CAT A SPAD
2V59 19:37 London Waterloo – London Waterloo had a Cat A SPAD on W79 signal. The signal had been at danger due to 2C99 in the section ahead. The Driver did not dispute the SPAD and minimal testing took place post event.

2B80 – Operational Incident
2B80 21:55 Southampton Central to Waterloo service (12-450) was diagrammed incorrectly into Platform 7 where it could not be fully accommodated. The Signaller followed the booked working and the Driver accepted the route in error. All passenger doors were accommodated and the passengers were dealt with safely.

Mount Pleasant Level Crossing
Person trapped in barriers
Eastleigh Area Signalling Centre reported that a person had been trapped within the barriers of Mount Pleasant CCTV level crossing and the Signaller had pressed “Crossing Clear”.
The Signaller then checked the picture again (with colleagues) and felt that something did not look right. It was therefore arranged for 2B36 to be cautioned. The Driver reported that there was a woman dressed in black with a pushchair, right up against the pedestal on the down side. Once 2B36 had passed, the female with the buggy left the crossing as the barriers raised.
The CCTV picture within Eastleigh ASC is compromised by being black and white and of poor quality.
This incident was subject to further investigation.

Possession irregularity Item 32
The Operations Delivery Manager Inner reported an irregularity had taken place within T3 Engineering work possession item 32 where Possession Support Staff had opened up a wrong hook switch at Nine Elms. (HS1094 vice HS1095).
There were no injuries sustained, the area was made safe and the investigation process then followed.

Worksite irregularity – Item 39
The PICOP of T3 Engineering work possession item 39 became aware of an irregularity involving a crane which had ran over an S&T cable on the Up main at 80m 46ch. The cable had been due for replacement prior to the error made, so no issues were experienced.

Ludgershall
Staff Accident
Whilst moving sleepers on the Ludgershall branch ready for installation, a Gannymede Contractor got some creosote on their glove and accidently wiped it their left arm and neck resulting in mild burns and irritation to the skin. The affected area was washed and cream was applied. The Contractor then visited Winchester hospital as a precaution where they treated him for dermatitis inflammation.
The Golden Hour was applied and there was no lost time.

Possession Irregularity Item 133
1B01 01:05 London Waterloo – Basingstoke reported running over detonators and a Possession limit board at Ashford on the down line and observed possession staff jumping out of the way. The PICOP of T3 Engineering work possession item 133 had not given any permission for the McGinley Possession Support Staff (PSS) to lay out protection, as signal protection to do so from the Signaller had not been granted.
The PSS had also made a previous error within the same possession when they keying back F194 signal vice F195 signal to danger (red) to protect the T3 on the up line.
There were no injuries sustained, the area was made safe and the investigation process then followed.

Cat A SPAD 6O35
DB Cargo 6O35 06:10 Westbury Down T.C. – Fareham A.R.C. Sidings had a Cat A SPAD at E32 signal protecting Eastleigh Junction. The Driver had experienced a micro sleep post acknowledging the automatic warning system (AWS) prior to the signal. Minimal post SPAD checks were arranged. The signal had no SPAD history.

Staff Accident
A member of staff from Eastleigh E&P was carrying out electrical re-earthing works at Worting Junction substation and whilst crimping up the cables, their finger was caught in the handles crushing and breaking a finger nail.
First aid was given and the Golden Hour applied.
There was no lost time.

Road Traffic Accident
An Inner Maintenance PWay Section Manager was involved in a road traffic accident on the way to work in a company van on the A335 in Eastleigh when another van which coming from the other direction veered over the central white line.
The Section Manager swerved to the left to avoid a head on collision, but both vans wing mirrors made contact.
The Golden Hour was applied for chain of care.

Staff Accident
A Mobile Operations Manager was stepping over a running rail to get to the cess, but had not seen an exposed drainage hole and fell into it. They sustained an injury to their left arm and jarred their right hip.
The Golden Hour was applied and although bruised, the MOM was able to resume their duty.

Hounslow
Staff Accident
Upon driving to work in a Network Rail leased vehicle on the A312 near Cranford (Hounslow), an Inner Maintenance Section Manager misjudged the speed of a car in front and collided with it as it was slowing down. There was minor damage to both vehicles, but no injuries.
The Golden Hour was applied for chain of care.

Staff Accident
A member of Outer PWay was using an iron man and whilst releasing the ratchet, it had spun around and caught the lower part of the right arm.
The Golden Hour was applied and there was no lost time.

Feltham
Crane collapse in the Lidl car park
A crane collapsed within the Lidl car park next to Feltham Station connected to Network Rail upgrade works being carried out by Contract Staff working on bridge 3/50. There were no reported injuries or infrastructure damage.
All the paperwork was said to be correct (ie lifting plans etc).
The incident was reported to HSE and is currently under further investigation.

Staff Accident
An Inner PWay Operative was walking on ballast which then gave way under their right foot causing them to fall to the ground bruising, grazing and cutting their left leg.
First aid was administered and the Golden Hour was applied.
There was no lost time.

Yeovil Junction
Cat A SPAD
5L51 had a Cat A SPAD at SE4733 Stop Board, passing it by 91 yards. The Driver advised that the Signaller had advised them to pull the train all the way down the sidings as an attachment was coming in on top. The Signaller disputed this, as the instruction had not included passing the Stop Board where further authorisation was required.
This incident is subject to further investigation.

Item 5 Line Blockage Irregularity
Waterloo Platform 20 was under line blockage as part of T3 possession item 5.
The Line Blockage had been taken correctly, but upon giving it up the incorrect authority number was quoted on 3 occasions. Upon reviewing the paperwork, it appeared that the COSS had written the correct authority number on the top of their form, although the 6 was not clearly defined. The COSS then wrote a 0 instead of a 6 where the authority number was meant to be recorded thus quoting the wrong number when giving up.

Staff Accident
A member of Inner Maintenance S&T was unloading light tools from a works vehicle into the S&T store when they noticed pain in their middle finger and saw a minor cut / abrasion.
The wound was cleaned, First aid and the Golden Hour was applied.
It could not be established what cut the individuals hand, but they had not been wearing gloves at the time.

Staff Accident
A member of PWay from the Outer DU received a bite/sting on the hand whilst walking in the cess as a Lookout.  It then started to burn and swell. The Golden Hour was applied the injured party was encouraged to seek medical attention.

Wimbledon
Staff Accident
Whilst unloading tools from the platform onto the track a member of Wimbledon PWay did not let go, lost their footing and fell into the 4 foot, striking his head on scrap rail. The Injured Party suffered a deep cut on their head approximately 1 ½ inches long.
First aid was administered on site and the Golden Hour applied.
The cut was glued at St Georges Hospital, but due to the swelling sustained further scans were required the following day resulting in this becoming a lost time injury.

Staff Accident
A member of Woking PWay injured their left arm whilst pulling off track keys which had fixed themselves to an AWS magnet in the back of the van. The injury began to swell and the Golden Hour was applied. This was not a lost time injury.

Winchfield
Staff Accident
Whilst standing in the up CESS in a position of safety a train went past on the up slow and something heavy came off the train striking a member of Basingstoke S&T on the ankle. The train involved had previously struck a deer, but the train part that came off the unit could not be found.
Upon returning to the depot the Injured Party placed their foot in cold water before attending hospital where they found no breaks or fractures.
The Golden Hour was applied and this became a lost time injury.

Possession Irregularity
A Signaller at Wimbledon made an error by overlooking 1B01 01:05  London Waterloo – Southampton Central in to platform 16 when arranging T3 possession item 20A affecting platforms 11-19. Upon granting signal protection to the PICOP at 00:49, they then had to give it up again at 01:08 to allow the train to depart.
This incident is subject to further investigation.

Staff Accident
A member of Salisbury PWay was walking in the cess over uneven ground when their right ankle gave way. They stumbled forward and fell onto the ballast.
The Golden Hour was applied and no lost time was anticipated.

Staff Accident (RTA)
An Outer DU PWay Operative was at a roundabout in their van with a car beside them in the next lane. When they then pulled away, the rear of the van clipped the car at low speed.
Apart from minor damage sustained, there were no injuries.
The Golden Hour was applied for chain of care.

Cat A SPAD 0W29
Freightliner light locomotive 0W29 passed E221 signal by half a Loco after assuming that the route would be set into the yard. There was no allegation against the signal.
This incident is subject to further investigation.

Alton / Basingstoke
Staff Accident (RTA)
A member of Telecoms Outer was stationary on the A339 and whilst waiting for a car in front make a right hand turn on to Whitedown Lane, another vehicle hit them from behind.
It was unclear what speed they had been travelling, but substantial damage was sustained to the 3rd party vehicle.
The Golden Hour was applied and no medical attention was required.

Staff Accident
A member of Havant PWay was cutting a rail with a disc cutter when the blade pinched in the rail pulling the disc cutter forward. The motion yanked the Injured Party’s hand pulling on their right hand thumb causing a strain.
The Golden Hour was applied, pain killers were taken and there was no lost time.

Staff Accident
A member of the Inner PWay sustained a sharp pain in their back and numbness in their legs which resulted in them falling backwards whilst using an impact wrench.
The Golden Hour was applied and the injured party attended A&E.
The Injured Party was prescribed painkillers and given crutches having seen a physio.
This was a lost time injury.

Staff Accident (RTA)
The Emergency Intervention Unit (EIU) Mobile Operations Manager (MOM) and British Transport Police (BTP) were stationary within their vehicle at a set of traffic lights.
A car then attempted to overtake the EIU vehicle, but drove into the side of it at low speed causing minor damage.
There were no injuries and the Golden Hour was applied for chain of care.

Thames Ditton / Hampton Court Junction
Staff Accident
A Distant Lookout from the Inner PWay had finished carrying out their duties and was carrying a lookout kit whilst walking in the wide way. He caught his foot on the wooden edging of a walkway which caused him to fall forward striking the ground with his face sustaining a minor injury to his chin. He put out his hand to break the fall, sustaining bruising and minor skin abrasions to his right elbow and forearm, as well as suspected damage to a bone in his shoulder.
The Golden Hour was applied and the Injured Party attended hospital where neck brace was put on as a precaution and pain killers prescribed.
This accident became a lost time.

Suspected Asbestos Exposure
Two members of staff were thought to have been exposed to asbestos dust on the ground which had come from an Eastleigh Works building being demolished.
The area had not been cordoned off and there were no warning signs of advice as they drove through and walked over the area.
The Golden Hour was applied.

A Raynes Park Electrical Control Room Operator received a door alarm at Teddington Substation and identified that a member of S&T cover had entered the building without the correct competency.

Possession Irregularity
The Operations Delivery Manager reported that a T3 possession irregularity had occurred within Item 45.
The PICOP had been arranging to shorten the block and had agreed the new limits, but had overlooked instructing Possession Support Staff (PSS) to remove the protection before authorising the Signaller to re-open the lines.
The error was realised quickly before the trains began to run and no members of staff were in danger.
This incident is subject to further investigation.

Worksite Irregularity
A Track Quality Specialist working within T3 possession item 152 received a call from Global Rail staff working at 11m 48ch about an RRV trailer which had derailed.
The derailment had damaged 3 impedance bond side leads.
The trailer was quarantined ready for inspection by Readypower and the incident was subject to further investigation.

Staff Accident
A Senior Technical officer (Outer) was removing an item of shelving within a store room, when a shelf bracket fell and struck him on the head.
The Injured Party self-administered first aid and the Golden Hour was applied.
There was no lost time.

Staff Accident (non-work related)
A member of Wimbledon PWay felt his knee give way whilst walking on level troughing and was assisted onto a train to return back to the depot by other members of the team.
The Golden Hour was applied and the Injured Party attended his local physio the following day, but was unable to return to work the following day.

Allegation of Irregular Working
P-Way found a Babcock machine controller and a lorry driver (without a visitors permit) line side without a Safe System of Work in place.
Although rarely used, the lorry was also close to the rails within the Sidings and may have become a risk had a train used them.
This incident was subject to further investigation.

Worting Junction
1L19 driver reported alleged workers slow to clear
The South Western Railway Driver on 1L19 reported having to apply the emergency brake on the approach to BE431 signal having seen track workers in the 4 foot of the down fast.
The Driver observed a Lookout waving their flag, but the workers looked slow to move to a position of safety.
Chain of care was carried out and an investigation took place.
The investigation concluded that the team had been working safely and had moved into a position of safety within 10 seconds.

Staff Accident
A member of Outer DU S&T staff was undertaking wire correlation / checking within a location cabinet and received an electric shock.
The shock entered through their left hand and travelled across the chest before exiting from the right hand.
There were no visible entry/exit wounds, but the Injured Party was reported to have become dazed and shocked by the incident.
The Golden Hour was applied and the Injured Party felt fine post incident.  They were encouraged to attend hospital as a precaution.
Initial indications are that the correct protocols were being carried out and the correct PPE was being worn.
This incident was subject to further investigation.

Staff Accident
A member of the Bournemouth PWay reported that whilst setting out an Emergency Speed Restriction warning magnet, they caught a finger nail and tore it.
The Golden Hour was applied and no lost time was anticipated.

Waterloo
Cat A SPAD W59 1P59
1P59 experienced a CAT A SPAD on W59 having read across to an adjacent signal showing a proceed aspect having seen it change up from a red to a yellow aspect.
The train was bought to a stand by TPWS.
There have been eighteen previous Category A SPADs at this signal as per RSSB data (1985 – year to date) and is classified as a Multi SPAD signal.

Portcreek Junction – Farlington Junction
TSR not in place
1S91 reported that no Temporary Speed Equipment was in place for the 30mph restriction on the Up Main for TSR2017/139050 between 41m10ch to 40m75ch for the condition of track.
This was confirmed by 2S11 and an immediate caution was implemented by the Signaller until correctly boarded.
Following PWay attendance it was established that the boards were removed in error instead of being spate’d (Speed Previously Advertised Terminated Early).  Cross boards should have been installed.

Staff Accident
A member of the Woking PWay entered the railway via an access gate and walked over steel plating screwed to wooden sleepers.
With the weather being wet, the area had become very slippery and the Injured Party twisted their right ankle.
The Golden Hour was applied and no lost time was anticipated.

New Milton
Signaller Irregularity
The Brockenhurst Signaller reported themselves after overlooking to cautioning for a rail defect.
Upon cautioning Cross Country 1O24 correctly, the Signaller had not placed BH370 (an automatic signal with an emergency replacement facility) back to danger and South West Railway service 1W37 entered the section.
Upon realising their error, the Signaller then made an urgent call to the Driver and the Driver advised of the circumstances.
Chain of Care was carried out with the signaller and arrangements were made for them to be relieved.
This incident was subject to further investigation.

Staff Accident
An Apprentice from Signals Works was knocking a bolt out with a punch and a hammer, but it slipped cutting him under his left eye.
The Golden Hour was applied and First Aid was given.
The Injured Party also attended hospital where butterfly stitches were applied.
There was no lost time.

Basingstoke
2J07 (5 Car) Not Fully Accommodated
Great Western Railway service 2J07 formed of a 5 car unit was not fully accommodated and the rearmost passenger doors were off of the platform.
The error was realised by the Guard who operated the doors manually.
Upon investigation it was established that owing to engineering works on the GWR Network, services had been strengthened and the Signaller had not been aware of the restriction.
This incident was subject to further investigation.

Signaller Verbally Abused
Upon services being fully loaded with passengers due to the good weather, some passengers were not able to travel and became aggressive with the Signaller whilst they were exchanging a signalling token with a Driver.
The Signaller was verbally and physically abused and chain of care was carried out.

Staff Accident
A member of staff from Eastleigh D&P sprained their left ankle whilst walking on a troughing route to a site of work.
The Golden Hour was applied and although the ankle had to be strapped up, there was no lost time.

Milford
Allegation of Irregular Working
The Havant S&T challenged the Safe System of work applied by a MOM when a member of the Police wanted to assess the site post a suicide in the area.
The plan the MOM used was confirmed as not being correct by the Local Operations Manager and although both parties were aware of their parameters of safety, the incorrect method of working in place.
The S&T were therefore right to challenge which has helped to remind and re-brief our Lone Worker staff.
This incident was subject to further investigation.

Item 31 W1582 Points Out of correspondence / irregularity
The Wimbledon Signaller observed W1582 points go out of correspondence and was concerned, as the points were being used protect T3 possession item 31.
Upon investigation it was established that the S&T had been working on the point end outside the T3 limits, but within their own Line Blockage, but had not communicated with the Signaller first prior to commencing work.
This incident was subject to further investigation.

Woking
A member of Woking CRE staff cut his middle whilst getting up from a chair. The chair had damage to the underside of the armrest which was not visible and was later thrown away.
The Golden Hour was applied and there was no lost time.

Wilton Junction / Quidhampton Sidings ECC
South Western Railway service 1L72 reported a lineside fire at 85m 40ch – 85m 60ch on the upside by the boundary fence.
Upon investigation it was established to be a Patrolman’s hut that was on fire and took just under 2 hours to put out.

Redbridge
Freightliner 4M58 passed E971 signal at red by approximately 120 metres after the Driver was alleged to have become distracted by railway staff working in the area and communicating with the despatch staff.
The signal had been at red due to a failure that the railway staff were attending.

Wimbledon EMU Depot
A member of Possession Support Staff placed the protection in the wrong place and blocked no. 2 siding vice 1.  Upon being asked to carry it out again, it was placed incorrectly again for a second time.
Upon further investigation it was established to have been a planning error.
