London Waterloo International
6G13 Hit detonator protection Item 6 Won 34
6G13 reported hitting a detonator after being authorised to enter WON 34 Item 6.
The weather conditions were very poor and the driver had turned his main light off to prevent the trackmen from being blinded.
The train had meant to wait for Possession Support Staff to give the Driver further instructions, but early indications show that the Driver may have misjudged where the detonators were positioned.
The Possession Support Staff were in a Position Of Safety and over 20 metres away when the detonator exploded.
This incident is subject to further investigation.

Staff Accident
A member of Wimbledon Works Delivery staff reported that they thought that they had received an electric shock from a track circuit whilst assembling the stressing kit which caused pins and needles and numbness.
The following day after a hospital visit however; it was diagnosed as a trapped/pinched nerve causing the sensations.
Golden Hour was applied and there was no lost time.

Wallers Ash Tunnel
Train hit termination board causing two Staff accidents
Two members of Babcock staff were carrying out a survey on the track when they were struck by debris after a train struck a speed restriction sign in the 6 foot.
The first member of staff sustained a strike in the stomach and the other in the back of the neck.
No injury was sustained and no time off work was expected as a result of this accident.
Golden Hour applied.

Shalford Junction – Gomshall
Possession Irregularity Item 160
The ODM reported that a Trainee PICOP had given up item 160 with Guildford ASC without confirming that the protection had been lifted.
Within 60 seconds the Mentor PICOP noticed the error and the Signaller at Guildford was contacted again to protect the line until protection removal confirmed.Possession Staff had not been instructed to go lineside when the incident occurred.

Staff Accident
A Contract Welder was setting up a weld and when placing wedges, one of them flicked up and struck him in the right hand side of his face causing a cut and a lump.
No first aid or hospital visit was required, but would consider it if he needed to.

Staff Accident
A member of Works Delivery staff was driving a van to an access point when he swerved to avoid a fallen tree.
He then collided with a metal post and twisted his ankle whilst he was applying the brake.He saw his doctor and was advised that he had pulled a muscle and should rest.
Golden Hour applied and this incident has since become a Lost Time Injury.

Possession Irregularity – Item 59
The PICOP for Item 59 reported that staff lifted work site limit boards before the PICOP had spoken with the tamper driver. The tamper had not moved.

A Technical Assistant advised that they had an accident that occurred on 14/11/16.Whilst he was driving on the A320 Guildford Road, a car pulled out of a junction to his right and clipped the back of his van.
There were no injuries.

3S84 Wrong route offered – SPAR
Guildford signaller advised that he had incorrectly set the route for 3S84 into Effingham sidings vice the down Cobham line. The Signaller contacted the driver and replaced the signal (GD723 calling on signal) but the train had not come to a stand and passed the signal by approximately 2 yards.

Staff Accident
A member of Works Delivery Eastleigh staff strained his back whilst lifting a 25kg bag of ballast out of a tonne bag.
6 hours after the event he was still receiving shooting pain when sitting down and trying to stand up.
Over the counter pain killers were used and no medical attention was required.Golden Hour applied.

Sunningdale Level Crossing
Staff Accident
A PWay Team Leader was stuck on the knee with a rhino horn attachment on an RRV whilst undertaking his duties as a machine controller.
The rhino horn that was being used was quarantined and an alternative arranged following the incident.
14:04 – The Team Leader attended hospital, but was subsequently discharged.
There were no broken bones, but they had badly bruised their kneecap and would be off work for approximately 5 days.
Golden Hour applied.

Wimbledon ASC
Fire
Thick black smoke poured from the Battery room onto the operating floor resulting in the box having to be evacuated for up to 90 minutes.
Signallers managed the situation by bringing trains to a stand in a controlled manor and advising fringe locations before they left.
The fault was identified to be a single battery cell which was breaking down, smoking and melting as it did so.
This is subject to further investigation about the incident and follow up review of the evacuation and post management processes.

Staff Accident
A contract cleaner for Interserve received an electric shock from the light switch by the sink within the signalling centre locker room as the metal plate had become live.
The cleaner was able to continue with her duties and the Shift Manager covered the switch with insulation tape, reported it to Close Call and the Property Action Help Line.

Staff Accident
Member of staff working for B and M for Investment Projects was working on a culvert on the 07/11/2016 at approximately 16:00hrs.
The Injured Party was tying up a brick clamp and knocked his left hand when it slipped.
There was no bruising or abrasion at the time, but overnight the injury had swollen and his manager advised him to go to hospital.
Upon hospital examination, the Injured Party had sustained a bruised tendon. There were no broken bones.
He was advised to take ibuprofen and was now back to work with no ill effects.

Staff Accident
A PWay Apprentice sustained arc eye whilst completing measured shovelling packing on the Down Brighton.
The Apprentice had a loaded shovel of ballast which was distributed and what may have been a metal object within the load, caused a flash over.
The Apprentices legs went to jelly and he had temporary blindness for around 10 minutes.
An emergency call was made to the Signaller and the Apprentice was taken to hospital for tests.
Post these tests, his eyes were seen to be fine and no lost time was anticipated.

West Byfleet / Byfleet and New Haw
Staff Accident
A member of PWay staff was carrying a scoping bag and a track spanner whilst walking along the troughing route on the Down side.As he did so, he slipped on loose ballast that was on top of it and was unable to break his fall and fell onto his left hand side injuring his rib cage.The Injured Party went back to the depot and carried out some office duties before going home, but as a precaution he went to hospital.
The hospital advised that he had internal bruising and no cracked ribs.
Golden Hour applied.
This is a lost time injury.

Staff Accident
A member of Feltham PWay reported that he had an accident the on 09/11/2016 at 12:30 whilst in the mess room after getting out of his seat and bumping into a colleague who had heated some food in the microwave.
The food spilt onto his right arm and he ran it under cold water.
No hospital or GP visit was required at the time however; if the symptoms did get worse he was going to visit his doctor.
Golden Hour applied.This was not a lost time injury.

Operational Irregularity
Basingstoke ASC reported that he granted a Line Blockage with a train in section.
The Line Block was for Item 80 on the Down Slow from beyond BE127 points to approach BE2512 points and 1B01 was in the section when it was granted.
The signaller advised that he thought it was shorter than it was.Note:
A near miss was not declared.
The incident is subject to further investigation, but poor commuincation and not coming to a clear understanding appears to have been a contributing factor.

A member of Lost Property Staff was taking a suitcase off of a middle shelf and when the weight caused the case to drop, it jarred his back.
Following the incident, he took pain killers.

Operational Incident – Signalling
Feltham SSM has advised that the signaller was talking past F422 signal to get 5N95 out of the depot due to an ongoing failure.
The Signaller had set up the route via the route card and had it checked about 10 minutes prior to the trains departure.
Upon the train being presented, the Signaller gave authority for the train to proceed, but overlooked lowering the barriers at the CCTV level crossing.
The signaller managed to contact the Driver before any movement took place.
For Cause screening arranged.
Level 1 completed and sent to the RAAI.
Poole (High Street) Level Crossing

Person Struck by Crossing Boom
Basingstoke DC signaller reported that when lowering the barriers, the down side boom struck a lady on the shoulder who had looked to be clear with her standing next to a wall and leaning her arm on it.
13:32 – The MOM checked the crossing with no defects found.
Strawberry Hill

Staff Accident
At 0140 hrs  during hours of darkness in dull and damp conditions whilst walking back to Strawberry Hill Station after completing the activity of placing short circuit Strap, the IP stepped over a running rail right foot first to walk in the 4ft,  the ballast on which he had placed his foot was uneven and moved under his foot unexpectedly. This caused his ankle to give way and twist sustaining a painful injury.  In this case this may have been exacerbated by a previous injury which has left him with a metal plate in this area anyway.  Hospital and X-ray to confirm there were not broken bones He was wearing a head torch and standard boots which are in good condition.
Immediate cause:   Environmental -Uneven surface, and momentarily lack of concentration.

Operating Incident (1A15)
When 1A15 arrived at the station the doors were released and after passengers left, the train was put into maintenance mode and doors were released thinking the doors not accommodated would not be released.
10:53 – CCU Download confirmed that the leading unit into Alton 450076 was put into maintenance mode. A detailed report was requested by who operated the maintenance mode button which is in a cupboard and should not be used when in passenger service. It is only meant to be used when in a depot.
Surbiton / Berrylands

Near Miss – 2J26
2J26 reported unsafe working practices with P-Way staff who were stood between Up Fast and Slow lines and was close to them as the train went by.
As 2J26 departed Surbiton station on the Up Slow line a lookout stood between the Up fast and Up slow.
1W96 on the Up Fast and sounded their horn and the Lookout step back towards the Up Slow.
At this point, the Lookout looked to be only a foot away from the up slow as 2J26 passed.
10:54 – The PWay COSS contacted WICC and advised them that they believe the advanced Lookout may have strayed a bit further than they should have and work suspended.
11:27 – MOM on site. 3 staff involved. 1x NR COSS, 1x Contract Lookout (Gannymeads) and 1 Contract Advanced Lookout (Gannymeads).
11:36 – MOM confirmed that the SSOWPs was all in order, Sentinel cards were in date and valid and All parties held the appropriate competences which were in date.
12:03 – FFCCTV images of 1W96 sent to N/Rail. 2J26 awaited.
COSS and Lookout competencies have been revoked at present pending the outcome of the investigation.
Brockenhurst

Staff Accident
At 1015 hrs during daylight in reasonably dry conditions working in red zone under lookout protection at Brockenhurst, the IP an Ultrasonics Operator using a Sperry Stick was called out to a position of safety.  He picked up his equipmentand moved toward the place of safety carrying this equipment andin doing so was required to step over several obstacles; a running rail into the 6ft, and a second runningrail with conductor rail.  When he placed his foot into the 4ft of the adjacent line the ballast in the track bed was lower than normal and adjacent to infrastructure equipment (See photo).  He lost his balance as he placed his right foot down adjacent to a TPWS ariel grid (Spider), his foot then became trapped under the grid and he fell trapping his foot and twisting his ankle in the process, sustaining the injury.  He was wearing standard boots in reasonable condition.
Immediate cause: Environmental – uneven surface,  and obscured vision.
Potential Underlying cause: limited vision whilst stepping across infrastructure obstacles, whilst carrying equipment.
Axminster / Honiton
2/11/16
Staff Accident
At 1135 hrs during daylight hours in sunny conditions, whilst carrying 20kg bag of cement mix over his shoulder on site at Honiton Tunnel, the IP a contractor working for Ganymede stepped over a troughing route onto the cess area right foot first, the cess area was grassy, softer and lower in height than he had anticipated. This resulted in him loosing his balance and his right ankle twisted outwards causing injury.  Attended hospital and x-ray to confirm no broken bones. He was wearing standard footwear with ankle support.
Immediate cause: Evironmental – uneven surface, and unbalanced manual handling technique.
Underlying cause….several possibilities (including training) not confirmed at this stage.

NVR – Network Rail vehicle involved in RTA at Eastleigh resulting in injury to the driver.
Wessex Outer DU – A MOS cut his right arm on the edge of the door at Mount Pleasant LC.
WX Inner DU – Technician trapped his hand whilst moving scrap rail at Weybridge resulting in a fractured index finger – NSC 27757.
Route Services – A driver got his/her foot caught under a rail at Clapham Yard. NSC 27797.
Southern IP – A contractor was knocked down outside the project delivery stores by a delivery vehicle Transit Van at Waterloo. (27781).
A contractor vehicle involved in RTA at Ascot, resulting in injury to both passenger and driver.
Wessex Inner DU – A PWAY MOS was putting in a check rail which slipped and caught his left hand middle finger, causing swelling at New Malden.
IP Southern – Jack handle jumped up when contractor was using it which struck him in the jaw causing bruising – NSC 27843
WX Outer DU – Apprentice was taking measurements for sideways movement with a steel rule when he caused a small flashover at Holme
2V43 (1537 Waterloo – Waterloo at W96 signal, Queenstown Rd– (Green to Red) – due to signaller error.
1W21 (1035 Waterloo – Weymouth) reported an irregular sequence at BC165 signal, Bournemouth – Due to signaller error
Wessex Inner DU – PICOP gave permission for 6N60 to leave the worksite at North Camp without checking the route was correctly set. (27879)
1L29 (1120 Waterloo – Exeter St Davids) at BE475 signal, Grateley – (Green to Red) – S &T error.
WX Outer DU – Apprentice was taking measurements for sideways movement with a steel rule when he caused a small flashover at Holme
OCC/COA – 2V43 (1537 Waterloo – Waterloo at W96 signal, Queenstown Rd– (Green to Red) – due to signaller error.

Contractor Accident A contractor for Works Delivery cut their thumb. It was cleaned and bandaged and required no hospital treatment. There was no absence from work. Golden Hour applied.

Wimbledon EMU Depot
Strap incorrectly placed Wimbledon Shunter reported Track Circuit ‘YJ’ showing occupied in the area of W707 points. Upon investigation it was established that the strapping position had been marked up incorrectly.

This is subject for further investigation.

Eastleigh / Portchester
T3 Item 84 Possession Irregularity Eastleigh ECRO reported a tripping in the Swanwick area to the PICOP. Upon investigation it was established that Straps were being placed for possession 84.

There were no injuries, but one end of the strap had vaporised.

This is subject for further investigation by Investment Projects.

Eastleigh South Junction / Fareham
T3 Item 84 Possession Irregularity The Havant signaller reported that 6J07 (a engineering train) had passed HT308 on a proceed aspect and then came across detonators and a Possession Limit Board.

The train had been travelling at 40mph. Upon investigation it was established that the PICOP had only communicated with the Eastleigh Signaller when the T3 was being arranged and had not realised that HT308 was a Havant signal.

This incident is subject to further investigation.

Staff Accident Eastleigh Works Delivery S&T was opening an access gate when the slide bolt became stuck. He then forced it resulting in the tip of the left index finger becoming trapped and cut. First aid was given on site.

Project Manager from Works Delivery reported that when item 126 was taken last night, a rack out was required within the Kingston Sub Station as cable and conductor rails were being worked on close to the Sub Station.
A team had attended at 07:00 and disconnected cables and started work on conductor rail, but this work should not have started until the Supervisor had sight of a certificate of isolation to show that the equipment had been isolated.This incident is subject to further investigation.

The Wimbledon Panel 5 Signaller routed a train toward a line blockage whilst the S&T were undertaking works on Vine Road within a line blockage. Limits:
W552 came off to a proceed aspect for 2H90, but the Signaller realised their error and placed the signal back to danger.This incident is subject to further investigation.

An Eastleigh S&T Team Leader was working on a set of points and was using a railway bar to push them over.
This caused a jar his elbow resulting in a mild ache.
No first aid given or hospital attendance was required.
