Incidents last week

NVR – Network Rail vehicle involved in RTA at Eastleigh resulting in injury to the driver.
Wessex Outer DU – A MOS cut his right arm on the edge of the door at Mount Pleasant LC.
WX Inner DU – Technician trapped his hand whilst moving scrap rail at Weybridge resulting in a fractured index finger – NSC 27757.
Route Services – A driver got his/her foot caught under a rail at Clapham Yard. NSC 27797.
Southern IP – A contractor was knocked down outside the project delivery stores by a delivery vehicle Transit Van at Waterloo. (27781).
A contractor vehicle involved in RTA at Ascot, resulting in injury to both passenger and driver.
Wessex Inner DU – A PWAY MOS was putting in a check rail which slipped and caught his left hand middle finger, causing swelling at New Malden.
IP Southern – Jack handle jumped up when contractor was using it which struck him in the jaw causing bruising – NSC 27843
WX Outer DU – Apprentice was taking measurements for sideways movement with a steel rule when he caused a small flashover at Holme
2V43 (1537 Waterloo – Waterloo at W96 signal, Queenstown Rd– (Green to Red) – due to signaller error.
1W21 (1035 Waterloo – Weymouth) reported an irregular sequence at BC165 signal, Bournemouth – Due to signaller error
Wessex Inner DU – PICOP gave permission for 6N60 to leave the worksite at North Camp without checking the route was correctly set. (27879)
1L29 (1120 Waterloo – Exeter St Davids) at BE475 signal, Grateley – (Green to Red) – S &T error.
WX Outer DU – Apprentice was taking measurements for sideways movement with a steel rule when he caused a small flashover at Holme
OCC/COA – 2V43 (1537 Waterloo – Waterloo at W96 signal, Queenstown Rd– (Green to Red) – due to signaller error.

Winchfield
Contractor Accident A contractor for Works Delivery cut their thumb. It was cleaned and bandaged and required no hospital treatment. There was no absence from work. Golden Hour applied.

Wimbledon EMU Depot
Strap incorrectly placed Wimbledon Shunter reported Track Circuit ‘YJ’ showing occupied in the area of W707 points. Upon investigation it was established that the strapping position had been marked up incorrectly.

This is subject for further investigation.

Eastleigh / Portchester
T3 Item 84 Possession Irregularity Eastleigh ECRO reported a tripping in the Swanwick area to the PICOP. Upon investigation it was established that Straps were being placed for possession 84.

There were no injuries, but one end of the strap had vaporised.

This is subject for further investigation by Investment Projects.

Eastleigh South Junction / Fareham
T3 Item 84 Possession Irregularity The Havant signaller reported that 6J07 (a engineering train) had passed HT308 on a proceed aspect and then came across detonators and a Possession Limit Board.

The train had been travelling at 40mph. Upon investigation it was established that the PICOP had only communicated with the Eastleigh Signaller when the T3 was being arranged and had not realised that HT308 was a Havant signal.

This incident is subject to further investigation.

Staff Accident Eastleigh Works Delivery S&T was opening an access gate when the slide bolt became stuck. He then forced it resulting in the tip of the left index finger becoming trapped and cut. First aid was given on site.
13/10/2016
Woking
Customer services executive observed a person dressed in all orange and a white bump cap running within the DBS siding at Woking. DBS following this incident up.

5/10/16 – Winchfield dept –  IP was making  up cables. In the process of putting on  lugs some  wire went up inside his gloves and caught his thumb.  Resulting in a cut and minor bleeding.

Possession Irregularity
Project Manager from Works Delivery reported that when item 126 was taken last night, a rack out was required within the Kingston Sub Station as cable and conductor rails were being worked on close to the Sub Station.
A team had attended at 07:00 and disconnected cables and started work on conductor rail, but this work should not have started until the Supervisor had sight of a certificate of isolation to show that the equipment had been isolated.This incident is subject to further investigation.

The Wimbledon Panel 5 Signaller routed a train toward a line blockage whilst the S&T were undertaking works on Vine Road within a line blockage. Limits:
W552 came off to a proceed aspect for 2H90, but the Signaller realised their error and placed the signal back to danger.This incident is subject to further investigation.

5L34 10+25 Salisbury to Salisbury had a Cat A SPAD on SY240 Ground Position Light signal.
No allegation against the signal or unit.This incident is subject to further investigation.

2N56 – Cat A SPAD WK408 signal
2N56 (1700 Guildford – Ascot) passed WK408 signal at danger by two coach lengths. The train was bought to a stand by TPWS.

Staff Accident
An Eastleigh S&T Team Leader was working on a set of points and was using a railway bar to push them over.
This caused a jar his elbow resulting in a mild ache.
No first aid given or hospital attendance was required.
Golden Hour was applied and he was able to resume for his next duty.

A member of Havant PWAY bruised the top of his foot when moving panels at the level crossing.
No first aid given or hospital visit required.
Golden Hour was applied and he was able to resume his next duty.
