Points Run Through 1524C Pts / Possession Irregularity 2F33 reported 2 changes of aspect on W25 Signal. (Green, Red, Green, Red). It was established that there had been a loss of detection on 1524c points which had been caused by an engineering train running through them as a Points Operator moved the wrong set whilst the train was in motion. The Points Operator had been under the instruction of a site manager (not an engineering supervisor) when the incident occurred. The Points Operator was meant to move 1517 points, but due to 1524 points not being labelled he had assumed it was the right set. When he started to move them, he realised the error but it was too late. The Engineering Supervisor was not aware of the instruction having been given, nor that a Points Operator was on site.Several concerns were raised about processes within this possession having not been followed, which will be subject to further investigation |
