in communication.
This incident was subject to further investigation.

Possession Irregularity Item 505
South Western Railway service 5T14 reported a very loud bang approaching HT59 signal on the down line.
It was identified that a member of Possession Support Staff had overlooked recovering one of the three detonators that had been placed after a T3 possession.
This incident was subject to further investigation.
07/12/2017
Byfleet and New Haw
Irregular Working 2135 pts/1A36.
Woking Distribution and Plant reported that whilst working on points heating, one of the heating strips was left unsecure for the passage of South Western Railway service 1A36.
The train struck the strip, but there was no injuries or infrastructure damage.
This incident was subject to further investigation.

Winchester
Debris blown on track
Station staff reported that part of the station roof had fallen onto the track due to high winds and South Western Railway service 1W36 had struck it.
Fortunately the section of roofing was a Perspex glazing panel which caused no injuries or further damage.
Upon investigation it was established that the material holding the panel in had become rotten and the inspection regime was reviewed.
A safety net was installed.

Staff accident
A PWay Team leader trapped their left middle finger within a sliding van door.
First aid was administered and the Golden Hour applied.
There was no lost time.

Millbrook
Staff Accident
Whilst walking back from site to the van, a Senior Technical Officer slipped on some wood hidden underneath the ballast.
He sustained a displaced fracture and tore both ankle ligaments which required the ankle to be re-set and pinned.
The Golden Hour was applied and it was likely that the Injured Party would be off for between 6-12 weeks.

Staff Accident
A Mobile Operations Manager had a low speed Road Traffic Accident at a roundabout when car drove into the side of his van.
On investigation it was established that there had been confusion over who’s right of way it was on behalf of the other driver.
The Golden Hour was applied for chain of care.
There were no injuries sustained.

Northam depot
5B07 Struck Possession Board
South Western Railway service 5B07 reported striking what was described as a Possession Limit Board on the reception line in the vicinity of E914 signal.
Upon inspection, the board said ‘Stop check the points are set correctly before proceeding’ and had been used by IP as part of a T3 possession.
This incident was subject to further investigation.

Wraysbury
Staff incident – Rotten Timber Decking
A Nash Contractor reported that a member of staff had fallen through rotten timber decking and bruised their left leg.
The Golden Hour was applied and there was no lost time.
Post incident was identified that the decking had been banned from use but not signed as such site.
The work to replace the decking had also been postponed due other emergency work requiring priority.
Temporary signs have since put in place and the area sprayed to reflect the danger.

Southampton Eastern Docks
Possession Incident, Item 33
A Shift Manager for IP track working within T3 possession item 22 reported that whilst propelling a train up the Eastern docks branch, a track panel on the train was out of gauge and had pulled E842 signal out of its base and caused cable damage.
The signal and cable was later repaired.
It was established that the panel not been loaded correctly before the movement took place.
This incident was subject to further investigation.

Staff Accident
A Team Leader from Off track fell down a hole and injured their right leg and lower back.
The Golden Hour was applied and there was no lost time.
29/11/2017 Waterloo
Equipment left on track
Platform staff reported that some items from the engineering work had been left on track within platform 7 after it had been given up.
A member of the P/Way removed what was reported to be a radio detection device.
The worksite belonged to the Wessex Capacity Alliance (IP) who shall be conducting an investigation.
03/12/2017 Wishford / Wilton North
Possession irregularity item 129
Train 1F07 reported to have ran over detonators and struck a marker / stop board on the Down Warminster line.
A possession had recently been given up, but the recovery of the blocking equipment had been overlooked.
The Signaller cautioned train 1F12 in the opposite direction and the Driver recovered the equipment left out on their line and returned it to Salisbury Signal Box.
The Possession Support Staff and ES were employed by Babcocks working on the behalf of IP.
IP will be conducting an investigation.

2P11 CAT A SPAD (WK375)
2P11 passed WK375 at danger by a third of a coach length after the Driver misread the signal on the gantry.
There was no allegation against signalling or rail head.
This incident is subject to a Level 2 investigation.
20/11/2017 Woking Down Yard
Derailment Engine 6Y55
The DB Cargo Shunter at within the down yard reported that a locomotive on 6Y55 had derailed within No 2 road.
Upon investigation it was established that the sand / aggregate pile had compacted into the check rail and allowed the locomotive wheel to ride over and derail.
The incidence occurred on Network Rail infrastructure.
This incident is subject to a Level 1 investigation.

Staff Accident
Whilst fitting an AWS electro magnet on WK608 an S&T member of staff trapped and crushed their right index finger.
The Golden Hour was applied and there was no lost time.

Staff Accident
A member of staff from Woking D&P twisted their ankle by the substation.
First aid was given on site and the Golden Hour was applied.

24/11/2017 Waterloo
Worksite irregularity, Item 29
ES for WON 34 Item 29 reported that a Wessex Capacity member of staff was working alone without a COSS present.
Upon being approached, the individual advised that he had signed in however; this was not reflected on the COSS paperwork.
This incident is subject to an IP investigation.

6J06 Oil Leak
The TQS working in item 41 (Berrylands – Byfleet Jn/Byfleet and New Haw – Effingham Jn – Addlestone Jn) reported that 6J06 on the up fast line country side of the station had an oil leak.
100 litres of biodegradable oil was lost.
2 staff were affected by the leak, an accident form was generated and the Golden Hour was applied.

Staff Assault
The Bournemouth Mobile Operations Manager observed a drunken male walking in the road coming towards his vehicle and reduced his speed.
The male then proceeded to throw themselves onto the bonnet of the vehicle, aggressively knocked on the driver’s side window and hurled abuse at the MOM about being ran over.
The incident was witnessed by a Bus Driver and was reported to the Police.
The Golden Hour was applied for chain of care.
13/11/2017 Templecombe
Operational Irregularity
Upon communicating with a COSS to take a line blockage, the Signaller made an error and granted the block with a train still in section on the belief that it had passed the work group.
It was upon the authority number being given and when the staff walked onto the track to start work, that the train then approached.
The staff were able to move in a position of safety and a Near Miss was not declared by the Driver.
This incident is subject to further investigation.
04/11/2017 Eastleigh South Junction
Irregular Working Contractors
CRE Section Manager saw two males wearing only orange tops. When seen, they appeared to run and hide in trees bushes behind a disused washer. Upon being approached, the males stated that they had been taking pictures of HV switchgear.
The men left site and were seen to get in a McGinley van.
The incident was reported to BTP and McGinley to follow up and investigate accordingly.
15/11/2017 Raynes Park
2F94 Cat A SPaD W186
2F94 reported passing W186 at danger by 15 feet after losing concentration.
There was no allegation against the signalling or the unit. This incident is subject to further investigation.
15/11/2017 Wilton Junction / Warminster
Near Miss – 3S80
3S80 called into Salisbury Signal Box to advise them of a near miss, which occurred at approximately 118m 20ch, near bridge E29/34 Tytherington.
The Driver advised that as the train came around a curve, they saw a member of staff with a blue bag and sounded the horn. The member of staff acknowledged.
As they continued, the Driver then came across two further staff members in orange clothing within the 4 foot, who looked surprised. One of the staff members pulled the other from the track into the cess.
Upon further investigation it was identified that Salisbury P-Way were dealing with a track defect in the area who had been taking line blockages in the area.
The Signaller was not implicated.This incident is subject to further investigation.
15/11/2017 Wilton Junction / Warminster
Staff seen crossing track on mobile phones – 3S80 An S&T Cover team working on the behalf of Works Delivery were seen to cross the track using mobile phones.
This incident is subject to further investigation.
16/11/2017 Waterloo
Staff Accident
A member of Ganymede staff working for Clapham DU carrying out a sleeper change, twisted their left knee damaging a ligament.
He attended hospital where he was fitted with a leg brace and given crutches for support.
The Golden Hour was applied and this became a lost time injury.
18/11/2017 Wimbledon
Staff accident
A member of staff Pway staff walked into a bracket at the back of their van whilst loading equipment causing a lump to appear and bruising under the right eye.
The Golden Hour was applied and there was no lost time.

Cat A Spad 3S83 (GD1331)
Guildford SB reported that 3S83 has passed ground position light GD1331 without authority.
The train passed the signal by 18-20 foot.
The signal should have been displaying a red aspect, but the driver disputed this and upon visiting the location the following day, the driver manager identified that sunlight had been affecting the signal giving a false aspect.

2P20 CAT A SPAD and Station Overrun
GSMR Rec call sent by 2P20 to report that the train would be unable to stop due to poor railhead conditions.
The emergency brake had been applied.
The train came to a stand 6 coaches beyond the signal and the barriers were not fully lowered.
03/11/2017 Maiden Newton / Dorchester West
Staff Accident
A member of Outer Pway was un-keying e-clips using a hammer, when one of the keys ricocheted off of the top of the boot off a member of staff put behind the key to block it and left a mark on their left shin.
Once they had been assisted back to the van, a cold compress was applied.
The Golden Hour was applied and no lost time was anticipated.
03/11/2017 Surbiton
Staff Vehicle Collision No Injured Parties
A Works Delivery vehicle had been parked on Upper Brighton Road whilst a team worked on the line.
Upon their return, they found that the vehicle had rolled back and collided with a short term hire vehicle.
There was light damage to the Network Rail vehicle.
The hire vehicle had a large dent on the near side rear panel and the rear near side window had also been damaged.
It was unclear if the handbrake had been applied correctly.
06/11/17 Wimbledon
2L10 Derailed As 2L10 from Basingstoke to London Waterloo crossed from the Main line to the East Putney (LUL) lines, the rear coach derailed on LUL (TFL) infrastructure.
The derailed coach travelled approximately 150 yards until the train came to a stand beside the Wimbledon Waitrose and under a signal gantry.
There were approximately 300 passengers on board who were full evacuated 2hrs 15 mins later.
There was 1 reported injury.
This incident had a good response and the line was open 2 days later.
Early indications point toward the track spreading under load.
LUL (TFL) will be conducting the investigation into this event.
09/11/2017 Eastleigh
Possession Irregularity – Item 60
The ODM reported that an irregularity had occurred within item 60 (Worting Jnc to Eastleigh East Jnc (2250 – 0500)) where Outer DU staff were working on 422 points within Eastleigh East Yard, but outside of the worksite.
There were further reports that an RRV being had also been involved.
01:21 – The On Call level 1Eastleigh P-Way manager was appointed IO, due to the ODM managing another item 60 incident involving a PICOP communication oversight between himself and Basingstoke.
09/11/2017 Clapham Junction
CAT A SPAD 9M48
Wimbledon SSM reported that 9M48 had passed W304 signal at danger within platform 2
Upon receipt of the RT3189, it was established that the Driver had pressed the TRTS and then departed without realising the signal was at red.
The train was brought to a stand by TPWS, one coach length beyond the signal.
The Driver made no allegations against the signalling or the unit.
(Note: Upon further investigation it was identified that a fault had been outstanding on the Automatic Route Setting which would normally set the route for a train when the TRTS is pressed).

Staff Accident
A Woking D&P Team Leader received a minor electric shock which affected his elbow and hands whilst working on a point heating cabinet.
He attended the A&E as a precaution

The insulated screw driver that was being used at the time was in perfect condition with no electrical break down. The screw driver was one of a set bought 6 weeks previous with GS 1000v rating.

Staff Accidents
A team of 4 Works Delivery staff were stung by wasps in multiple places when they disturbed a hidden nest.
Antihistamine was administered to those who required it and the Golden Hour was applied.
There was no lost time.
02/10/2017 Dunbridge
SPAD SY1 – 2S43
2S43 passed SY1 signal at Danger by up to 50 metres after experiencing poor railhead conditions from Kimbridge.
The Driver stated that he had applied the brakes but the train could not be stopped in time began to slip.
Once he realised he was going to pass the signal at danger, he applied the emergency brakes.
The TPWS also intervened.
03/10/2017 Chilworth
Staff Accident
A member of Works Delivery staff received a crushed left hand whilst running out a new cable.
He attended a Minor Injuries Clinic who advised that they had sustained soft tissue damage and required regular ice to be applied.
The Golden Hour was applied and there was no lost time.

Electric Shock Risk!!!!
At the weekend we had 350m’s of cable feeding one of our FTN sites stolen.
The thieves were very clever and have a high level of technical knowledge in as such as they knew how to cut the live cable without causing the protection to trip.
What this meant though is that the thieves left bare ends of cable protruding from the troughing route that were live at 415v!!
Can I please ask that if anybody sees bare ends of cable protruding from troughing routes etc to please not touch but report to wicc so we can assign the correct action.

Whilst assisting with the recovery of swans from the line, a Mobile Operations Manager received scratches on his arms caused by a swans claws.
This was the MOMs first incident involving swans and had not been wearing long sleeves due to the hot weather.
Golden Hour applied and there was no lost time.

Staff Accident
A member of welding staff struck their left thumb when using a small hammer to place a wedge in the rail. The thumb became blistered and required draining at hospital.
The hospital prescribed a course of antibiotics.
The Golden Hour was applied and no there was no lost time.
01/10/2017 Clapham Junction
Staff Accident
A Works Delivery Operative crushed his right hand and he sustained a deep laceration after an Iron Man fell onto it whilst loading it onto a vehicle.
He attended hospital and was detained whilst the injury was cleaned, x-rayed and stitched.
There were no broken bones.
The Golden Hour was applied and lost time was anticipated.

Staff Accident
A Weld Inspector trapped his left thumb in a van door.
The injury became sore and he was advised to seek medical assistance at A&E if it became any worse.
The Golden Hour was applied and this became a non-lost time injury.
23/09/17 Waterloo
Possession Irregularity Item 4 (Waterloo-Queenstown Road).
Upon being authorised to do so, engineering train 6Y81 made its way from a Worksite to exit the possession.
Upon arriving at the exit protection, the train exploded the first detonator having not seen it through lighting glare.
There were no injuries.
This incident is subject to further investigation.
25/09/17 Raynes Park
Staff Accident – Nash Contractor
A Nash Contractor was carrying out vegetation management and received a rash which was believed to be connected to brushing against an Oak Processionary moth (Thaumetopoea processionea).
The correct PPE been worn.
The Injured Party took anti histamines and arranged to see a doctor.
The Golden Hour once the injury appeared, was carried out.
The injured party was able to resume duty.
Note: The oak processionary is a moth whose caterpillars can be found in oak forests. They may pose a human irritant because of their poisonous setae, which may cause skin irritation and asthma.
26/09/17 Clapham Junction
Staff Accident (Electric shock) Whilst carrying out maintenance on and a dead test on W633A points heating, an E&P member of staff received an electric shock to the arm.
He then started to sweat and his throat became dry, but after 10 minutes it subsided.
He attended hospital as a precaution.
The equipment had been isolated prior to the work taking place and investigations are ongoing to establish why the equipment was still live.
The Golden Hour was applied and no lost time was anticipated.
