1P44 Near Miss
1P44 reported a near miss on the approach to the station at approximately 35m 15ch (Ashtead TP Hut) with 2 members of staff with no lookout. One had been within the four foot.
They reached a position of safety within 4 seconds of the train passing them and seen to be exiting the railway via an access gate.
This incident is subject to further investigation.

Near Miss Reported 2F26
2F26 reported a near miss London side of the station with an Individual Working Alone who was slow to acknowledge the warning given by train.
The individual reached a position of safety within 9 seconds of the train passing.
This incident is subject to further investigation.

Staff Accident
A member of Woking S&T tripped on point rodding which resulted in him falling into cess and bruising his knee and elbow.
The Injured Party was able to complete their shift.
The Golden Hour was applied and apart from also suffering from a stiff neck, no lost time was anticipated.
27/04/2017 Eastleigh
Staff Accident
A Ground Control contractor was clearing vegetation and came across a bird nest.
Whilst tying black and yellow tape around it to alert other users, he disturbed a bees nest and was stung in the neck.
First aid was given where the penetration site was after it became slightly swollen.
The Golden Hour was applied and this was not a lost time injury.

Possessing Irregularity Item 34
Eastleigh ECR reported that a possible irregularity had occurred within won 3 item 34 part 7.
The ODM attended site and identified that the straps had been placed at 113m 65ch vice 113m 62ch due to a check rail.
The ECR had not made aware of the changes and it was likely that the B2 isolation documents had been incorrect for the past 3 nights.
This incident is subject to further investigation.
19/04/2017 Waterloo
Staff Accident
An E& P Engineer slipped whilst walking from Waterloo Station to an office in Paris Gardens and tripped on poor underfoot conditions as a result of ongoing building works.
The injured party twisted their right ankle.
First aid was administered by means of a bandage and an Ibuprofen tablet was taken for the pain.
The Golden Hour was applied and this became a lost time injury.

Staff Accident
Whilst walking to location case from the up platform, a member of S&T struck their head on a concrete section of a footbridge.
He was struck under the visor of his hard hat and experienced a headache and felt sick.
The Golden Hour was applied and there was no lost time.

Item 75 – Worksite Irregularity
Outer ODM carrying out checks on a Works Delivery possession discovered a worksite irregularity within item 75 where the straps had been placed in the wrong location.
The straps are meant to be at 31m 36ch on the Up and Down Guildford lines, but were placed at 31m 40ch on the Up and Down Guildford lines.
In addition, the strap on the Up Guildford was located between the Detonators and the Possession Limit Board.
The ES arranged for the straps to be moved into the correct possession.
This incident is subject to further investigation.

Staff Accident
A Works Delivery Welder working within item 75 was installing pads under the rails.
When he went to tap in a wedge with a small welders hammer, he missed the wedge and caught his thumb and knuckle on left hand causing a slight cut, but no bruising.
First aid was administered and the Golden Hour applied.
No lost time anticipated at this time.

Staff Accident (Contractor)
A Ganymead Contractor working for Eastleigh Works Delivery within item 75 sustained a minor burn on his left wrist whilst he was changing the welder’s cooper plate for the base plate and leant on the weld.
Gel from a burns kit was applied to the area which prevented any marks or blistering to appear.
The Golden Hour was applied and no lost time was anticipated.

A member of PWay staff trapped their right hand little finger whilst lifting a trolley onto platform 5.
The Golden Hour was applied and there was no lost time.

A Technician from Clapham DU suffered a leg cramp whilst driving which caused him to accidently depress the accelerator and crash into a lamp post.
Apart from suffering shock, there was no visible injuries post incident.

SY48 CAT A SPAD
5O86 passed SY48 signal at Danger without authority and was brought to a stand ¾ coaches beyond the signal by a TPWS intervention.
The Driver advised that he had read across to SY50

Staff Accident
The Woking MOM was leaving a parking bay within the station and caught the off side of the van on a crash barrier.
The vehicle was being driven forward at the time, having been reversed in.
The Golden Hour was applied for chain of care, although there were no injuries.
15/04/2017 Waterloo
Points Run Through 1524C Pts / Possession Irregularity
2F33 reported 2 changes of aspect on W25 Signal. (Green, Red, Green, Red).
It was established that there had been a loss of detection on 1524c points which had been caused by an engineering train running through them as a Points Operator moved the wrong set whilst the train was in motion.
The Points Operator had been under the instruction of a site manager (not an engineering supervisor) when the incident occurred.
The Points Operator was meant to move 1517 points, but due to 1524 points not being labelled he had assumed it was the right set.
When he started to move them, he realised the error but it was too late.
The Engineering Supervisor was not aware of the instruction having been given, nor that a Points Operator was on site.Several concerns were raised about processes within this possession having not been followed, which will be subject to further investigation

Operational Close Call / Possession irregularity
The PICOP of item 11 (Waterloo – Queenstown Road 00:15 – 04:00 Mon) reported that hook switch 2163 (platform 11) was closed, when it should be open.
Further investigations were undertaken to understand how this occurred, but unfortunately it has not been established when the error was made for us to understand the chain of events and learn from it.

Strapping Issue/Possession Irregularity
An ODM who had investigated as to why a Line Blockage had been refused, identified that other line blockages associated with item’s 43, 52, 62 and 69 in week 52 to facilitate strapping out, had not been used either.
It had appeared that the ES for these possessions may have applied an agreed strapping trial, but had not taken into account that this was specifically aimed at Network Rail staff within a particular location.
This incident is subject to further investigation.

Trespasser / Verbal assault
PICOP for Item 6 advised that the ES reported a trespasser (a male in his early 20’s), in his work site who was aggressive and angry.
The ES had not been physically attacked, but had been verbally assaulted in this incident.
Chain of care was carried out and BTP were informed.

71. Near Miss / Irregularity Item 153
5C88 reported a near miss with staff 1 coach length beyond F328 when they reportedly crossed in front of the train.
The Driver reported also reported striking conductor rail shrouds.
Osborne were working within Item 153, which was a line blockage between trains and whilst arrangements were made to apply additional protection, the Controller Of Site Safety had begun work without the Protection Controllers (PC) authority.
The PC had been aware of the approaching train.
This incident is subject to further investigation.
08/04/2017 Clapham Yard
Derailment
SWT Movements Manager reported that loco 47848 had derailed whilst carrying out a propelling movement and roads No.1 to 41 were blocked.
The incident appeared to have occurred over No.5 points and is under further investigation by SWT.
Thanks go to Network Rail who assisted with the recovery of this incident.

Staff Assault
A member of Waterloo S&T reported that he had been assaulted outside the station, outside the depot, getting items out of top box on his motorbike.
A drunk male started to tamper with his bike and was politely asked to stop.
The male continued to tamper with the bike and the member of S&T pushed the male away, but was punch in the left hand side of his jaw which became sore.
He also sustained a few scratches.
The Golden Hour was applied and he was able to continue his duty.

The Outer ODM was carrying out checks on a Works Delivery possession discovered a worksite irregularity where the straps had been placed in the wrong location.
The straps are meant to be at 31 miles 36 chain on the Up and Down Guildford lines, but were placed at 31 miles 40 chain which had effectively increased the worksite by 4 chains.
The strap on the Up Guildford was also located between the detonators and Possession Limit Board.
Upon this being identified, the error was corrected.
This incident is under further investigation.

27/03/2017 Rowlands Castle
1P56 Bang (Object struck)
1P56 reported a bang under the train at the Buriton bank area on the up line and lost power to the train causing unit damage.
The MOM carried out a site visit and at Idsworth (up line 61 miles and 14 chains), found some loose arms that should have been attached to bomax matting.
The matting had been removed, but there were some metal bars which were attached at one end, but not the other within the four foot.
A Senior Technical Officer visited site and found some loose bolts on the RRAP which may have caused the incident. He tightened them securely and planned to carry out a track walk with lookout protection the following day.

1P71 Cat A SPAD HT59
1P71 had a Cat A SPAD at HT59 signal approaching the high level.
The signal had been at danger due to 1F99 in the platform ahead.
The TPWS intervened and applied the brake.
No allegation was made against signalling.
Early indications point toward the Driver losing concentration due to a number of activities the Driver is expected to focus on within a very short distance.

Hounslow
Staff Accident
Whist stepping over rails, an E&P Operative stubbed his foot and stumbled to the ground having twisted his ankle.
His ankle became very sore and swollen.
The Golden Hour was applied and the Injured Party visited the hospital the following day.
They were diagnosed was having soft tissue damage and were advised to rest for a few days to allow it to recover.
The Injured Party returned to work on nights on 02/04/17.

National Health & Safety week runs from 2nd April to 8th April.
To mark the start of the week Becky Lumlock and AJ Yates hosted a live safety hour on Monday 3rd April at Basingstoke. This was focused on collaborative working and how we can work together to proactively reduce harm.
We had a fantastic turnout on the day with around 90 people attending.
