GZAC Declined
The Basingstoke WOE signaller declined a GZAC Line Blockage between SE2819 points to SE4740 points owned by Osborne, as the worksite was Crewkerne tunnel.
Had the Signaller not queried where they were working, they would have been at risk from approaching trains.
This incident was Close Called.

Woking Down Yard
Staff Accident
An Operative from Guildford off track was putting a petrol blower back into the van when he accidently touched the hot exhaust section of the blower causing a burn on the tips of 3 left hand fingers.
First Aid was given and the Golden Hour was applied.
No lost time was anticipated.

Finchdean Sub-Station / Rowlands Castle
Near Miss / Operational Close Call
A Senior Contracts Manager from Environmental Forestry advised that a near miss had occurred with a South West Trains service and a Lookout crossing a limited clearance bridge (ref: 17/100 at 62m27ch).
The bridge was 18 metres in length and the Lookout was said to have reached a place of safety within 4 seconds of the train passing.
The incident had initially been reported as a Close Call to Control and had not been reported as a Near Miss by the Driver.
This incident is subject to further investigation and is awaiting further evidence to fully understand events.

Staff accident
A Clapham Team Organiser was bumped into accidentally by another passenger on some stairs and she consequently landed awkwardly and sprained her right ankle.
She was offered first aid at the time but declined, although did visit A&E who confirmed the sprain and was advised not to put any unnecessary weight on it.
The Golden Hour was applied and lost time was anticipated.

Andover Yard
Andover Yard
A Wessex Route Health and Safety Rep reported that he was carrying out a follow up visit after finding the access gate to the Yard being left open.
The gates were in such a state, that they could not be closed and secured.
It also appeared that they had been like this for some time, as there were a number of public cars and good vehicles parked within the boundary.
This was Close Called.
Note:
This type of issue is currently being presented at the Safety Workshop and poses a risk to the public.
Please ensure all access gates are in good condition and are secured correctly after use.

Leatherhead
Coping stone displaced
An On Track Machine hit a coping stone within platform 2 at the 18 mile post.
The stone was removed and repositioned, but was still a tripping hazard.
The MOM attended and sprayed some signage to highlight the area, but also noted other stones in a similar condition requiring repair.
The fault was reported to the Property Action Line.

CAT A SPAD
1C93 (1732  Southampton Central – Barnham) questioned a TPWS activation at E811 signal on the down Netley line after he thought it had been displaying a single yellow.
18:31 – The S&T logger was reviewed which confirmed the signal to have been at danger due to another train (1T49) occupying the platform ahead.
00:10 – Eastleigh S&T completed their testing post incident and all was in order.

Late Reported Staff Accident
A member of Clapham PWAY reported that he had cut his finger on 12/03/2017 13:30 whilst removing a seized bolt on an IBJ. As he tried to remove a piece of metal that was obstructing the nut, he cut his finger through his protective gloves.
He visited the hospital where the wound was cleaned and dressed.
Due to the late report, the Golden Hour was not applied.

Contractor injury
A McGinley’s contractor member of staff was working on W1502B points when he tripped over a rail in the 4 foot hurting ankle.
He was assisted off of the track and was taken to hospital by another contactor from McGinley, where he was diagnosed as having sprained it.
The Woking Section Manager acted as a point of contact and maintained communication with the Contractors manager and WICC for updates.

Contractor injury
A contractor working for SGL Forestry cut his arm on a stationary chainsaw which required 5 stitches.
The Golden Hour was applied and he was able to return on light duties.

Line block irregularity
A Vital Protection Controller gave up a Line Blockage with the Feltham Signaller and as the train (5O02) passed him, but was unsure if his hand signaller who had been responsible for keying back F297 to danger had been in in a position of safety beforehand.
Vital carried out a welfare check and were considering conducting their own investigation.

5Z15 – Operational irregularity
An Emergency REC call was sent to the Driver of 5Z15 by the Wimbledon Panel 5 Signaller when he noticed the train making a wrong direction movement without authority after it had performed a shunt.The incident is subject to a review and further investigation, but early indications show that the train had not travelled far enough in rear of W1090 ground position light signal due to the Driver not being able to see the stop car marks which were dirty.Upon the Driver then changing ends, he began to move forward to sight the signal which was now behind him.The Wimbledon MOM attended and cleaned up the stop car marks and created a new temporary 10 car stop mark to mitigate against further incidents.

Staff Accident
A member of Works Delivery staff was lowering the jack on a rail when his middle finger on his left hand got caught and cut open.
First Aid was given on site and he visited St Thomas’ Hospital where he was given pain killers, a tetanus injection and was signed off work for a week.
The Golden Hour was applied.

West Crossings
Points run through – 6N01-1590pts-Item 3
A Works Delivery Manager reported that engineering train 6N01 had ran through 1590b points and half the train was on the Up Windsor with the other half on the Down Windsor.
A waggon which had been traversing the two lines on pivot axles had also collided with an RRV trailer carrying a tamping unit.
Upon investigation it was discovered a team of 3 testers were on site and within the Waterloo relay room at the time and had been carrying out their own work when the points moved causing the incident to occur.
There were no injuries, but the infrastructure was damaged which required repair.
The work required put the possession times back and it overran Monday morning causing significant delays and cancellations to the morning peak.
This incident is subject to a review to fully understand what went wrong and an investigation will follow.

Eastleigh Depot Carriage Siding
Possession Irregularity Item 505
5M18 reported running over a T3 possession detonator which had not been removed close to E245 Ground Position Light signal on exiting No.1 road from the depot.
This incident is currently under investigation.

Dorking / Leatherhead
Line Blockage Irregularity
Wimbledon Panel noticed ‘LK’ track circuit flick on the Up line by W492 signal.
Upon speaking with the Dorking signaller it was established this was caused by S&T who had a line blockage from CBK37 at Dorking to W492 signal at Leatherhead Jnc.
Dorking had not advised Wimbledon that this was in place.
This incident is subject to a Sussex investigation.

Strawberry Hill Level Crossing
Operating Irregularity (P) Item 115
The Feltham Shift Manager reported that an irregularity had occurred with a tamper (6J61) which had been authorised out of the possession.
The Signaller had used the correct route card for the movement, but had overlooked to lower Strawberry Hill CCTV barriers.
The tamper traversed over the crossing with just the read road lights flashing which had illuminated due to the train movement.
Due to the time of night, no users were on the crossing at the time.
This incident is subject to further investigation.

Staff Accident
A member of Havant PWay was undertaking a two person lift with a saddle of a stressing kit. The other person who had been assisting the Injured Party tripped and lost his balance on other tools on site.
The Injured Party consequently lost his balance himself and was now carrying the load and felt a sharp pain in the lower right hand side of his back.
The Golden Hour was applied and no lost time was anticipated.
This accident is subject to further investigation.

Possession Irregularity Item 68/ 6J29
The ODM reported that an irregularity occurred within item 68 where a Tamper (6J29) had been authorised out of a worksite and to go to BE399 signal on the Down Fast for the possession to be cleared around the train.
The Tamper Driver however; continued to BE407 having become confused with the previous nights arrangement.
This incident is under further investigation by Balfour Beatty.

Cat A SPAD – 2H10
2H10 experienced a Cat A SPAD at W50 on the Up Main Slow after losing concentration.
The train passed the signal by 3 ½ coaches.
No allegations were made toward the signal and remained in use.
The Train was authorised to continue to Waterloo to prevent trains being delayed and put at risk in rear.
This incident is subject to further investigation.

Staff Accident
A member of Woking S&T was taking part in a three man lift with an AWS electro magnet.
The team were crossing from the down to up line, but due to uneven ballast and the third rail, the team aborted the lift and tried again in a more suitable location.
Post the lift, one of the team felt a strain in their lower back muscle.
The Golden Hour was applied and no lost time was anticipated.
This accident is subject to further investigation.

35. Staff Accident
An Assistant Team Leader was driving a flat bed and slowed down as his neighbour was stood at the passenger door of their car.
As he did so, the neighbour’s dog pushed the rear door open which caught the rear of the flat bed.
There were no injuries.

Clapham Junction
85. Staff Accident
A member of Clapham P-Way was working within the depot and lifting part of some stressing kit onto a pallet
Upon lifting it about 4 inches off the ground, he sustained shooting pain in back having pulled a muscle.
Golden Hour applied.

Christchurch
34. Staff Accident
A member of staff from Wimbledon Works Delivery tripped over a fishplate spanner in the 4 foot and fell.
His knee hit the ballast causing a cut and his body hit the rail causing a lot of pain.
He attended Southampton General Hospital to have the knee glued closed.
The Golden Hour was applied and the injured party returned for duty the following night.

13. Staff Accident
A member of Basingstoke S&T twisted their right ankle after ballast gave way underfoot near the end of platform 1.
No first aid required.
The Golden Hour was applied and the injured party returned to work the following night.

Poole
Possession Irregularity
An irregularity occurred within T3 item 508, a possession between Poole and Wareham which included the Hamworthy goods line and No.1 and 2 sidings at Poole.
Signal protection had been granted 00:41 and as Blocking staff were laying their protection in the Poole area, a train from the sidings began to depart into the station due to a Signaller error.
There were no injuries and incident was not declared a near miss.
This incident is subject to further investigation.
