Worksite Irregularity WON 19 Item 5  Possession
The PICOP reported that when changing Item 5 from Part 1 to Part 2, two short circuiting straps blew off.
The straps were required to be at 1m 32ch, but had been placed at 1m 34ch in error.
There were no injuries and this incident is subject to an IP investigation.

7O43 Derailment
DB Cargo reported that 7O43 had derailed whilst performing a shunt back into the East Stone Terminal.
Upon inspection, the derailment had occurred at either 1C or 1D points and caused substantial damage to 1A points.
The nature of the derailment is unknown currently and subject to further investigation.

Late reported staff assault
A Lookout from Ganymede was looking out for a team, when youths began to throw stones (Small and Big) at him.
He did not sustain any injuries and was re-positioned into a safer location.

Staff accident
A Wimbledon grinding team were lifting a frog grinder down onto the track from the platform, two men were at track level, while the third man was atop the platform. The men on the track were lowering the grinder to the track but it dropped faster than expected, causing one of the team members hands to be trapped between the grinder and the platform.
The injured person sustained a deep cut on his small finger and was taken to the hospital.
The injury was cleaned, x-rayed and stitched. There were no broken bones.
He was then discharged and advised to come back after ten days to have stitches removed.
The Golden Hour was applied and no absence from work was anticipated.
This incident is subject to further investigation and the use of this grinder is under review to mitigate against further harm.

CAT A SPAD W107 – 5Y17
South West Trains empty coaching stock 5Y17 experienced a Cat A SPAD on W107 signal having read across to W109 signal showing a proceed aspect.
The train passed the signal by approximately 5 metres.
There was no allegation made toward the signalling equipment.
This incident is subject to a review and follow up investigation.

Possession Irregularity Item 70
The PICOP of Item 70 reported that two Vital Rail staff had incorrectly applied protection at Winchfield instead of Winchester.
Initial investigations suggest that they had been suitably briefed and had used good protocols, until the error of being at the wrong station was realised.
This incident is subject to further investigation by the Operations Delivery team.

CAT A SPAD W153 – 2D67
South West Trains passenger service 2D67 experienced a Cat A SPAD on W153 signal having been distracted by a loud AWS warning.
The train passed the signal by approximately 3 metres.
There was no allegation made toward the signalling equipment.
This incident is subject to a review and follow up investigation.

Addlestone Crossing
Whilst loosening bolts on an Addlestone level crossing pedestal, Injured Person felt a sharp pain in lower back. The Injured Person assumed this to be a muscular injury, but hadn’t seen a specialist to confirm. Injured Person advised this was reported late as he didn’t get the opportunity to report at the time, due to the WICC informing the IP they were too busy.

An ES and his assistant were instructed to place protection on Siding number 6 and to open hookswitch 8113. Following this, they were instructed to test and strap, adjacent to H/S 8113, but discovered that the 3rd rail was live. They then strapped the 3rd rail country side of shunt signal W1044, as it became apparent to them that this was where the strap was meant to be placed. The team believed this to be an error with the B2.
PICOP was informed of the issue, who then raised the issue to the ECO.
Team were then called back by the PICOP, asking what side of W1044 shunt signal the strap had been applied to, team informed the PICOP that it was country side of the 3rd rail.
PICOP informed the team that the strap should be London side of W1044 shunt signal according to the ECO, despite the ES clarifying numerous times, that the con-rail was live and couldn’t be strapped there.
ECO was adamant that it was a leaky hookswitch causing the live reading and not an energised 3rd rail.
ES was then told the ECO would remove the breaker and that the ES was to test and apply his strap, adjacent to hookswitch 8113, even after stating this would blow the strap up.
The strap was placed and team stood clear, as instructed and the breaker was put back in and consequently the strap was blown. This incident is subject to further investigation.

Information about what a SPAD is from last week

Category A Signals Passed At Danger (SPADs), where signals have been passed when a stop aspect, end of in-cab signalled movement authority, or indication (and any associated preceding cautionary indications) was displayed correctly and in sufficient time for the train to be stopped safely at the signal or end of in-cab movement authority.

  • A SPAD (Previously Category A SPAD) is where the train proceeds beyond its authorised movement to an unauthorised movement.[8]
  • A Technical SPAR (Previously Category B SPAD) is where the signal reverted to danger in front of the train due to an equipment failure or signaller error and the train was unable to stop before passing the signal.
  • A Signaller SPAR (Previously Category C SPAD) is where the signal was replaced to danger in front of the train by the signaller in accordance with the rules and regulations and the train was unable to stop before passing the signal.
  • A Runaway SPAR (Previously Category D SPAD) is where an unattended train or vehicles not attached to a traction unit run away past a signal at danger.

Staff Accident
A member of Wimbledon S&T sustained a tick bite whilst working within the train depot.
He visited his doctor the following day and blood was taken for tests for Lyme disease.
The Golden Hour was applied and this became a non-lost time injury.

CAT A SPAD W2 (2M50)
2M50 had a CAT A SPAD on W2 Signal and stopped with the cab just under the gantry.
There were no allegations against the signalling after the Driver reported that he heard a noise out of his open window and looked across to see what it was.
This incident is subject to further investigation.

W1600B Points run through
A member of the Clapham PWay reported that 1600 B points had been run through and damage had been sustained to 3 stretcher bars which would require replacement.
Upon investigation, it was discovered that engineering train 6C03 had ran through them after the Engineering Supervisor and Driver thought they were set correctly by viewing them from the cab.
This incident is subject to further investigation

Staff accident
A member of PWay staff hit his head whilst climbing into the work van.
He sustained a little bump on the head and some pain in the neck.
The Golden Hour was applied and there was no lost time.

Staff Accident (Taken ill)
A Mobile Operations Manager attending to a fatality suffered chest pains and was given immediate first aid by a BTP officer.
The Ambulance service once on scene carried out CPR and he was taken to hospital for treatment.
Later in the afternoon he was reported as being sat up in bed and talking.
The Golden Hour was applied and although the MOM will be off work recovering, this will not be classed as a Lost Time Injury.

Possession Irregularity – Item 49A
The Eastleigh Shunter arranged to accept engineering train 6G16 into Eastleigh East Yard, but discovered that the detonator protection and Possession Limit Board for item 49A (Shawford – Eastleigh East Yard 00:40-16:00) had been placed the wrong side of 440 points.
The PICOP was advised and arrangements were made to correct the error.
This incident is subject to further investigaton.

Operational irregularity
A Workforce Health, Safety & Environment Advisor advised that he had seen a member of PWay on the down line re-erecting ESR warning boards without wearing the correct or full PPE.
This incident is subject to further investigation.

Elm Road Level Crossing
Flashing light in the 4 foot
South West Trains Service 2H14, reported a flashing red light that looked to be of the same design as the ones that are placed on skips within the four foot.
The lamp was removed by 6N02 (an engineering train) and placed into the cess.
Upon further investigation, it was established that the lamp had been used within a T3 possession to protect the Level Crossing and had not been removed prior to the block being given up.
This incident is subject to further investigation.

Staff Assault/Accident
An S&T Team Leader reported that whilst he was working within a location cabinet by F194 signal, 2 teenagers were throwing stones at him.
One of the stones struck him on the knee aggravating an existing injury.
The Golden Hour was applied and this became a lost time.

Cat A SPAD LUL
1I54 reported having a Cat A SPAD at the W3590 repeater signal.
The train passed the signal by 2 to 3 coaches through a loss in concentration.
There was no allegation made against signalling equipment.

Wimbledon
Vehicle Fire
A Works Delivery signal works Senior technician reported that a vehicle fire had occurred within the maintenance depot at Dundonald Road.
The fire was caused by a used miniature gas torch which had been placed in a bag under the middle crew cab seat.
There were no injuries sustained and the fire was extinguished with water.
Minor damage was sustained to the underbody of the seat.
This incident is subject to further investigation.

Staff accident (Assault)
2 males under the influence of alcohol got out of a stationary South West Trains service on the Up Main Slow just outside the station and crossed all lines until they reached the International Terminal.
One of the males then struck a Skanska Contractor over the head with a Champagne bottle.
This incident was followed up by Wessex Capacity locally.

Possible Unsafe Working 5Y00
A South West Trains Driver on a set of empty coaching stock under instruction reported that they had applied their emergency brake after coming across a P-Way gang working in the 4 foot.
The Driver stated that the Lookout had not allowed sufficient distance.
Post reviewing the Forward Facing footage, the Section Manager reported that the P-Way were in a safe place in enough time.
A near miss had not been declared by the Driver or PWay team.
(Note: The Driver had previously driven for Great Western and was experienced over this line of route).

Staff Accident
A member of Guildford S&T was walking along the down cess towards Ash TP hut when he came across Blue vortok fencing that was prohibiting him.
As he went to walk around it, he tripped gashing his left shin on a newly laid section of troughing route and cut his lip on the vortok fencing.
The Golden Hour was applied and there was no lost time.

Staff Accident (RTA)
Ganymede contractors were driving to a work site to assist with installing an ESR.
The vehicle they were in stopped at a Pelican crossing, but was struck from behind by another vehicle travelling between 30-40mph.
Following the RTA they were sent home and one was showing signs of whiplash.
The Golden Hour was applied and this remained a non-lost time injury.

Possession irregularity – Item 113
A possession irregularity occurred when an Engineering Supervisor allowed a COLAS rail grinder to exit a Worksite and proceed toward the detonator protection without the PICOP’s authority.
During the movement, the grinder was also authorised across level crossings by an Attendant, without the them first being advised and authorised to do so by the PICOP.
COLAS and the Inner Delivery unit are investigating this incident further with the help of the Operations Delivery team.

Lighting Rig knocked over by RRV
A Works Delivery Manager reported that an RRV had knocked over a lighting tripod that then fell on a live conductor rail floater within a possession.
The strike caused a flashover and scorch damage.
A concern was raised by the Manger, who believed that the conductor rail concerned should have been isolated as staff had been stepping over it carrying tools. Fuel was also stored nearby alongside the rail, he clearly states this has never been like that before.
The ODM and ES confirmed that as far as the live floater was concerned, it was not an irregularity as some floaters where points are concerned are to be considered to be live unless tested to prove otherwise.
The RRV accident is subject to further investigation.

Works delivery staff issues
A Works Delivery night supervisor reported that a member of staff was called into his office following a report that he had taken a prohibited substance at the weekend.
When For Cause screening was mentioned, the individual refused to be tested and left work.
This incident is subject to further investigation.

Bethell’s Level Crossing
Staff Accident
An Emergency Intervention Unit Mobile Operations Manager attending to an incident received an electric shock from a fence which affected his back as he jumped backwards.
No first aid or medical attention was required.
The Golden Hour was applied and there was no lost time.

Wrong Route – Platform 7
A 12 carriage empty South West Trains service (5F93), was routed into platform 7 and accepted by Driver in error.
Platform 7 cannot accommodate 12 car trains and a shunt was arranged to re-platform the service.
This incident is subject to further investigation by the Network Rail operations team and SWT.

Crampmoor Level Crossing
Staff Accident
Whilst looking down to mark up for a disk cutter to change a rail, a ballast shoulder gave way.
The Injured Party slipped down with the ballast and hit his head on a clamp sitting on the rail causing cut to his forehead.
First aid was given on site, but the cut required 2 stiches at hospital.
The Golden Hour was applied and there was no lost time.

Weybridge / Walton on Thames
Staff Accident
A member of PWay staff was putting out an Emergency Speed Restriction (ESR).
As he went to pick up an Automatic Warning System (AWS) magnet, the magnet was drawn to the running rail trapping his left hand causing a crush injury and bruising.
The Injured Party was still able to continue working.
The Golden Hour was applied and there was no lost time.

Two members of Salisbury PWay believed they were stung by wasps.
They stopped work and left the area.
One had been stung on the bridge of nose and the other on the right armpit.

Accident Report
A member of Havant S&T was tucking signal cabling into a troughing route using a Lid Lifter when a shard of fibre glass came off of the shaft, passed through the glove and impaled into the skin area between the left thumb and finger.
First aid was given on site and the inch long shard was removed by the Team Leader.
The Golden Hour was applied and no time was lost.

Alleged Staff near miss
An alleged near miss occurred on a bridge structure with a member of Basingstoke S&T whilst investigating a fault on BE183 signal.
The Lookout had given a warning, but the COSS believed that he was in a position of safety within 7 seconds of the train passing.
Chain of care was carried out on all staff and this incident is subject to further investigation.

Staff Accident
An E&P Manager reported a minor RTA whilst driving out of Ludlow Road onto Guildford Park Road into traffic when he did not notice an oncoming vehicle.
No damage or injuries were sustained when his front tyre rubbed against the other vehicles near side bumper.
The Golden Hour was applied mainly for chain of care, as no injuries were sustained.

Possession irregularity – Item 113
A COLAS rail grinder 6Z08 was authorised out of a Worksite having completed its work and was making its way to detonator protection to exit the T3 possession, but the PICOP had not been asked permission for this movement to take place.
In addition to this, Crossing Attendant that had been booked on to manage the barriers between Bracknell and Wokingham also allowed the train to continue by carrying out his duties without the PICOP having advised him to do so.
This incident is subject to further investigation.

Lighting Rig knocked over by RRV
A Works Delivery Manager reported that an RRV within the T3 possession had knocked over a lighting tripod which had then fallen onto a live conductor rail floater.

The Operations Delivery Manager and Engineering Supervisor confirmed that as far as the live floater was concerned, an irregularity had not occurred as some floaters where points are concerned are to be considered to be live unless tested locally.
The issue was referred back to the Planning team to establish is any alterations could be made to mitigate against this.
The incident involving the tripod is under further investigation.

All Wessex Line Manager from Works Delivery and Maintenance who have employees identified as requiring  the Tier 3 HAVS Surveillance programme will receive an automated email from OH Assist.  As Line Manager this is only to make you aware that your employee/s have been referred.  You have to take no further action at this time.  If however, the information is incorrect (i.e. the employee doesn’t work for you any longer; has left Network rail or haven’t used vibrating tools for more than 2 years) then please contact grazia.elsehimy@networkrail.co.uk as well as NWRhealthsurveillance@ohassist.com to inform them of the error.

Road Traffic Accident
Two members of Bournemouth S&T experienced a Road Traffic Accident whilst going round a roundabout when a car pulled out from the left-hand side.
The car struck the rear of their van, but there were no injuries.
The Golden Hour was applied.

Staff Accident (assault)
A member of Network Rail station staff confronted a well-known beggar in Burger King about begging on the station and asked him to leave.
The beggar raised his fists up to them and was restrained to the ground after the staff member felt threatened.
A SWT Driver saw the incident and assisted the member of station staff until BTP arrived.
The Golden Hour was applied.

Byfleet and New Haw
Line side fire
4Y19 reported a line side fire on the Down cess near signal WK549.
Upon the MOM arriving, the fire had spread and covered 50 yards of the embankment.
By 17:14 the fire was out after the fire brigade attended.
The cause was felt to be connected to a tin can or a piece of metal foil that had come into contact with the conductor rail.
Please be aware that in dry conditions it can assist the acceleration of a fire, so be mindful the combustible materials that are around us.
A fire can start from a spark from a train pick up shoe or a discarded cigarette, so please look for the risks that are posed to us and report or remove them.
Remember to make use of the Close Call system too.

Cat A Spad 2174
A London Underground District Line service had a Cat A SPAD at W367 signal.
The Driver made no allegation against the signalling and stated that they had a loss of concentration.
This incident will be subject to an LUL investigation.

Eastleigh East Yard
Derailment
Balfour Beatty reported that one axle had derailed involving a Tamper on the Crain Pit Road over CJ points.
The Track Section Manager reported that there was no flange climb on the switch rail, but it had appeared that the front wheels had split the points and the axle that derailed ran down the back of the switch to the point of derailment.
There was some damage to the tips of the switch rail.
This incident shall be investigated by the Yard owner.

South West Trains service 2E64 reported a Cat A SPAD at HT60 signal on the up main.
The Driver made no allegation against the signalling and stated that they had a loss of concentration.
This incident is subject to a review and follow up investigation.

Pirbright Junction – Woking Junction
Possession Irregularity Item 35
Train 3Z10 reported to the Signaller that they had come to a stand at a stop board with detonators on the Up Fast Line.
Upon investigation it was established that a Vital Possession Support Staff (PSS) contractor had placed the protection incorrectly leaving the down fast unprotected.
This incident is subject to a Level 1 investigation.
The PSS had been briefed prior to applying the protection, but early indications show that there was a possibility that they had become disorientated with the layout once on site.

Rough Ride / Rail Buckle
South West Trains service 1W65 reported a rough ride / kink in the rail at 573 points on the down line country side of the station which resulted in the line being blocked.
Upon the PWay attending a known location for rough rides, they noted a slight dip and authorised normal working. This was not the area where the defect was.
Once opened at line speed, 1W25 travelling at 65mph reported another jolt on the left hand rail of the down main on or near E577 points, a further train was cautioned and reported that the defect was very bad. The line was blocked until it was rectified overnight.
Early indications show that communications may have become confused and assumptions were made which took the PWay to the wrong location.
This incident is subject to a review and further investigation.

6N01 Load Moved
6N01, the 1200  Vauxhall – Eastleigh East Yard reported that they had felt the load move.
The train had been within a Wessex Capacity possession and the load consisted of old track still fastened to their sleepers.
The load had been checked before it had left site by a Load Examiner.
The train stopped by BE401 signal between Hook and Basingstoke and the down lines were blocked due to the load now being out of gauge.
Upon a further examination by the Driver and Load Examiner (who had also checked the train earlier) it was identified that a strap was missing allowing the load to shift.
The down lines were affected for 3 hours until the train was safely recessed and dealt with at Basingstoke.
This incident is subject to further investigation.
