Surbiton
Staff Accident
A Works Delivery contractor was transporting a 150lb length of conductor rail with an Iron Man.
Once on site the individual was lowering the con rail and it slipped from the ratchet and landed on his right foot.
The pain got progressively worse and an ambulance was called.
The injury was confirmed to be a Lost Time Injury due to a broken metatarsal and put in a cast.
Golden Hour applied.

Wimbledon
Item 7 Possession Irregularity
6U41 Stone blower was working in item 7 WON 43 at Wimbledon.
The Driver was asked to go into platform 6 by the ES, but stopped short of the intended location due to an engineering trolley on the line in front of the train being used by the Raynes park CRE team who had mistakenly placed it there due to a breakdown in communication.
The train was doing no more than 5mph.
This incident is subject to further investigation.

Staff Accident
S&T Section Manager slipped on wet floor within the lobby area resulting in a strain to the bottom left hand side of his back.
The water had been bought in from outside and a mat was not in place.
A “Caution – Wet Floor” sign was erected in lobby post incident.
Golden Hour was applied
No lost time was anticipated.

A member of PWay was loading a small jerry into a box when the petrol splashed up into his eyes.
Eye wash was used and Golden Hour applied.
No lost time was anticipated

2O37 – CAT A SPAD
Train service 2O37 passed F17 signal at danger (red) believing it was displaying a single yellow.
This incident is subject to further investigation.

Wylye Level Crossing
Staff Accident
A member of Salisbury P-Way was working on the Warminster line slewing rails and putting in nylons during wet conditions when he slipped. He fell forward onto hands hurting his wrists.
Golden Hour was applied.
No lost time was anticipated.

Train service 2C73 passed W79 signal at danger (red) believing it was displaying a single yellow.

Near Miss 2D27
2D27 reported a near miss with a member of Clapham PWay staff approaching the station.
The Driver stated that he had sounded the horn and member of staff acknowledge the warning, but kept walking towards the track. The emergency brake was applied. This incident is subject to further investigation.

Incident Waterloo Platform 15
It was noted by the Station Control that a member of Platform Staff was retrieving something off the track with Litter Pickers without a block taken. This incident is subject to further investigation.

Farnborough
Staff Accident
A member of Works Delivery staff working within the MMT at was coming down the steps within the train stepped onto the ballast and his right ankle gave way. He fell, but did not sustain any other injury.
The injury had been pre-existing, but all the correct PPE had been worn.
Golden Hour applied.
No lost time. A referral to Rehabworks was being considered.

Dorchester West Station
2O66 – Struck Detonator
2O66 reported to have struck a detonator on the approach to the station (DR179 signal). The Signaller advised there had been a T3 in the area overnight (WON 42 Item 113).
09:09 – Yeovil MOM found the remains of the detonator. ODM made aware. This incident is subject to further investigation.

A Wimbledon Welder was loading welding equipment onto the van when the generator slipped and fell onto the middle finger. (Left or right not declared).
Golden Hour was applied.No Lost Time was anticipated.

A member Off Track was sprayed with effluent when GWR 1F14 (1023 Portsmouth H and Cardiff) passed.
They finished their shift to go home and clean up.
16:15 – The individual had a Hepatitis jab.
Golden Hour applied.No lost time.

2L52 CAT A SPAD
2L52 (1524 Basingstoke to Waterloo) had a CAT A SPAD at WK130 signal on the Up Main Slow.
Pictures from FFC show the signal was displaying a red aspect on approach, although the Driver stated that he has thought it had been showing a double yellow due to sunlight shining on the signal.
The train passed the signal by 1 coach length.This incident is subject to further investigation.

Staff accident
When attending to a swan on the line, the Feltham MOM sustained an injury at the top of his leg, underneath the buttock.
Golden Hour applied.
There was no lost time.

Staff Accident
A member of Works Delivery staff was about to treat an icy patch within the DU compound when he slipped and broke his upper right arm humerus bone.
Golden Hour was applied.
This was a Lost Time injury.

A member of Salisbury S&T slipped when attending to a signal post telephone failure at E468 signal.
He put his hand out to stop his fall, but had chipped a bone in his hand as a result.

2P62 Platforming irregularity
A 5 carriage train was signalled on top off an 8 carriage train leaving a coach not being accommodated.
No doors were released and a shunt to rectify the situation was arranged.

Chertsey Level Crossing
Crossing misuse irregularity
The Signaller working Feltham Panel 4 lowered the barriers and pressed crossing clear with a person within the crossing. F301 signal also cleared to a proceed aspect.
The Signaller realised the error, but saw the person leave the crossing by climbing over the barrier.

A massive thank you to everyone who worked over the Christmas period!  We had a lot of engineering work and other trackside activities going on and I am pleased to say that we have not had any accidents.

Longhedge Junction
Possession Irregularity – Item 64
An Engineering Supervisor of Herne Hill PWay arrived at his worksite and found some BCM contractors already working there.
The Contractors had not signed in with this ES, but had signed in with another who was not connected to this worksite.
The COSS had used a Live Line Tester before work took place.
On further investigation, three other earlier COSS’s had made the same error by contacting an incorrect ES but had been directed to the right person.
This incident is subject to further investigation.

CAT A SPAD 6V41 E763
6V41 reported passing E763 signal at danger on the Down fast line after cross reading E765 signal.
The Driver realised his error and applied the emergency break, but the train passed the signal by just over a loco length.

Queenstown Road
Item 3 from 0100 Sat 24th December – 0600 Tuesday 27th December
The controller overseeing the Kirow 810 crane was attempting to line up 2 long bearers with a switch panel.
During the operation the front 2 wheels of the first bogie went over the end of the rail.
The crane was able to reverse and there was no infrastructure damage.
This incident is currently under investigation by IP.

Points Run Through 745A
745A points by platform 1 were ran through an LUL engineering train after an ES error in not setting up the route correctly with the Signaller.
This incident is subject to further investigation by LUL.

Wimbledon Park (Depot exit)
Operating incident (Item 60 TFL)
Raynes park ECRO advised that a tripping occurred on section R173.
Engineering Work Manager to advise that a Track Operative working for London Underground was conducting track maintenance was drilling railway sleepers on the Westbound that was under possession. The Operative had stood the drill up which then fell over bridging the gap between the Westbound running rail and a conductor rail leading to the sidings which was not under possession.

Staff accident
Whilst attending to a swan on the line, the Feltham Mobile Operations Manager sustained an injury at the top of his leg, underneath the buttock.

The MOM was able to continue duty with no lost time.
