Road Traffic Accident
Two members of Bournemouth S&T experienced a Road Traffic Accident whilst going round a roundabout when a car pulled out from the left-hand side.
The car struck the rear of their van, but there were no injuries.
The Golden Hour was applied.
25/06/17 Waterloo
Staff Accident (assault)
A member of Network Rail station staff confronted a well-known beggar in Burger King about begging on the station and asked him to leave.
The beggar raised his fists up to them and was restrained to the ground after the staff member felt threatened.
A SWT Driver saw the incident and assisted the member of station staff until BTP arrived.
The Golden Hour was applied.

12/06/2017 Byfleet and New Haw
Line side fire
4Y19 reported a line side fire on the Down cess near signal WK549.
Upon the MOM arriving, the fire had spread and covered 50 yards of the embankment.
By 17:14 the fire was out after the fire brigade attended.
The cause was felt to be connected to a tin can or a piece of metal foil that had come into contact with the conductor rail.
Please be aware that in dry conditions it can assist the acceleration of a fire, so be mindful the combustible materials that are around us.
A fire can start from a spark from a train pick up shoe or a discarded cigarette, so please look for the risks that are posed to us and report or remove them.
Remember to make use of the Close Call system too.
14/06/2017 Southfields
Cat A Spad 2174
A London Underground District Line service had a Cat A SPAD at W367 signal.
The Driver made no allegation against the signalling and stated that they had a loss of concentration.
This incident will be subject to an LUL investigation.
13:11-19:15 Eastleigh East Yard
Derailment
Balfour Beatty reported that one axle had derailed involving a Tamper on the Crain Pit Road over CJ points.
The Track Section Manager reported that there was no flange climb on the switch rail, but it had appeared that the front wheels had split the points and the axle that derailed ran down the back of the switch to the point of derailment.
There was some damage to the tips of the switch rail.
This incident shall be investigated by the Yard owner.
16/06/2017 Fratton
Cat A SPAD 2E64
South West Trains service 2E64 reported a Cat A SPAD at HT60 signal on the up main.
The Driver made no allegation against the signalling and stated that they had a loss of concentration.
This incident is subject to a review and follow up investigation.
17/06/2017 Pirbright Junction – Woking Junction
Possession Irregularity Item 35
Train 3Z10 reported to the Signaller that they had come to a stand at a stop board with detonators on the Up Fast Line.
Upon investigation it was established that a Vital Possession Support Staff (PSS) contractor had placed the protection incorrectly leaving the down fast unprotected.
This incident is subject to a Level 1 investigation.
The PSS had been briefed prior to applying the protection, but early indications show that there was a possibility that they had become disorientated with the layout once on site.
17/06/2017 Totton
Rough Ride / Rail Buckle
South West Trains service 1W65 reported a rough ride / kink in the rail at 573 points on the down line country side of the station which resulted in the line being blocked.
Upon the PWay attending a known location for rough rides, they noted a slight dip and authorised normal working. This was not the area where the defect was.
Once opened at line speed, 1W25 travelling at 65mph reported another jolt on the left hand rail of the down main on or near E577 points, a further train was cautioned and reported that the defect was very bad. The line was blocked until it was rectified overnight.
Early indications show that communications may have become confused and assumptions were made which took the PWay to the wrong location.
This incident is subject to a review and further investigation.
17/06/2017 Hook
6N01 Load Moved
6N01, the 1200 Vauxhall – Eastleigh East Yard reported that they had felt the load move.
The train had been within a Wessex Capacity possession and the load consisted of old track still fastened to their sleepers.
The load had been checked before it had left site by a Load Examiner.
The train stopped by BE401 signal between Hook and Basingstoke and the down lines were blocked due to the load now being out of gauge.
Upon a further examination by the Driver and Load Examiner (who had also checked the train earlier) it was identified that a strap was missing allowing the load to shift.
The down lines were affected for 3 hours until the train was safely recessed and dealt with at Basingstoke.
This incident is subject to further investigation.

Staff Accident
A PWay Team Leader was walking along the cess when some loose ballast gave way twisting his left knee.
An ice pack was used to reduce any swelling and the Golden Hour was applied.
This incident did not result in any lost time.
04/06/2017 Eastleigh Pre-Assembly Depot
Derailment
DB Cargo reported that a Coalfish wagon had become derailed between 4 and 6 Road over a set of hand points whilst carrying out a shunt move.
Upon examination by Network Rail PWay (who patrol, but are not responsible to maintain), they found that there had been 3 to 4 sleepers replaced consecutively after a previous derailment, but not packed.There was also evidence of soil / sand being used and not ballast.The incident was subject to a DB Cargo investigation and Network Rail patrolling responsibilities were visited.
06/06/2017 Sunbury
Staff Accident
A member of Feltham S&T was driving along a road in Sunbury when a the car in front braked to turn into a driveway.
The S&T Driver went to brake themselves, but their foot slipped off the peddle resulting in a low speed collision between 5 to 10mph.
There were no injuries sustained to either party and no apparent damage to the vehicles.
The Golden Hour was applied and there was no lost time.
06/06/2017 Tisbury
Late Acknowledgment of train horn
1L37 reported that a track worker did not acknowledge the train horn until the 3rd time of sounding it.
The Driver did not declare the incident as a Near Miss.
09/06/2017 White Hart Lane Level Crossing
Level Crossing Irregularity
The Signaller on Wimbledon Panel 5 lowered the crossing and cleared an Up line signal for train service 2U22 to proceed.
Upon doing so, they had realised that they had trapped a member of the public within the crossing next to the Up side Facing barrier and corrected their error by making an urgent call on the GSMR to the driver and replacing the signal to danger (red).
Once the signal timed out, the barriers were raised and the member of public released.
This incident is subject to further investigation.
09/06/2017 Eastleigh
Staff Accident
Upon carrying out de-vegetation work at the Electrical Control room, a Ground Control contractor noticed 7 ticks on his body.
Having been trained to do so, the ticks were removed safely.
Ground Control followed the incident up and there was no lost time.
10/06/2017 Barnes / Feltham Junction
Possession Irregularity – Item 115
Whilst travelling on a passenger train at approximately 14:54hrs, a member of Feltham PWAY noticed that the protection for the T3 possession may not have been applied correctly in the Hounslow / Whitton Junction area.
Upon the Operations Delivery Manager (ODM) attending site, it was identified that an irregularity had in fact occurred and that the detonator protection was placed 7 chains out of position and beyond the Work Site marker board.
The protection had been like this since approximately 01:16hrs.The ODM corrected the error made and arranged a follow up investigation into the incident which had involved McGinley contract staff.

A member of Havant S&T cut their left hand index finger.
First aid was given and they attended a walk-in clinic.

Brake Failure on a Road Railer
Whilst a Road Railer was being driven back from Witley to Havant, its brakes failed approaching a roundabout resulting in the vehicle going straight across it.
Apart from the Driver being in shock, no injuries were sustained.
The Golden Hour was applied and there was no lost time.
29/05/2017 Portcreek Junction
Worksite Irregularity
The Section Supervisor for Inner distribution (On Call) advised that he had been informed that Works Delivery staff had been working on point heating equipment whilst being live within T3 item 62.
Works Delivery do not hold this competency.
The Section Supervisor made the area safe by cutting out the electrical supply to the strips and would reconnect at a later date.
The incident was Close Called and is subject to further investigation.
30/05/2017 Surbiton
Staff Accident
An Inner Assistant Track Section Manager was changing trains at Surbiton and boarded a very crowded train.
Another passenger got on and pushed into their bag twisting it around and forcing their left shoulder into a hand rail.The Injured Party had an existing rotator cuff injury which was expatriated by the action.
The Golden Hour was applied.
02/06/2017 New Malden
Staff Accident
Upon the completion of works within a T3 possession and loading grinding equipment on the back of a van, a Welding Team Leader trapped his left hand causing some swelling and bruising.
The Golden Hour was applied and this was not a lost time injury.
04/06/2017 Kew Bridge
Staff Accident

04/06/2017 Eastleigh Pre-Assembly Depot
Derailment

A Contractor working on the behalf of IP was thought to be intoxicated during a COSS briefing.
When challenged, the contactor ran off.

Line Blockage Irregularity GZAC 5321705
The Woking Local Operations Manager reported that a Line blockage irregularity had occurred when the Signaller granted a block with a train in section and yet to pass the work group.
Although the Signaller came to a clear understanding about the arrangements, they had noted the wrong signal within their mind.
The COSS (working for IP) was still on the phone to the Signaller as the train approached, which prevented the incident from escalating further.
A near miss was not declared.
This incident is subject to further review and investigation.In addition to this incident, it was also identified that the planned Safe System Of Work did not fully cover the area required.
This had not been picked up by the COSS and will also be further investigated.
25/05/2017 Guildford
Unsafe Working
Two members of Works Delivery staff were witnessed crossing over 2 open lines in order to reach a T3 possession.
The possession concerned had all 6 lines blocked around Guildford and extended to Worplesdon and Shalford Junction.
When challenged, they reported that they had been taken off of another job and asked to attend to assist with disconnections which was why they had no paperwork or contact details for the ES or the COSS.
This incident is subject to further investigation.
28/05/2017 Woking
Staff Accident
A member of Woking S&T was walking along the track, went to turn around and tripped over the back drive of a set of points.
A layer of skin came off of the injured party’s left leg and it became swollen.
Apart from first aid being required, the injured party did not require further medical attention.

CAT A SPAD W367 (Set 2I72 LUL)
London Underground service 2I72 reported passing W367 signal at red (danger) without authority by half a coach. The Driver admitted fault.
Upon investigation it was identified that the Driver had 4 seconds of warning time due to the level of foliage.
The foliage was dealt with by the LUL contractors and it will be investigated by LUL managers.
18/05/2017 Lenthay Level Crossing
Staff Accident
A Works Delivery technician was one of a four man team loading a trolley onto a flatbed lorry.
One of the team slipped and the trolley hit the individual on the side of the head.
He initially felt ok, but on the drive home he had to pull over and was sick.
In addition to this some of his teeth felt loose after the impact, so went to hospital.
The hospital took X-rays of his neck and there was nothing untoward. The Doctor suspected whiplash and possible concussion. He was prescribed painkillers advised to rest.
The Golden Hour was applied and this accident became a lost time injury.
19/05/2017 Dorchester South
Safety of the Line Issue 1W12
1W12 was stopped by the Bournemouth Mobile Operations Manager with a red flag due to their vehicle becoming stuck within 3 to 4 feet away from the track.
The MOM had been carrying out some routine checks on the narrow roadway on the Up Side toward a relay room.
When the MOM went to turn their vehicle around, they were conscious of a steep embankment behind them which had hindered their ability to carry out the manoeuvre.
19/05/2017 Petersfield
Vehicle Accident
An outer Signalling Engineer was reversing out of a carpark space at the station when the vehicle bumped into a bollard.
It was very rainy at the time and it was thought that the vehicle had hit a curb.
Upon inspection, there was slight damage to the vehicle, but no injuries were sustained.
As there were no staff injuries, only a 6hr update was provided using the Golden Hour process.

05/05/2017 Corton Steps Level Crossing
Staff Accident
A member of Off Track was chain sawing a Hawthorne bush when it became caught on a piece of piece of wood.
The chainsaw whipped back and a thorn pierced through their glove into their finger.
Colleges on site cut off the glove, but the thorn had to be removed at hospital.
The injured party was put on gas and air whilst the thorn was removed and the hand dressed.
Golden Hour was applied and no lost time was anticipated.
09/05/2017 Haslemere
Staff Accident
A Cable and Track apprentice rubbed his right eye when some dust went into it whilst leaving a site of work.
Later that night his eye felt sore, so he cleaned it with eye wash and arranged to visit the hospital.
The hospital identified a small scratch on the eyeball and was advised if he was experiencing any other problems over the next couple of days, to see an eye specialist.
The Golden Hour was applied and no lost time was anticipated.

Staff Accident
A member of PWay was driving a work van to a work site when he was involved in a minor wing mirror collision with a delivery van.
There were no injuries and contact details were exchanged.
The Golden Hour was applied for chain of care and no time was lost.
10/05/2017 Eastleigh Pre-Assembly Depot
Locomotive 66238 Derailed
DB Cargo Duty Manager reported that one bogie of locomotive 66238 had become derailed within No 1 Siding.
The incident was said to be on Network Rail infrastructure and patrolled by Network Rail staff however, Network Rail were not responsible for maintaining the area.
Upon initial investigation, the rail appeared to have become out of gauge.
Investigations are ongoing.

Staff Accident
A member of Guildford PWay was cleaning the boot of the team pool vehicle when he cut his hand on some broken glass.
The broken glass was suspected to be a from the rear windscreen which had been broken previously.
The Golden Hour was applied and no time was lost.

Staff Accident
A Work Delivery Supervisor reported that a Vital Rail Operative had sprained his ankle by tripping over a large screw whilst walking on track within a T3 possession.
The injured party left the worksite, but decided not to attend hospital.
The Golden Hour was applied and he was expected to resume duty on Monday (15/05/17).

1P44 Near Miss
1P44 reported a near miss on the approach to the station at approximately 35m 15ch (Ashtead TP Hut) with 2 members of staff with no lookout. One had been within the four foot.
They reached a position of safety within 4 seconds of the train passing them and seen to be exiting the railway via an access gate.
This incident is subject to further investigation.

Near Miss Reported 2F26
2F26 reported a near miss London side of the station with an Individual Working Alone who was slow to acknowledge the warning given by train.
The individual reached a position of safety within 9 seconds of the train passing.

Staff Accident
A member of Woking S&T tripped on point rodding which resulted in him falling into cess and bruising his knee and elbow.
The Injured Party was able to complete their shift.
The Golden Hour was applied and apart from also suffering from a stiff neck, no lost time was anticipated.

Staff Accident
A Ground Control contractor was clearing vegetation and came across a bird nest.
Whilst tying black and yellow tape around it to alert other users, he disturbed a bees nest and was stung in the neck.
First aid was given where the penetration site was after it became slightly swollen.

Possessing Irregularity Item 34
Eastleigh ECR reported that a possible irregularity had occurred within won 3 item 34 part 7.
The ODM attended site and identified that the straps had been placed at 113m 65ch vice 113m 62ch due to a check rail.
The ECR had not made aware of the changes and it was likely that the B2 isolation documents had been incorrect for the past 3 nights.
This incident is subject to further investigation.
19/04/2017 Waterloo
Staff Accident
An E& P Engineer slipped whilst walking from Waterloo Station to an office in Paris Gardens and tripped on poor underfoot conditions as a result of ongoing building works.
The injured party twisted their right ankle.
First aid was administered by means of a bandage and an Ibuprofen tablet was taken for the pain.
The Golden Hour was applied and this became a lost time injury.

Staff Accident
Whilst walking to location case from the up platform, a member of S&T struck their head on a concrete section of a footbridge.
He was struck under the visor of his hard hat and experienced a headache and felt sick.
