2V27 reported a near miss by 1780 points with 2 members of staff standing in the 4 foot. The driver stated that when blowing the horn they had to jump out of the way to get clear.This incident is subject to further investigation

Staff Accident
A Colas Site Supervisor working on behalf of IP was assisting in moving a piece of pipe using a crane when some liquid ran out of the end and fell onto his clothing which caused a burning sensation after 30 seconds. He removed his clothing and noticed a red patch on his skin and applied water.First aid was given on site, but he did not seek further medical attention.

Unsafe Work 1L39 reported that the train had struck a piece of equipment approximately 1 mile on the approach to the station. The Driver stated that there was a team in a position of safety within the cess as he felt the train struck what appeared to be measuring equipment (Tripod and gauge). This incident is subject to a Babcock investigation.
Early indications are pointing toward the gauge being sucked into the train having not been fixed down properly.

Staff accident A contractor working for Ekspan required 10 stiches in their stomach after being cut by a metal spike attached to a bridge bearer they were removing.
The stitches were due to be removed after 10 days and he was due to resume work the following day albeit at a different site.

Staff accident A member of Works Delivery was crossing the track on 06/09/16 when his left leg locked and caused his calf to swell. He returned to work the following day, but was sent home and advised to visit his GP.
The Injured party visited a walk in clinic and was advised to rest for a couple of days.
Due to the nature of role however, he was able to continue without any lost time.

Staff accident PWay Operative was using a hammer to unkey a rail in order to carry out a stress, but caught his finger causing a cut to his left index finger. First aid was given and the bleeding stopped. The injury was cleaned and bandaged.

Reading Spur
Staff Accident Member of Basingstoke S&T pulled a muscle in the right hand side of the lower back when removing the front cover from 8787 points. (A set of HPSA’s). The Injured Party was working with one other person (Team Leader) and was using correct manual handling techniques. Section Manager reported that the Injured Party attend the A&E in Reading but there was a 4+ hour wait to be seen, so went to the Basingstoke hospital. He was told to take it easy for 3 – 4 weeks, prescribed codeine and advised to take Paracetamol and Ibuprofin if he experienced further pain.

Staff Accident An operative from Guildford Off-Track was loading the work van with scrap fencing, when a piece cut him on his arm. The cut was approximately an inch in length, believed to be deep and first aid was given on site. The injured party attended a walk in clinic where the wound was dressed the wound. The Golden Hour was applied and no lost time was anticipated.

Staff accident A CRE operative was installing a 21 foot cable with a group. A Lug was at awkward angle and the group attempted to straighten it up by tightening the bolt with a big spanner. The Injured Party was in front of person with the spanner holding an impact wrench when the spanner slipped off the bolt. The person holding the spanner fell forward and the spanner struck the Injured Party in the mouth causing a cut lip and broken/wobbly tooth. Due to the late report of this incident, the Golden Hour was not applied, but the Responsible Manager did follow the incident up with the individual involved.

Cat A SPAD 1D43 1D43 (1554 London Waterloo – Dorking) passed W91 signal on the down main slow at Danger by 2 coaches. The signal had been at danger due a failure on W151 and a backlog of trains and the Driver had not controlled his train correctly upon leaving Vauxhall and reacted too late at W91 to stop. This incident is currently under further investigation.

Waterloo International
Staff accident A Ganymead contractor (working for Works Delivery) was digging out timbers by 1585pts and a whacker plate compacting ballast disturbed a wasp nest. A wasp then stung the Injured Party in the throat which caused swelling and difficulty breathing. Having attended St.Thomas hospital and taken medication (including anti-histamine cream) , he was sent home and advised to seek further medical attention if he did not feel any better the following morning. The Golden Hour Applied and the Injured Party was back to work the following night.

1L29 struck Cow 1L29 reported striking a cow at 126m 20ch at line speed.
The train did not de-rail. Having inspected the train, the Driver reported no running damage seen, but the cow catcher was broken. The MOM later found a broken 2nd strand of a 3 wire fence at 125m 64ch where the cow may have gained access.

Cat A SPAD (P) FY1 / Stop Board / Terminal gate Collision DB Cargo advised that 6B41 0648 Eastleigh to Fawley had a Cat A SPAD and collided with the closed terminal gates, pushing them back by three feet. The Driver alleged poor railhead conditions.
This incident is subject to further investigation by the Freight Operator DBS.

Staff Accident A member of the PWay was loading stressing kit onto a trolley to push back when the long bar of the stressing kit slipped and squashed his little finger causing a small cut. The finger was cleaned, a plaster applied and no hospital visit was required.

Staff Accident A Wimbledon Grinder picked up a spent fire extinguisher by the handle to place on the platform.
When as he did this, pressure was applied to the handle which fired the extinguisher and whipped the nozzle into his face causing a sore eye. Sterilisation water was used to clean the eye and a visit to the hospital was required. There was no damage to his eye, just swelling.
He was provided eye drops and resumed work the following night.
The Golden Hour was applied.

Staff Accident An Eastleigh Distribution Technician was cutting back vegetation when one of his feet got caught in a bramble bush.
On trying to get it free he felt his right knee twist causing him to fall over. The knee became swollen and a hospital visit was required where it was confirmed that there was soft tissue damage.
It was recommended that the Technician rest it for the rest of the week and take pain killers. This accident was anticipated to accrue at least 5 days (3 working days) time free from duty.
The Golden Hour was applied.

Staff Accident – Wimbledon Track Operative was a passenger within a works van when it hit the kerb causing it to lift in the air.
The Operative felt a sharp pain to his left shoulder where the seatbelt was positioned, and an ache further down his left arm momentarily.
A hospital visit was not anticipated.
The Golden Hour was applied.

Item 70 – Points Run Through 29B
PICOP for item 70 (Shalford junction to Petersfield) reported via the Signaller that 29 points had been run through within worksite B after Tamper 6J61 authorised to move by the ES.

There seems to be trend developing of staff not recording safety hour conversations by the deadline. As result, this week’s safety hour attendance has dropped to 49% across this route. This is well below target and is unacceptable and doesn’t show all the good work you are doing with your teams. You shouldn’t need chasing via call each week to record this as it should be reported each week (by you or your deputy if you are away).
Please can you ensure that youall record your safety hour conversation either via email or app by Friday 12:30pm every week.

Attached is audio taken from my ‘Go, Look, See’, which demonstrates the massive difference between a lookout warning with and without machinery running, even at low revs.
From what we understand –
2 men were involved in a near miss with a passenger train(1D31) at approximately 13:30, whilst retrieving an impact wrench from the 4 foot.
The team involved had been fitting fishplates and clamps to a rail defect on the down BTH3, between Leatherhead and Boxhill.
The Lookout gave an audible warning on his horn, but this may have been initially missed, due to the Impact Wrench running.
There was not a Touch-Lookout with the team.
It is believed that the Safe System of Work had been tested prior to the commencement of work, but not tested with the Impact Wrench running.
The COSS believes he reached the position of safety with approximately 6 seconds to spare.
The COSS called in the near miss on himself.
Statements were taken from all members of the team. This incident is under investigation

16/08/2016 – Sway / Brockenhurst – Member of Eastleigh Telecoms was opening an Location Case when he caught his foot on a pipe which resulted in him losing his footing and his right leg slipping about 2 feet causing some bruising and a scratch to his right leg.
No first aid or hospital visit was required.

16/08/2016 – Basingstoke – Member of staff from Eastleigh Pway reported having an accident when within the canteen at. Basingstoke ROC when he stood up and his back became painful. No first aid required, but a hospital visit was arranged.
Golden Hour applied. Drumbeat: Woke up stiff this morning, but has taken painkillers and resumed duty this morning. Incident not being recorded on SMIS.
10/08/2016 – Wareham Sidings – A member of works delivery Eastleigh whist wearing Network Rail approved sun cream noticed his right forearm was sunburnt and blistered.
The affected area has cleared up, he spoke to his GP and they have said it must be where he was digging and must of rubbed the cream off his arm.

Near Miss – 1P15 reported a near miss at the London end of the station as the train came around the curve at 90 mph, sounded the horn and one of the group crossed from the 4 foot on the down to the up to a position of safety. The person reached the position of safety approximately 5 seconds before driver passed. This incident was discussed on the Safety Hour last week and is subject to further investigation.

Pirbright
Road Traffic Accident – A Welding Team Leader had a road traffic accident on the B3012, Gole Road whilst driving the works van when negotiating a right turn. The vehicle was travelling at approximately 30mph when the brakes were applied, but the vehicle continued off of the road and into a ditch. The vehicle had not been used for 2 weeks and it was thought that the brakes had failed. Apart from being stung by stinging nettles and being in shock, no injuries were sustained. Work colleagues had been following at the time and chain of care was carried.

Woking Down Yard
A Pway operative was digging in Woking Down Yard with some colelagues. He caught his foot on the bar of a rail and when he returned to the depot he saw his foot was bleeding. No first aid was given on site. He went to a community hospital to have it examined. The cut was cleaned, but the hospital were unable to ascertain if the toe was broken as they do not x-ray toes. No lost time. Golden hour applied.

A Havant PWay operative strained a muscle in the left side of his back whilst carrying a section of an Iron man. The operative was assigned light duties for the remainder of the shift and has since resumed normal duties. Golden hour applied.

A Contract Welder sustained a torn ligament having stepped out of the van onto a hidden sleeper at a vehicle access point.

This resulted in a Lost Time injury.

Eastleigh
A potential possession irregularity occurred when the Signaller and PICOP came to an arrangement for 6U04 to come to a stand at E87 signal and then exit a T3 possession.
Unbeknown to them, E87 was still within a worksite and was not where the detonator protection was placed.
When the detonator protection was lifted, the Signaller then routed the train to exit and concerns were raised by staff on the ground that it had been routed via another line which was still blocked.
No movement took place.
Initial investigation demonstrated the following;
A breakdown in communication took place between the Signaller and PICOP.
There was no reference in the WON about the possession being substandard.
The Worksite on the Down Fast was shown as London side of 465A points rather than countryside.
This incident is still under investigation.

Portsmouth Harbour
Portsmouth Harbour Station Control Point reported that 1T19 (formed 5 coaches) had been signalled into Platform 4 on top of 2P40 (formed 8 coaches).
The incident was identified as being an error caused by the Network Rail Train Planning Unit. This incident is still under investigation.

London Waterloo
1P34 (8 car) was wrongly routed into platform 13 that already had an 8 car within the platform.
The incident was identified as being an error caused by the Network Rail Train Planning Unit who had not programmed the change within the Automatic Code Insertion system which is there to aid the Signaller.
The change had however, been recorded within an Altered Carriage Notice which the Signaller had not seen.This incident is still under investigation.

Liss (Haslemere-Petersfield)
During Emergency Special Working which was being carried out by Haslemere and Petersfield Signal Boxes, ‘Is Line Clear’ was not requested by the Haslemere Signaller for 1P69 after ‘Out of Section’ was given for 1P67.
The Petersfield Signaller realised Haslemere’s error and prevented the train from entering the section until a clear understanding was reached.
This had been a repetitive task which had led to an omission.

A member of Eastleigh Works Delivery reported that a colleague was opening the gate for the Up Siding car park.
As he drove toward it, he misjudged the gate and hit the gate post causing a dent to the passenger side door and braking a window.Golden Hour applied.

Hinchley Wood
The Emergency Intervention Unit (EIU) Mobile Operations Manager was queuing in traffic within the EIU vehicle driven by a British Transport Police (BTP) officer on the A244 Copsem Lane, near Hinchley Wood and a Skoda City Go drove into the back of them at low speed.
Golden Hour applied.
The metal bumper on the back of the EIU vehicle was damaged, the MOM and the BTP officer sustained no injuries.

Section Supervisor reported a closed call within item 145 when an RRV was de-stabilised and dropped items onto cattle grids multiple times resulting in slight damage. The RRV operator was stood down. This incident is subject to further investigation.

Worting Junction
DBS Freight 6O15 reported having a CAT A SPAD at BE183 signal on the down slow at Worting Junction. The Driver had had got off of the locomotive for personal needs and when he looked up at the signal, he thought it was showing a proceed aspect.
Having got back into the cab the Driver took power but soon realised that he had looked at the wrong signal.
The Driver applied the brake and the TPWS activated. The signal was passed by 10 yards.
This incident is subject to further investigation.

Farncombe
Farncombe signaller granted a line blockage to a COSS between WZ 6 (the down platform starting signal at Farncombe) and WZ 205, but had agreed a call back time of 11:30 which was when 2P27 was booked to stop at the station.
At 11:30 when receiving a call from the COSS who only gave his name, Signaller cleared WZ 6 to proceed thinking that he was giving it up when in fact it was a call to request a further 2 minutes.
The Signaller replaced WZ 6 signal to danger (red) in front of 2P27 which was still stationary at the time which prevented the incident escalating.
This incident is subject to further investigation.

Guildford
Whilst in two lanes of traffic on the A320 (underneath the A3 bridge at Stoke Park cross roads in Guildford), a car pulled into the outside lane and made contact at a very slow speed with the S&T van. No injuries and no hospital attendance required.

20/07/2016 – Wimbledon – Staff accident Member of Wimbledon S&T banged his head on the lid of a flame vault tool locker, causing a minor abrasion to the side of his head. First aid treatment was carried out and injured person was ok to continue work. Known fault on the Flam vault hinge. Close Call Report stuff before it causes an accident.
21/07/2016 – Fareham – Staff accident IP was utilising a spike extractor to remove spikes from baseplates when a spike split allowing the jaw to slip up and the handle of the tool shot up and struck a glancing blow to his chin, a plaster was fitted and he returned to work that night. Check all tools for damage before use and at regular intervals .
21/07/2016 – Totton – Possession Irregularity – Item 64 ODM reported whilst carrying out site visits, that he has discovered a possession irregularity within Item 64. The PLB and detonators were placed on the London side of 573 points instead of the country side. This was also beyond the limits of the possession albeit still within a Line Blockage. Upon initial investigation it had appeared that the diagram had been misread. The investigation is ongoing.
21/07/2016 – Feltham – Staff Accident (RTA). Member of Feltham S&T advised that when on route to a fault approaching a roundabout in the Hanworth area (Apex Corner), the van was driven in to the rear of a car in front. No injuries were sustained.
22/07/2016 – Reading – Staff Accident. Distribution and Plant Operative twisted his knee getting out of a van. He had immediate pain, but it went off. After 2 days however, the injury became swollen and was then reported. The injured party was happy to continue working however, further medical attention is being sought. Reminder: report all accidents at the time no matter how trivial the seem.

10/07/2016 Liss Level Crossing Petersfield Signaller reported that a Strapman had placed straps outside of the possession limits which caused Liss AHBs to fail. ODM reported that the mileage for the strap was incorrect. MOM on site and confirmed the incident to be a planning error. IO went to site and also confirmed that the straps had been placed in the correct position as per the paperwork provided.
10/07/2016 Wimbledon Wimbledon ASC reported a possession irregularity with 6Z08 exiting Item 36. The Driver had called from W176 having been advised by the PICOP to move up to the protection to clarify where the protection was. The next call received was from W172 outside the possession.
13/07/2016 Eastleigh 2B89 reported that railway staff were using the walking route with the white light extinguished. No near miss.
13/07/2016 Elm Road level crossing Member of staff from Eastleigh renewals reported an accident when rodding up into a cable troughing route. The resistance suddenly became loose causing his hand / thumb to come into contact with a concrete lid bending the fingernail back causing bleeding. PPE was being worn and first aid was administered.
13/07/2016 Croydon Senior Technical Officer Clapham depot reported that he was involved in an accident after picking up a Network Rail vehicle in readiness for night duties. Whilst driving along Windmill Road, a vehicle attempting to turn left into another road braked heavily having seen some girls who had jumped in front of the vehicle. The Network Rail vehicle hit the rear of the car in front causing damage to their rear bumper. There was no damage to the Network Rail vehicle or injuries to the Drivers.
14/07/2016 Ascot 5C09 reported hitting the platform edge while running through platform 2 causing slight damage to the protection bar by the driver steps. P-Way identified the platform edge to be out of gauge by 100mm. 20 mph speed imposed. The gauging appeared to be a result of recent works connected to the platform lengthening programme.
14/07/2016 Woking 2P23 had a Cat A SPAD on WK375 on the down fast line by approximately 2 metres. It was identified that the Driver had a momentary lapse in concentration and read across having seen the signal on the Down Slow clear. When moving forward, the Driver then realised but it was too late to react.
Trespass Incidents are on the increase – We must remain vigilant. We must challenge people on the line and report trespassers / suspicious people.
