Staff accident
When attending to a swan on the line, the Feltham MOM sustained an injury at the top of his leg, underneath the buttock.
Golden Hour applied.
There was no lost time.
03/01/2017 Eastleigh
Staff Accident
A member of Works Delivery staff was about to treat an icy patch within the DU compound when he slipped and broke his upper right arm humerus bone.
Golden Hour was applied.

Staff Accident
A member of Salisbury S&T slipped when attending to a signal post telephone failure at E468 signal.
He put his hand out to stop his fall, but had chipped a bone in his hand as a result.
Golden Hour was applied.
This was a Lost Time injury.
04/01/2017 Waterloo
2P62 Platforming irregularity
A 5 carriage train was signalled on top off an 8 carriage train leaving a coach not being accommodated.
No doors were released and a shunt to rectify the situation was arranged.
05/01/2017 Chertsey Level Crossing
Crossing misuse irregularity
The Signaller working Feltham Panel 4 lowered the barriers and pressed crossing clear with a person within the crossing. F301 signal also cleared to a proceed aspect.
The Signaller realised the error, but saw the person leave the crossing by climbing over the barrier.

Possession Irregularity – Item 64
An Engineering Supervisor of Herne Hill PWay arrived at his worksite and found some BCM contractors already working there.
The Contractors had not signed in with this ES, but had signed in with another who was not connected to this worksite.
The COSS had used a Live Line Tester before work took place.
On further investigation, three other earlier COSS’s had made the same error by contacting an incorrect ES but had been directed to the right person.
This incident is subject to further investigation.
22/12/2016 Millbrook
CAT A SPAD 6V41 E763
6V41 reported passing E763 signal at danger on the Down fast line after cross reading E765 signal.
The Driver realised his error and applied the emergency break, but the train passed the signal by just over a loco length.
25/12/2016 Queenstown Road
Item 3 from 0100 Sat 24th December – 0600 Tuesday 27th December
The controller overseeing the Kirow 810 crane was attempting to line up 2 long bearers with a switch panel.
During the operation the front 2 wheels of the first bogie went over the end of the rail.
The crane was able to reverse and there was no infrastructure damage.
This incident is currently under investigation by IP.

Points Run Through 745A
745A points by platform 1 were ran through an LUL engineering train after an ES error in not setting up the route correctly with the Signaller.
This incident is subject to further investigation by LUL.
30/12/2016 Wimbledon Park (Depot exit)
Operating incident (Item 60 TFL)
Raynes park ECRO advised that a tripping occurred on section R173.
Engineering Work Manager to advise that a Track Operative working for London Underground was conducting track maintenance was drilling railway sleepers on the Westbound that was under possession. The Operative had stood the drill up which then fell over bridging the gap between the Westbound running rail and a conductor rail leading to the sidings which was not under possession.

Staff Accident
A member of S&T tripped on a piece of rail in the four foot while leaving a line blockage and injured his knee. First aid was given on site.

14/12/2016 Fleet
Staff Accident
A Contractor was walking to a site of work at Fleet Substation which took them over a floor of a removed building.
The floor was covered in algae causing the individual to slip, hurt his right knee and jar his back.
First aid was not required and the individual did not anticipate any time off work as a result.

Staff Accident
A Signaller tripped over a root in the compound car park of the signalling centre grazing her right hand and chin.
First aid administered.

Staff Accident
A Contract Welder was setting up a weld and when placing wedges into the holder, one of them flicked up and struck him in the face on the right side causing a cut and a lump.
No first aid or hospital visit was required, but would consider it if he needed to.

30/11/2016 Eastleigh Depot
Staff Accident
A member of Eastleigh Signal Works staff sustained a strained back whilst attempting to lift some plant equipment from the stores. (Golden Hour Applied).
16:43 – 6hr update: Injured party no longer in any pain and will hopefully be returning to work in the morning depending on if the pain has returned or not. He had not been to hospital or the doctors but had taken some pain killers.

Staff Accident
Member of Eastleigh WSC-D staff reported in sick after suffering with back pain whilst standing up on the previous shift (30/11/16).
The Injured Party had been working with back pain that had progressed through the shift signed off early due to the pain. This was not reported as an accident, but follow up calls (applying Golden Hour) were carried out once informed.
03/12/2016 Parkstone
Emergency Special Working irregularity
Bournemouth Signaller made an urgent call to 1Z08 after the train had passed BC178 signal at danger by approximately half a mile.
The Driver advised Bournemouth that the Basingstoke Dorset Coast Signaller had given them authority to pass BC178 at danger as part of the Emergency Special Working arrangements which had been implemented for a significant axle counter failure.
The Basingstoke Signaller should not have done this and the Driver should not have accepted the authority from the incorrect Signaller.

6G13 Hit detonator protection Item 6 Won 34
6G13 reported hitting a detonator after being authorised to enter WON 34 Item 6.
The weather conditions were very poor and the driver had turned his main light off to prevent the trackmen from being blinded.
The train had meant to wait for Possession Support Staff to give the Driver further instructions, but early indications show that the Driver may have misjudged where the detonators were positioned.
The Possession Support Staff were in a Position Of Safety and over 20 metres away when the detonator exploded.
This incident is subject to further investigation.

Staff Accident
A member of Wimbledon Works Delivery staff reported that they thought that they had received an electric shock from a track circuit whilst assembling the stressing kit which caused pins and needles and numbness.
The following day after a hospital visit however; it was diagnosed as a trapped/pinched nerve causing the sensations.
Golden Hour was applied and there was no lost time.

Train hit termination board causing two Staff accidents
Two members of Babcock staff were carrying out a survey on the track when they were struck by debris after a train struck a speed restriction sign in the 6 foot.
The first member of staff sustained a strike in the stomach and the other in the back of the neck.
No injury was sustained and no time off work was expected as a result of this accident.

Possession Irregularity Item 160
The ODM reported that a Trainee PICOP had given up item 160 with Guildford ASC without confirming that the protection had been lifted.
Within 60 seconds the Mentor PICOP noticed the error and the Signaller at Guildford was contacted again to protect the line until protection removal confirmed.Possession Staff had not been instructed to go lineside when the incident occurred.

Staff Accident
A member of Works Delivery staff was driving a van to an access point when he swerved to avoid a fallen tree.
He then collided with a metal post and twisted his ankle whilst he was applying the brake.He saw his doctor and was advised that he had pulled a muscle and should rest.
Golden Hour applied and this incident has since become a Lost Time Injury.

The PICOP for Item 59 reported that staff lifted work site limit boards before the PICOP had spoken with the tamper driver. The tamper had not moved.

Staff Accident
A Technical Assistant advised that they had an accident that occurred on 14/11/16.Whilst he was driving on the A320 Guildford Road, a car pulled out of a junction to his right and clipped the back of his van.
There were no injuries.

2N44 reported passing F336 signal at danger by 4 coaches after being unable to stop due to slippery rail head conditions.This incident is subject to further investigation.

2E57 passed E811 signal by 4 coaches after being unable to stop due to slippery rail head conditions.This incident is subject to further investigation.
18/11/2016 Effingham Jct
3S84 Wrong route offered – SPAR Guildford signaller advised that he had incorrectly set the route for 3S84 into Effingham sidings vice the down Cobham line. The Signaller contacted the driver and replaced the signal (GD723 calling on signal) but the train had not come to a stand and passed the signal by approximately 2 yards.

Staff Accident A member of Works Delivery Eastleigh staff strained his back whilst lifting a 25kg bag of ballast out of a tonne bag.
6 hours after the event he was still receiving shooting pain when sitting down and trying to stand up.
Over the counter pain killers were used and no medical attention was required.Golden Hour applied.
