Incidents since last post

There was excellent work carried out by all concerned during the wintery weather conditions.
The Teamwork demonstrated by Train Crew and response Teams amongst others to clear the line of trees, debris, snow from platforms and points was second to none.
Thanks to all of you who made it into work to keep the railway running and our offices responsibilities functioning.

28/01/2019 Bentley / Alton Line Blockage irregularity
A Team of 3 Inner Maintenance Ultrasonics staff were working between Bentley and Alton within a planned Line Blockage (GZAC 6115047). The Team had just finished their work and were heading back to an access gate when a train approached and blew the horn. The Team were able to stand clear in plenty of time. Although the Line Blockage had been between Bentley and Alton, the Signaller had thought the team were working within the Bentley loop and had ran the train in error. The Signaller was relieved of duty. This incident is subject to further investigation.
28/01/2019 Godalming Road Traffic Accident
A Senior Technician from Woking Telecoms reported a minor road traffic accident in Godalming when a South Coast Ambulance struck and broke the Drivers side wing mirror. There were no injuries and chain of care was carried out. The damage was reported to Fleet.
31/01/2019 New Malden / Shacklegate Junction Possession irregularity
DB Cargo reported that an Operator on board Autohopper 6G26 within T3 possession item 160 (New Malden / Shacklegate Junction) was not happy to continue to work due to multiple issues.
  • There were a lack of dust masks
  • The COSS speculated to have to leave site to collect a van from Kensington
  • Whilst unpinning wagons to drop stone, the loco began to move
  • The ES was within the loco.
The Operations Delivery Manager was appointed Investigating Officer and an investigation then took place.
31/01/2019 Chertsey / Egham Possession irregularity
T3 possession item 178 between Staines and Ascot was estimated to overrun by 20-25 minutes due to grinding that was required. Upon the possession being given up at 05:17hrs, it was established that F300 signal had not been keyed back to auto working due to the Hand Signaller forgetting to do so. Trains were cautioned passed the signal at danger (red) until 05:59.
01/02/2019 Salisbury Road Traffic Accident
A Welding Team Leader from Eastleigh was driving up the slope into the Salisbury Pway depot in a Network rail van and when trying to turn the corner, the vehicle went straight on in the snow and hit the fence. There were no injuries sustained and chain of care was carried out. Fleet were advised that damage was sustained to the Driver’s front wing and part of the bumper was pulled off. The fence damage was also reported.
02/02/2019 Liss SWR Cat A SPAD
The Driver of SWR empty stock 5T94 Basingstoke to Fratton (which had been diverted via Woking owing to an icing incident at Basingstoke), reported having a Cat A SPAD at PF229 signal. The Driver advised that they had been following a stopping service (1P29) on single yellows for quite some time and had become caught out expecting another yellow vice red when 1P29 stopped at Liss station. This incident is subject to further investigation.

Incidents since last post

17/01/2019 Staines Staff Accident
A Feltham S&T Senior Technician was re-terminating a cable at an awkward position within F252 signal. Whilst applying the nuts, they felt a sharp pain and noticed that their finger on their left hand was bleeding severely. It was unknown as to what broke the skin. First Aid was administered and the Golden Hour applied. There was no lost time.
17/01/2019 Eastleigh Staff Accident
A Telecoms Manager was erecting a screen within the office. The top part of the screen became loose causing the screen to fall onto their hand / wrist. The Injured Party sustained a cut and bruising. The Golden Hour was applied and there was no lost time.
18/01/2019 Surbiton Report of Unsafe Working
After receiving advice of a potential trespasser, an Inner Mobile Incident Officer (MIO) attended site and found a Vodaphone employee working on the behalf of Mitie behind the old signal box at Surbiton within a caged Vodaphone cabinet. The individual did not have a Sentinal card and was not wearing the full PPE whilst trackside. The MIO escorted them from site, contacted Mitie and Close Called the incident. The Route Workforce Health, Safety and Environment Advisor followed this incident up locally.
20/01/2019 Aldershot North Junction Possession Irregularity
A Signaller on Woking Panel 3  reported that they had granted a Line Blockage which overlapped with T3 possession (item 155). The Signaller had believed that they were able to utilise the exit signal of the T3 to protect the Line Blockage under a back to back arrangement, but had not clearly established that this was not possible. The error was realised when the Controller of Side Safety (COSS) for the Line Blockage reported seeing the T3 Possession Limit Board within their block. It was noted within this incident that the Line Blockage had also been planned this way and had not taken into account the conflicting T3 possession. This incident is subject to further investigation.
22/01/2019 Woking Staff Accident
A Woking PWay Technician was walking in the cess on the Down Slow near Woking and upon crossing into the 4ft, they caught their foot on a cable, fell, twisted their ankle and banged their shoulder. The Golden Hour was applied and this became a lost time injury.
25/01/2019 Wimbledon Staff Accident
The Local Operations Manager Response Inner was trying to remove a cable tie with a pair of scissors when the scissors they were using followed through and cut his right hand. Their hand became quite swollen and bled quite badly. A dressing was applied. The Golden Hour was carried out and post visiting hospital, a butterfly stitch was applied. There was no lost time.
27/01/2019 Guildford Staff Accident x 4 – (Road Traffic Accident)
Four members of Guildford PWay were involved in a road traffic accident on the A3 near Guildford travelling approximately 65mph. The weather conditions were wet and a car ahead of them in lane 1 was travelling very slowly between 5-10mph. The van closed very quickly due to the difference in speed. The Network Rail Driver turned into the next lane to avoid a collision but lost control and the van spun around slamming into the central reservation barrier. All on board were ok, but there was extensive damage to the van which needed to be recovered. The Golden Hour was applied and all staff involved remained ok.
27/01/2019 Havant Staff Accident
A late reported accident was received at about a Ganymede Contract member of staff who was assisting an Outer Maintenance Welding Team to remove equipment from a T3 possession. The Contractor was seen to slip over on a cable, but got up straight away and appeared not to have suffered any injuries. Subsequently it was found that the Contractor had sustained a cut and swollen knee. The Golden Hour was applied once the accident was known about and no lost time was anticipated.

Please note that the Possession Irregularity at Eastleigh was initially posted with incorrect information.  The straps were not meant to be removed, but they were placed at the incorrect location.  The post below has now been correct

09/01/2019 Wokingham Junction / Bracknell Staff Accident
A member of S&T Works Delivery staff was walking in the Down Cess to site of work between Wokingham Junction and Bracknell. Upon walking through an area that was overgrown, the member of staff fell into a section of troughing route that had a missing lid hurting their knee. The Golden Hour was applied and although the knee was sore, there was no lost time.
13/01/2019 Eastleigh South Junction Possession Irregularity
Eastleigh Works Delivery PWay reported that a possession irregularity had occurred having located a rail that they were about to remove on the Up Portsmouth between 74m 5ch and 74m 26ch, had isolation straps applied to it.
The straps have been placed at the wrong location (74m22ch vice 74m29ch as planned) at the time the worksite was set up.
This incident is subject to further investigation.
13/01/2019 Walton-on-Thames Staff Accident
A member of PWay Works Delivery Track staff was using a Setter for installing track nylons and whilst leaning against it with some force, they damaged their shoulder muscle ligament and found it difficult to lift their shoulder without quite a lot of discomfort and pain. The Golden Hour was applied and the Injured Party was able to return to work, albeit on light duties.

Incidents last week

13/12/2018 New Kew Junction Fallen tree – securing ropes snapped
Environmental Forestry working within a Line Blockage on the Kew East Spur reported that a section of tree had fallen onto the line after their securing ropes snapped. There was no reported injuries or damage to the infrastructure.
16/12/2018 Windsor and Eton Riverside TPWS Grid damaged during T3 possession item 168
Colas Rail S&T reported that the Train Protection Warning System (TPWS) grid for F286 signal within platform 1 at Windsor and Eton Riverside had been damaged whilst a rail was being lifted. Feltham S&T attended to rectify the damage.
16/12/2018 Botley Damaged Cable during T3 possession item 109
The Eastleigh Electrical Control Room Operator received a call from the Engineering Supervisor of T3 possession item 109 to report that a tamper had clipped a cable at Botley between 483A and 483B points. The cable had become exposed and a temporary repair was arranged.
24/12/2018 Longcross Category A Signal Passed at Danger – 1C22
The Driver of SWR 1C22 09:00 Reading to London Waterloo reported having a CAT A SPAD at F312 signal on the Up Main after departing Longcross. The train passed the signal by 5 coach lengths after the Driver experienced a loss of concentration. There were no allegations against the signal, that had been displaying a red aspect due to SWR 2C22 08:46 Reading to London Waterloo within the section ahead.
29/12/2018 Feltham S&T Depot Staff Accident
An Operative from Feltham S&T was walking down some steps at the Feltham S&T depot at the end of their shift when they missed the edge of one of them. In their motion to adjust, they then felt a sharp pain in their lower back. The incident was not reported straight away, but the pain persisted and a Doctor’s appointment was planned for 31/12/18. The Golden Hour applied for chain of care and this became a lost time injury.

10/12/2018
Raynes Park

Staff Accident
A member of staff from the Inner CRE team was getting PPE out of a Transit van within the Raynes Park Depot when the door slid shut on their right hand little finger.
The Injured Party sustained a cut and some bruising and once a plaster was applied, they were able to continue.
The Golden Hour was applied and there was no lost time.

11/12/2018
Waterloo

Short Circuit
A member staff from Raynes Park Distribution received a report from a Works Delivery Manager at Waterloo that a stack of fishplates had fallen in a ‘daisy chain like fashion’ on platforms 8 and 9.
The fishplates had then come into contact with the negative return and a buffer stop cage and the danger was identified when a gate to the cage sparked when opened.
There were no reported injuries and the incident was Close Called.

12/12/2018
Queenstown Road

Line Blockage Irregularity
A Wimbledon Signaller reported that they had signalled engineering train 6J61 out of T3 possession item 30 and onto the Windsor Reversible at Queenstown Road where an S&T Line Blockage was still in situ.
The Signaller had been dealing with a high workload at the time and fortunately no-one had been trackside when the error occurred.
This incident is subject to further investigation.

12/12/2018
Woking

Staff Accident
A member of staff from Woking Distribution and Plant was driving from Woking Station to the depot when a taxi braked abruptly in front of them and then reversed clipping the Network Rail vehicle.
The D&P staff member sounded their horn, but the taxi Driver did not react to it.
Apart from minor damage to both vehicles, there were no injuries.
The Golden Hour was carried out for chain of care.

13/12/2018
Brockenhurst

Staff Accident
A member of staff from Works Delivery Signals had a Road Traffic Accident involving a deer whilst driving back from the New Forrest in the Brockenhurst area.
Minor damage was sustained to the vehicle, but there were no injuries to the driver.
The Golden Hour was applied for chain of care.
